Summary: Despite the widespread use of internet for health information, patients often do not disclose this information during consultations. This study compared patients' reported internet use with video recordings of consultations, finding that patients skillfully use internet information to support their desired actions. Patients respect and adapt their communication in both interviews and consultations according to their understanding of doctors' and patients' epistemic rights.

Summary: Ethnic inequalities in the experiences and outcomes of severe mental illness are driven by a mix of social determinants of poor health, societal disadvantage, structural racism, and conflicting interactions with care systems. Traditional training and education have limited impact on addressing these issues, therefore clinical processes should focus on systemic influences to improve the cultural precision of care delivery. Clinical ethnography has been shown to be valuable in addressing both systemic and individual care needs for diverse communities.

The study found that despite its apparently high use, there was very low awareness among the interviewees. Summary: Establishing and running remote consultation services present challenges politically, organizationally, economically, technically, relationally, and clinically, with important ethical dimensions. The Planning and Evaluating Remote Consultation Services (PERCS) framework focuses on digital maturity and inclusion efforts, providing guidance on addressing ethical dilemmas and tensions in strategic, allocation, and clinical decisions. Attention to practicalities and emergent narratives, guided by contextual judgement, are necessary for managing these complex challenges beyond standard operating procedures.
