Obstructive sleep apnea (OSA) affects 8-10% of the population, is characterized by chronic intermittent hypoxia (CIH), and causally associates with cardiovascular morbidities. In CIH-exposed mice, closely mimicking the chronicity of human OSA, increased accumulation and proliferation of pro-inflammatory metabolic M1-like macrophages highly expressing CD36, emerged in aorta. Transcriptomic and MeDIP-seq approaches identified activation of pro-atherogenic pathways involving a complex interplay of histone modifications in functionally-relevant biological pathways, such as inflammation and oxidative stress in aorta macrophages. Discontinuation of CIH did not elicit significant improvements in aorta wall macrophage phenotype. However, CIH-induced aorta changes were absent in CD36 knockout mice, Our results provide mechanistic insights showing that CIH exposures during sleep in absence of concurrent pro-atherogenic settings (i.e., genetic propensity or dietary manipulation) lead to the recruitment of CD36(+)(high) macrophages to the aortic wall and trigger atherogenesis. Furthermore, long-term CIH-induced changes may not be reversible with usual OSA treatment.

Sleep disorders in children with asthma

Joel Reiter, Maya Ramagopal, Alex Gileles-Hillel, Erick Forno

Summary: Asthma and sleep disorders are common in children and often coexist, with one potentially influencing the other. Children with asthma may experience complaints of poor sleep quality, insomnia, and daytime sleepiness, while also having higher rates of parasomnias. The bidirectional interaction between sleep-related breathing disorders and asthma may worsen each condition, emphasizing the importance of considering both when managing asthma in children.

Sleep in children with cystic fibrosis: More under the covers

Joel Reiter, Oded Breuer, Malena Cohen-Cymberknoh, Erick Forno, Alex Gileles-Hillel

Summary: Cystic fibrosis (CF) is a chronic multisystem disease with manifestations from birth, involving the entire respiratory system and leading to intestinal malabsorption. Sleep quality and efficiency are often impaired in children with CF, with potential associations between sleep disturbances and respiratory outcomes. Further research is needed to better understand the impact of sleep disorders on pulmonary status in these children.

Neurological and neurodevelopmental symptoms in children with familial Mediterranean fever and their siblings

Oren Biro, Alex Gileles-Hillel, Talia Dor-Wollman, Eli M. Eisenstein, Yackov Berkun

Summary: The study found a higher prevalence of ADHD, learning disabilities, headaches, and febrile seizures in children with FMF. Headaches and febrile seizures were significantly more common in children with FMF compared to their siblings. ADHD and learning disabilities were associated with poor adherence to colchicine treatment. Overall, it highlights the importance of addressing neurodevelopmental aspects in children with FMF for better disease management.

A highly diverse set of novel immunoglobulin-like transcript (NILT) genes in zebrafish indicates a wide range of functions with complex relationships to mammalian receptors

Dustin J. Wcisel, Alex Dornburg, Sean C. McConnell, Kyle M. Hernandez, Jorge Andrade, Jill L. O. de Jong, Gary W. Litman, Jeffrey A. Yoder

Summary: Multiple novel immunoglobulin-like transcripts (NILTs) have been identified in salmon, trout, and carp, but a definitive framework of NILT diversity is still lacking. In this study, the researchers identified five NILT loci in the Atlantic salmon genome, defined 86 NILT Ig domains in zebrafish chromosome 1, and described 41 NILT Ig domains in an alternative haplotype for the same genomic region. They found an unprecedented diversity of Ig domain sequences and combinations for NILTs within a single species. The results suggest that this gene family likely plays multiple roles in host immunity.

Obstructive sleep apnoea is related to melanoma aggressiveness through paraspeckle protein-1 upregulation

Carolina Cubillos-Zapata, Miguel Angel Martinez-Garcia, Elena Diaz-Garcia, Sara Garcia-Tovar, Francisco Campos-Rodriguez, Manuel Sanchez-de-la-Torre, Eduardo Nagore, Antonio Martorell-Calatayud, Luis Hernandez Blasco, Esther Pastor, Jorge Abad-Capa, Josep Maria Montserrat, Valentin Cabriada-Nuno, Irene Cano-Pumarega, Jaime Corral-Penafiel, Eva Arias, Olga Mediano, Maria Somoza-Gonzalez, Joan Dalmau-Arias, Isaac Almendros, Ramon Farre, David Gozal, Francisco Garcia-Rio

Summary: In patients with obstructive sleep apnoea (OSA), high levels of paraspeckle component (PSPC)1 are associated with increased tumor aggressiveness in cutaneous melanoma. Intermittent hypoxia and soluble PSPC1 play a role in promoting epithelial-mesenchymal transition (EMT) and acquisition of cancer stem cell (CSC)-like features in melanoma cells. These findings suggest a potential link between OSA severity, PSPC1 levels, and melanoma aggressiveness.

The clinical yield of bronchoscopy in the management of cystic fibrosis: A retrospective multicenter study

Alex Gileles-Hillel, Limor Yochi Harpaz, Oded Breuer, Joel Reiter, Reuven Tsabari, Eitan Kerem, Malena Cohen-Cymberknoh, Patrick Stafler, Meir Mei-Zahav, Yazeed Toukan, Lea Bentur, David Shoseyov

Summary: This multicenter retrospective analysis suggests that bronchoscopy has a significant impact on treatment decisions for patients with cystic fibrosis, with a considerable proportion of patients experiencing a change in their treatment regimen. There were no serious adverse events reported, indicating that bronchoscopy is a safe procedure. Further studies should explore whether bronchoscopy-guided decision-making can also lead to improvements in clinical outcomes for patients with cystic fibrosis.

Multi-Step Extracellular Matrix Remodelling and Stiffening in the Development of Idiopathic Pulmonary Fibrosis

Constanca Junior, Anna Ulldemolins, Maria Narciso, Isaac Almendros, Ramon Farre, Daniel Navajas, Javier Lopez, Mar Eroles, Felix Rico, Nuria Gavara

Summary: The extracellular matrix (ECM) of the lung, composed mainly of collagens, elastin, and proteoglycans, provides support to lung cells. Aberrant increased deposition of ECM in lung fibrosis changes filament organization and stiffens the ECM, leading to the profibrotic profile of pulmonary cells. The study examines changes in ECM properties during fibrosis development and identifies potential pharmacological targets to hinder its progression.
