Metabolic shift in the emergence of hyperinvasive pandemic meningococcal lineages

Hyperinvasive lineages of Neisseria meningitidis, which persist despite extensive horizontal genetic exchange, are a major cause of meningitis and septicaemia worldwide. Over the past 50 years one such lineage of meningococci, known as serogroup A, clonal complex 5 (A:cc5), has caused three successive pandemics, including epidemics in sub-Saharan Africa. Although the principal antigens that invoke effective immunity have remained unchanged, distinct A: cc5 epidemic clones have nevertheless emerged. An analysis of whole genome sequence diversity among 153 A:cc5 isolates identified eleven genetic introgression events in the emergence of the epidemic clones, which primarily involved variants of core genes encoding metabolic processes. The acquired DNA was identical to that found over many years in other, unrelated, hyperinvasive meningococci, suggesting that the epidemic clones emerged by acquisition of pre-existing metabolic gene variants, rather than 'virulence' associated or antigen-encoding genes. This is consistent with mathematical models which predict the association of transmission fitness with the emergence and maintenance of virulence in recombining commensal organisms.

Summary: The interpretation of whole-genome sequence data in clinical and public health microbiology is crucial, especially for non-specialists. Two protein-based vaccines have been used for the prevention of invasive serotype B meningococcal disease, requiring decisions on vaccine usage to be based on index case isolates. A new MenDeVAR Index provides rapid evidence-based information on meningococcal vaccine antigen variants for practitioners, developed independently of vaccine manufacturers.

Summary: The incidence of invasive meningococcal disease in the UK decreased significantly from 1999 to 2014, particularly in serogroup C and serogroup B diseases. A survey conducted in 2014-15 showed a lower carriage prevalence of meningococci in adolescents compared to previous surveys, with a decline in behaviors associated with carriage.

Summary: This study investigated the evolution and spread of drug-resistant Neisseria gonorrhoeae strains in Japan and globally. The emergence of mosaic penA alleles in ST-1901 and ST-7363 was found to be due to recombination events with other Neisseria species, followed by acquisition of wildtype penA alleles. Furthermore, fluoroquinolone resistance mutations in ST-7363 were found to be due to independent mutations rather than horizontal gene transfer. These findings highlight the role of antibiotic use and genetic recombination in driving the spread of drug-resistant gonorrhea.

Summary: During the early months of the COVID-19 pandemic, there was a significant and sustained reduction in invasive diseases caused by respiratory pathogens such as Streptococcus pneumoniae, Haemophilus influenzae, and Neisseria meningitidis. This reduction was observed in most countries worldwide despite differing stringency in COVID-19 control policies. The introduction of COVID-19 containment measures likely led to a significant decrease in life-threatening invasive diseases caused by these pathogens.
