Summary: Parathyroidectomy (PTX) is an effective treatment for patients with kidney failure and secondary hyperparathyroidism (SHPT). Perioperative hyperkalemia can be a serious complication, but the usefulness of preoperative thresholds for serum potassium levels (SPL) is not clear. This study compared different clinical procedures in preventing or treating perioperative hyperkalemia and found that early postoperative dialysis can be performed with a low risk for complications in patients with postoperative hyperkalemia.

Summary: Overall, the survival of patients with primary hyperparathyroidism (PHPT) in a community population in the United States was not significantly different from that of white Minnesota residents. However, individuals with a maximum serum calcium level > 10.8 mg/dL had an increased risk of mortality. Survival was improved after parathyroidectomy in patients with serum calcium levels >10.8 mg/dL.

Summary: This study compared the risk of mortality between parathyroidectomy (PTx) and cinacalcet treatment in hemodialysis patients. The results showed that PTx resulted in greater reductions in intact PTH, calcium, and phosphorus levels compared with cinacalcet. PTx was associated with a lower risk of mortality, particularly among patients with severe secondary hyperparathyroidism.

Summary: This trial compared oral cinacalcet with total parathyroidectomy (PTx) in dialysis patients with advanced secondary hyperparathyroidism (SHPT). The results showed that both cinacalcet and PTx effectively improved biochemical abnormalities but did not significantly reduce cardiovascular surrogate outcomes or improve health-related quality of life. Cinacalcet may be used as an alternative to PTx for treating advanced SHPT.

Summary: Secondary hyperparathyroidism (SHPT) is a common complication of end-stage kidney disease (ESKD), and hungry bone syndrome (HBS) frequently occurs in patients on maintenance dialysis receiving parathyroidectomy for refractory SHPT. This retrospective study found that lower preoperative serum calcium levels, higher alkaline phosphate levels, and higher potassium levels were associated with the occurrence of HBS after parathyroidectomy. However, there was no significant association between HBS and the persistence, recurrence, or mortality rate of hyperparathyroidism.

Summary: This study aims to examine the incidence, associations, and outcomes of posttransplant parathyroidectomy and calcimimetic use in a cohort of Medicare-insured US kidney transplant recipients. The study found that the rate of posttransplant parathyroidectomy remained stable over time, while the use of calcimimetic increased during the study period. Long dialysis vintage and pre-transplant calcimimetic use were strongly associated with posttransplant parathyroidectomy and calcimimetic use. Roughly 1 in 4 patients were hospitalized within 90 days of posttransplant parathyroidectomy, with hypocalcemia-related diagnoses being the most common complication. Parathyroidectomy was not associated with an increase in acute kidney injury compared to calcimimetic initiation.

Summary: A simple dialysis dementia scoring system was developed and validated to identify individuals at high risk for dementia. The risk score included age and 10 comorbidities as predictors, with high-risk patients having a three-fold increased risk of dementia compared to low-risk patients. The system showed acceptable discrimination and calibration during validation, indicating its potential as a screening tool for high-risk groups in clinical practice.

Summary: Parkinson's disease (PD) and carbon monoxide (CO) poisoning exhibit parkinsonian features due to presynaptic dopaminergic deficits, but their clinical features and treatment responses differ, suggesting involvement of other neurotransmitters. Using PET imaging, this study explored VMAT2 distributions in patients with PD, CO poisoning, and age-matched controls. Results indicated specific reductions in VMAT2 signals in different brain regions for each condition, with correlations to motor symptoms and severity scales in both groups. This research enhances understanding of monoaminergic deficits in CO poisoning and PD patients.

The findings suggest that pregnant women have a moderate but insignificant decrease in mortality risk compared to nonpregnant women. Subgroup analysis shows a significantly increased mortality risk for pregnant women when the injury severity score is similar to nonpregnant women.
