Summary: Fulminant type 1 diabetes is a subtype of type 1 diabetes characterized by rapid destruction of beta cells. It is common in East Asian countries, but rare in Western countries. Factors such as human leukocyte antigen and environmental factors, including viruses and drug-induced hypersensitivity syndrome, may contribute to its development. Further research is needed to understand its etiology and clinical characteristics. Prompt diagnosis and appropriate treatment are crucial due to its life-threatening nature.

Summary: This study evaluated the characteristics of brain lesions in multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) using chi-separation imaging. The findings showed that MS lesions exhibited a higher proportion of hypodiamagnetic lesions compared to NMOSD lesions, suggesting that this feature can be used to differentiate between the two diseases.

Summary: The study aimed to assess the proportion of MM2-type sCJD and describe the clinical features of MM2C and MM2T-type sCJD in a Chinese cohort. Among the 209 patients with sCJD, 24.4% were diagnosed with MM2-type sCJD, with 44 cases of MM2C-type and 7 cases of MM2T-type. The absence of typical sCJD symptoms within 6 months was found in 61.3% of MM2C-type sCJD patients on admission, but all had cortical hyperintensity on DWI. MM2C-type sCJD was associated with slower disease progression and the absence of typical clinical features, while MM2T-type had distinct characteristics such as higher male proportion, earlier age of onset, longer disease duration, and higher incidence of bilateral thalamic hypometabolism/hypoperfusion.

Summary: This study identified more accurate autoantibodies for diagnosing autoimmune hepatitis (AIH) using a protein macroarray, which showed higher specificity and sensitivity compared to conventional autoantibodies. These antibodies could be used as promising markers to improve the diagnostic workup of liver diseases, particularly in cases of seronegative AIH.

Summary: Despite the lack of consensus criteria for the diagnosis of cholangitis, the BILE criteria appears to be a promising approach in identifying high-risk patients. Further validation studies are needed to confirm its diagnostic performance and ability to prevent unnecessary ERCP.

Summary: This study used machine learning to develop a profile of individuals who are likely to be diagnosed with ankylosing spondylitis (AS) in the future. The models showed that lower back pain, uveitis, and NSAID use under age 20 are associated with AS development in men, while women tend to have an older age of symptom presentation, along with back pain and multiple pain relief medications. However, when applied to a general population with low prevalence rates, the positive predictive value of these models would be very low.

Summary: This study found that melanocytic lesions with atypical or asymmetrically distributed peripheral globules, especially when located on the extremities, are more likely to be melanoma; on the contrary, lesions with typical and symmetrically distributed peripheral globules, and with no other concerning dermoscopic features, are unlikely to be malignant.

Summary: Behcet's disease is a chronic inflammatory disorder characterized by oral and genital ulcers, skin lesions, and uveitis. The diagnosis is solely based on clinical features as there is no specific laboratory test. International diagnostic criteria were introduced in 1990, but they have limitations in diagnosing atypical presentations. In 2013, new criteria were introduced to improve sensitivity with minimal compromise on specificity. Efforts should continue to enhance the currently accepted international classification criteria, possibly by incorporating genetic testing and ethnicity-specific features.

Summary: Polycystic ovary syndrome (PCOS) is a complex and heterogeneous disorder that affects women in the reproductive age group. It has features of both reproductive and metabolic disorders. The most widely used criteria for diagnosis are the 2003 Rotterdam criteria, which require the presence of two out of three criteria. Artificial intelligence, specifically machine learning, is being explored as a promising tool for PCOS diagnosis and prediction. This review focuses on the pathophysiology, clinical features, and diagnostic challenges of PCOS in females.

Summary: The study identified specific indicators, including cauda equina disorder (CED) and perimedullary flow voids (PFV), for the diagnosis of spinal dural arteriovenous fistulas (SDAVFs). The diagnostic model constructed using PFV and CED signs showed good diagnostic performance. The 3D-T2-SPACE and CE-MRA sequences had good localization values for SDAVF and combining them improved diagnostic accuracy compared to digital subtraction angiography (DSA).

Summary: This study assesses the diagnostic performance of temporal artery ultrasound and biopsy in the diagnosis of giant cell arteritis (GCA). The results show that both ultrasound and biopsy have relatively high sensitivity and specificity, although the overall diagnostic accuracy of ultrasound is slightly lower than that of biopsy.

Summary: The study demonstrates that both immunofluorescence and RT-QuIC have high diagnostic accuracy in distinguishing synucleinopathies from non-synucleinopathies, with immunofluorescence showing higher diagnostic accuracy and a good level of agreement with RT-QuIC in both skin and CSF.

Summary: This study investigated the clinical significance and antigen specificity of autoantibodies to insulinoma-associated antigen-2 (IA-2A) in Japanese patients with type 1 diabetes. The results showed that IA-2A-ELISA is a reliable marker for the diagnosis of type 1 diabetes and the prediction of future insulin dependency.
