Islet transplantation is a safe and effective procedure; however, it depends on the critical step of isolating high-quality islets from a whole pancreas. Human islet isolation requires considerable experience and expertise and is frequently seen as an art rather than a science. Without scientific knowledge of pancreatic anatomy, however, real experience cannot be gained. In particular, an understanding of ductal anatomy is important to perform human islet isolation. This review is based on clinical experience performing more than 900 human islet isolations over 10 years and aims to highlight the importance of pancreatic anatomy and surgical techniques in islet processing.

Summary: This study highlights the molecular link between the increase in UCP2 expression in beta-cells and beta-cell failure, and suggests that targeting the UCP2/AldB axis may be a promising approach for the recovery of beta-cell function.

Evaluating the Potential for ABO-incompatible Islet Transplantation: Expression of ABH Antigens on Human Pancreata, Isolated Islets, and Embryonic Stem Cell-derived Islets

Kevin Verhoeff, Nerea Cuesta-Gomez, Patrick Albers, Rena Pawlick, Braulio A. Marfil-Garza, Ila Jasra, Nidheesh Dadheech, Doug O'Gorman, Tatsuya Kin, Anne Halpin, Lori J. West, A. M. James Shapiro

Summary: This study evaluated the expression of ABH glycans in human pancreas, isolated islets, and hESC-derived islets. The study found that endocrine and ductal cells in the pancreas do not express ABH antigens, while acinar tissues strongly express them. Clinical pancreatic islet transplantation should remain ABO-matched due to the presence of acinar tissue expressing ABH glycans in islet preparations.

C-peptide Targets and Patient-centered Outcomes of Relevance to Cellular Transplantation for Diabetes

Kevin Verhoeff, Braulio A. Marfil-Garza, Khaled Dajani, David L. Bigam, Blaire Anderson, Tatsuya Kin, Anna Lam, Doug O'Gorman, Peter A. Senior, A. M. James Shapiro

Summary: This study analyzed patients undergoing islet transplantation and found that C-peptide levels are correlated with blood glucose control and insulin independence. The study determined threshold values for insulin independence and hypoglycemic freedom based on C-peptide levels, as well as BETA-2 scores, providing reference values for evaluating pancreatic or stem cell therapies.

A predictive computational platform for optimizing the design of bioartificial pancreas devices

Alexander U. Ernst, Long-Hai Wang, Scott C. Worland, Braulio A. Marfil-Garza, Xi Wang, Wanjun Liu, Alan Chiu, Tatsuya Kin, Doug O'Gorman, Scott Steinschneider, Ashim K. Datta, Klearchos K. Papas, A. M. James Shapiro, Minglin Ma

Summary: This study presents a computational platform for investigating the therapeutic capacity of bioartificial pancreas devices. The platform takes into account stochastic properties like cell size distribution and random localization, and optimizes device structures and estimates cell doses. It also proposes a new islet equivalence conversion table and develops a machine learning model for generating coefficients for user-defined devices.

E2F1 transcription factor mediates a link between fat and islets to promote ? cell proliferation in response to acute insulin resistance

Jun Shirakawa, Yu Togashi, Giorgio Basile, Tomoko Okuyama, Ryota Inoue, Megan Fernandez, Mayu Kyohara, Dario F. De Jesus, Nozomi Goto, Wei Zhang, Takahiro Tsuno, Tatsuya Kin, Hui Pan, Jonathan M. Dreyfuss, A. M. James Shapiro, Peng Yi, Yasuo Terauchi, Rohit N. Kulkarni

Summary: This study investigates the replication pathways of β cells under acute insulin resistance and identifies the involvement of insulin-independent factors CENP-A and transcription factor E2F1 in the multiplication of β cells induced by S961. Inhibition of E2F1 blocks β-cell proliferation in S961-injected mice. Furthermore, adipocytes activate E2F1 to promote β-cell replication.

Pancreas and Islet Transplantation: Comparative Outcome Analysis of a Single-centre Cohort Over 20-years

Braulio A. Marfil-Garza, Joshua Hefler, Kevin Verhoeff, Anna Lam, Khaled Dajani, Blaire Anderson, Doug O'Gorman, Tatsuya Kin, Omar Yaxmehen Bello-Chavolla, Donald Grynoch, Anne Halpin, Patricia M. Campbell, Peter A. Senior, David Bigam, A. M. James Shapiro

Summary: This study provides the largest single-center analysis of islet (ITx) and pancreas (PTx) transplantation. It shows that PTx has higher mortality, procedure-related complications, and readmissions compared to ITx. However, PTx has better insulin independence, graft survival, and glycemic control. This study provides data to improve shared decision-making on the risks and benefits of ITx and PTx.

Efficacy of a Novel Augmented Reality Navigation System Using 3D Computer Graphic Modeling in Endoscopic Transsphenoidal Surgery for Sellar and Parasellar Tumors

Yoshiaki Goto, Ai Kawaguchi, Yuki Inoue, Yuki Nakamura, Yuta Oyama, Arisa Tomioka, Fumi Higuchi, Takeshi Uno, Masaaki Shojima, Taichi Kin, Masahiro Shin

Summary: In endoscopic transsphenoidal skull base surgery, a precise three-dimensional computer graphic model was created from preoperative radiographic data and superimposed on a visual image of the actual surgical field, allowing immediate three-dimensional understanding of the lesion and surrounding structures. The efficacy of this augmented reality (AR) navigation system was evaluated in 15 patients with sellar and parasellar tumors, with an average score of 4.7 indicating its usefulness. In two cases, AR navigation was deemed less useful due to difficulties perceiving the depth of the lesion.

Data-Driven Multi-modal Partial Medical Image Preregistration by Template Space Patch Mapping

Ding Xia, Xi Yang, Oliver van Kaick, Taichi Kin, Takeo Igarashi

Summary: This paper introduces a learning-based method that provides good initialization for partial image registration. By learning the mapping from small patches of the image to positions in the template space and computing a rigid transformation matrix, accurate registration can be achieved. Experimental results show that MSE registration with this method outperforms other baseline methods.

Threshold field painting saves the time for segmentation of minute arteries

Naoyuki Shono, Takeo Igarashi, Taichi Kin, Toki Saito, Nobuhito Saito

Summary: The study tested a segmentation workflow called threshold field painting for segmenting morbid minute arteries, and found that it significantly reduced segmentation time and improved usability scores. The method was more efficient in segmenting normally nonexistent minute arteries, such as MMA feeders, compared to the conventional method.
