Summary: Extracellular chromatin and neutrophil activation play crucial roles in myocardial infarction, with high levels correlating with infarct size, microvascular obstruction, and cardiac function. DNase activity is important for maintaining microvascular patency and improving outcomes in STEMI patients.

Summary: The study found that NSTE-ACS patients with MVD undergoing MV-PCI and CO-PCI had similar mortality rates at long-term follow-up, but the MV-PCI group showed improved event-free survival in terms of fewer coronary reinterventions.

Summary: Patients with non-ST-elevation myocardial infarction (NSTEMI) have more extensive coronary atherosclerotic disease burden, including non-obstruction lesions, which could contribute to their worse long-term outcomes compared to those with ST-elevation myocardial infarction (STEMI).

Summary: This study used a nationwide database to analyze the trends and outcomes of isolated coronary artery bypass grafting after ST-elevation myocardial infarction. The results showed a decline in the use of this procedure, but no change in in-hospital mortality. A recent cohort study found that performing the grafting on day 2 had similar in-hospital mortality compared with day 3 or more, but lower rates of complications and shorter hospital stays.

Summary: Sildenafil citrate is commonly used for erectile dysfunction, but its recreational use without a prescription may lead to cardiovascular issues such as myocardial infarction. Physicians should be cautious of underlying medical conditions when prescribing sildenafil to avoid potential complications.

Summary: There may be no significant benefit for patients with ST-segment elevation myocardial infarction complicating cardiogenic shock treated with multivessel percutaneous coronary intervention compared with culprit vessel-only PCI, and the risk of developing acute renal failure increases.

Summary: This study aimed to evaluate the applicability of T wave inversion in ECG of patients with STEMI as an indicator of early spontaneous reperfusion. The results showed that patients with anterior STEMI and inverted T waves had significantly higher TIMI flow, while this relationship was not seen in non-anterior STEMI.

Summary: In STEMI patients treated with emergency PCI, 73 out of 123 patients developed reperfusion arrhythmias, with 63 cases of reperfusion bradycardia. Factors such as age, infarction area, and vascular blood flow TIMI classification showed significant differences between the RA group and NRA group. Culprit vessel of right coronary artery and grade 0 lesions vascular blood flow TIMI classification were identified as independent risk factors for reperfusion bradycardia in STEMI patients undergoing emergency PCI.

Summary: In hemodynamically stable patients with NSTEMI and MVD, a strategy of multivessel percutaneous coronary intervention (MV-PCI) improves prognosis. For patients with complex coronary anatomy, staged MV-PCI is associated with a lower risk of major adverse cardiac events (MACE) compared to immediate MV-PCI.

Summary: In the United States, approximately 720,000 adults experience myocardial infarction (MI) annually. The 12-lead electrocardiogram (ECG) is crucial for classifying MI. ST-segment elevation on the 12-lead ECG is present in about 30% of MIs and is classified as STEMI that urgently requires intervention. However, the remaining 70% of MIs, called NSTEMI, lack ST-segment elevation and exhibit various changes on the 12-lead ECG. Among NSTEMI cases, 33% actually have occlusion of the culprit artery, leading to similar myocardial damage and worse outcomes compared to NSTEMI without occlusion.

Summary: This study compared the efficacy and safety of ischemia-guided non-culprit vessel angioplasty with routine non-culprit vessel angioplasty in patients with multi-vessel disease presenting with ST elevation myocardial infarction (STEMI). The results showed that ischemia-guided non-culprit PCI resulted in low ischemia burden and was equally effective as routine non-culprit vessel PCI in reducing ischemia burden. This approach may offer cost-savings, particularly in low-resource settings.

Summary: This study evaluated the predictive value of global and regional left ventricular myocardial work for left ventricular remodelling in patients with ST-segment elevation myocardial infarction (STEMI) before discharge. The results showed that the cardiac work index before discharge is independently associated with early adverse left ventricular remodelling in STEMI patients treated with primary percutaneous coronary intervention (PCI) and optimal medical therapy.

Summary: This study compared the characteristics, management, and outcomes of patients with spontaneous coronary artery dissection (SCAD) presenting as ST-segment-elevation myocardial infarction (STEMI) versus non-ST-segment elevation myocardial infarction (NSTEMI). The study found that STEMI patients had worse angiographic profiles, received more percutaneous coronary intervention (PCI), and had higher rates of in-hospital reinfarctions and cardiogenic shock compared to NSTEMI patients. However, the short and long-term prognosis were similar between the two groups.

Summary: Lower cardiorespiratory fitness is associated with higher cardiovascular disease risk in people with rheumatoid arthritis at an increased CVD risk. This study found that in CVD-RA patients, CRF is low and is associated with step count, sedentary time, and moderate-to-vigorous physical activity time, suggesting that improving CRF may help enhance physical activity levels and reduce CVD risk in RA patients.

Summary: This study investigated physiotherapists' clinical experience with a guideline-centered, home-based cardiac rehabilitation protocol for frail older patients. The findings identified three principal themes that influence protocol-adherence and the feasibility of protocol-implementation, including the feasibility of exercise testing, patients' motivation and PTs' motivational techniques, and interdisciplinary collaboration in monitoring patients' risks. Recommendations are needed to optimize home-based cardiac rehabilitation for frail patients.

Summary: The objective of this study was to investigate the safety and potential effects of aerobic exercise on cardiorespiratory fitness, disease activity, and cardiovascular disease risk factors in patients with rheumatoid arthritis and high risk for cardiovascular diseases. The results showed that a 12-week exercise intervention was safe and led to improvements in cardiorespiratory fitness and disease activity in these patients.

Summary: Through a systematic review of 33 articles, the methodological quality and quality of evidence on athlete screening were found to be suboptimal, and the efficacy of screening could not be reliably established. The prevalence of screen detected HRCC was very low and the false positive rate was high. Given the limitations of the evidence, individual recommendations need to be prudent.
