Offline fusion of co-registered intravascular ultrasound and frequency domain optical coherence tomography images for the analysis of human atherosclerotic plaques

Aims: To demonstrate the feasibility and potential usefulness of an offline fusion of matched optical coherence tomography (OCT) and intravascular ultrasound (IVUS)/virtual histology (IVUS-VH) images. Methods and results: A total of nine matched OCT, IVUS, and IVUS-VH cross-sections were fused according to a methodology described in this report. On the basis of the fused images, an OCT-IVUS-VH tissue classification algorithm is proposed taking into account the individual strength of both techniques. Conclusions: Offline fusion of co-registered IVUS and OCT is feasible and combines the strengths of both imaging modalities, potentially improving the diagnostic accuracy of plaque characterisation.

Summary: Intravascular imaging plays an important role in assessing the effectiveness of drug therapies targeting atherosclerosis. Serial intravascular ultrasound, optical coherence tomography, or near-infrared spectroscopy-intravascular ultrasound studies allow for evaluation of the impact of different treatments on plaque burden and morphology. However, established intravascular imaging surrogate endpoints have limited efficacy in predicting the prognostic value of these therapies, necessitating the development of alternative imaging endpoints based on multimodality/hybrid intravascular imaging.

Subclinical leaflet thrombosis and antithrombotic therapy post-TAVI: An LRT substudy

Summary: This study evaluated the impact of antithrombotic regimens on subclinical leaflet thrombosis (SLT) in low-risk patients who underwent transcatheter aortic valve implantation (TAVI). The results showed that oral anticoagulation was associated with significantly lower rates of SLT at 30 days compared to dual antiplatelet therapy (DAPT) or single antiplatelet therapy (SAPT), and there was no apparent benefit of DAPT over SAPT. Additionally, short-term oral anticoagulation did not increase bleeding risk compared to SAPT or DAPT.

Summary: This study demonstrates the feasibility and effectiveness of a novel biorestorative graft in an ovine coronary artery bypass graft model, suggesting its potential as an alternative to saphenous vein grafts. These findings could pave the way for the first-in-man trial of using this new material for coronary artery bypass graft patients who struggle with harvesting SVGs.

Summary: Background estimation of regurgitant fraction using videodensitometry (VD-AR) is a new objective grading tool for paravalvular regurgitation (PVR) after transcatheter aortic valve implantation (TAVI). This study found that VD-AR > 17% was independently associated with 3-year mortality. Compared to subjective visual evaluation, VD-AR resulted in a smaller proportion of patients classified as prognostically relevant for PVR.

Summary: This study compared treatment recommendations based on the SYNTAX score 2020 derived from coronary computed tomography angiography (CCTA) and invasive coronary angiography (ICA). The results showed moderate to substantial agreement between the anatomical SYNTAX scores derived from CCTA and ICA, suggesting that CCTA could be used as an alternative to ICA when making decisions regarding the modality of revascularization.

Impact of periprocedural major adverse events on 10-year mortality after revascularisation

Summary: This study aimed to evaluate the impact of non-fatal periprocedural major adverse events (PMAE) on 10-year mortality and investigate whether PMAE affect mortality predicted by SYNTAX score II 2020 (SSII-2020) and whether optimal medical therapy (OMT) has a positive effect on the prognosis of patients with non-fatal PMAE. The results show that non-fatal PMAE are more common following CABG than PCI, but their prognostic impact is similar, being significant in the first year and then diminishing out to 10 years. Therefore, patients with non-fatal PMAE may require more careful follow-up and additional preventive treatment in the first year post-procedure.
