Summary: This study aimed to evaluate the predictive value of hemodynamic features for stroke relapse in patients with intracranial vertebrobasilar atherosclerotic stenosis treated with PTAS using q-DSA. The results showed that preprocedural prolonged time to peak in the target vessel and shortened trans-stenotic time to peak difference were associated with stroke relapse. Hemodynamic parameters defined by q-DSA provided incremental predictive value for stroke recurrence.

Summary: Arterial pulse waves (PWs), such as blood pressure and photoplethysmogram (PPG) signals, are rich sources of information for assessing vascular age and identifying individuals at elevated cardiovascular risk. This review explores the possibilities and limitations of reduced-order biophysical models and theoretical and empirical methods for analyzing PW signals and extracting clinically relevant information. Detailed mathematical derivations are provided, demonstrating the relationships between these models and methods. Suggestions for future research are also outlined to fully utilize the potential of modeling and analyzing PW signals for accurate vascular age assessment.

Summary: Artery-to-artery embolism (AAE) is associated with a high risk of recurrent stroke in symptomatic intracranial atherosclerotic disease (ICAD). This study aimed to investigate the cerebral hemodynamic features associated with AAE in ICAD. Computational fluid dynamics models were used to simulate blood flow across ICAD lesions, and the translesional pressure ratio and wall shear stress ratio were calculated. The results showed that high wall shear stress ratio was independently associated with AAE. Excessively elevated wall shear stress in ICAD may increase the risk of AAE.

Summary: This article presents a numerical model for simulating and predicting blood flow dynamics in diseased arterial vessels. The model utilizes a one-dimensional hyperbolic system of quasilinear partial differential equations, incorporating a diagnostic disease descriptor (k(D)). The results demonstrate that disease conditions induce abnormal flow in the artery, increasing blood pressure, flow, and distensibility.

Summary: Despite advances in modern medicine, atherosclerosis remains a significant medical and social problem leading to cardiovascular diseases with high disability and mortality rates. The development of atherosclerosis is linked to lipid accumulation in arterial walls and disrupted inflammation regulation mechanisms, with fatty acids playing a key role in inflammation processes. Endothelial cells are crucial in vascular biology, demonstrating connections between lipid metabolism, innate immunity, hemodynamics, and other vascular cell functions.

Summary: The characteristics of plaque in the middle cerebral artery (MCA) and hemodynamics may help predict the occurrence of ischemic stroke and neurological impairment. Intraplaque hemorrhage (IPH) and anterograde score (AnS) are independent predictors for stroke occurrence. Tmax > 6.0-s volume, IPH, hypertension, and AnS are associated with neurological impairment in patients.

Summary: This study aims to identify multimodal imaging predictors of high-risk patients with ICAD refractory to medical treatment and explore a refined risk stratification model based on these predictors. With 400 symptomatic patients with ICAD enrolled, multimodal imaging examination was conducted and aggressive medical management was implemented. The primary outcome is ischemic stroke or death attributable to the lesion of the target vessel within 1 year.

Summary: This study investigated the hemodynamic outcomes of transcatheter interventions in patients with Williams and Alagille syndromes using simulation models. The results showed that extensive interventions were effective in reducing PA pressures and right ventricular stroke work, while transcatheter therapy was less effective for long-segment stenosis. Regardless of the strategy chosen, a virtual treatment planning platform could identify critical lesions for optimizing right ventricular afterload.

Summary: In patients with tetralogy of Fallot and major aortopulmonary collaterals (MAPCAs), the pulmonary blood supply is highly variable. Our study analyzed serial lung perfusion scintigraphy (LPS) and found that pulmonary flow distribution prior to discharge was balanced, and there was minimal change over time. However, there was considerable patient-to-patient variation in both metrics.

Summary: The acute hemodynamic effect of vasopressin on the Fontan circulation was investigated in this study. The results showed that vasopressin administration significantly increased systolic blood pressure, systemic vascular resistance, and left atrial pressure, while decreasing pulmonary vascular resistance. However, it had no effect on cardiac index or pulmonary artery pressure. These findings suggest that vasopressin may be an option for treating systemic hypotension during sedation or general anesthesia in Fontan patients.
