Summary: The results of recent RCTs were confirmed in this real-world study showing that drains are not necessary for standard pancreatoduodenectomy and distal pancreatectomy. Omission of drains led to a reduction in complications and shorter hospital stay.

Summary: Spontaneous internal pancreatic fistula is a rare and complex complication of acute pancreatitis, more common in males, alcoholics, severe AP, and infected necrosis. Diagnosis requires a high index of suspicion, with treatment strategies including medical and nonsurgical interventions, and some cases may require surgical intervention.

Summary: Based on a systematic review of 13 studies, it was found that EUS-guided pancreatic duct intervention showed superior outcomes compared to ERP-guided intervention in terms of pancreatic duct opacification, cannulation success, stent placement, and possibly clinical outcomes such as pain resolution. However, due to limited data, the adverse event rates between the two treatment modalities could not be accurately compared.

Summary: The aim of this study was to investigate the association between preoperative biliary drainage (PBD) and morbidity following cephalic pancreaticoduodenectomy (CPD) for pancreatic ductal adenocarcinoma (PDAC) as well as its prognostic impact. A retrospective study was conducted involving 128 patients, who were divided into two groups: those who underwent PBD (group 1) and those who did not (group 2). The results showed that PBD was associated with increased bacterial colonization of the bile, but did not significantly increase postoperative complications or influence overall survival and disease-free survival.

Summary: This study found that H2O2-assisted endoscopic necrosectomy had a higher clinical success rate and a shorter time to resolution with equivalent complication rates relative to standard necrosectomy.

Summary: Prophylactic intra-peritoneal drainage after pancreatic surgery may benefit high-risk PD patients but may not provide clear benefits for DP patients. Drain placement in PD patients showed lower mortality but higher rates of CR-POPF, while in DP patients, it increased CR-POPF, wound infection, and readmission rates.

Summary: Postoperative pancreatic fistula (POPF) is a severe complication following pancreatic surgery, and its pathophysiology is poorly understood. Recent studies have shown that postoperative or post-pancreatectomy acute pancreatitis (PPAP) plays a critical role in the development of POPF.

Summary: The study findings suggest that NAC may improve survival rates, and PBD should be reserved for patients with jaundice requiring NAC. It is important to note that the use of PBD is not always necessary and may lead to postoperative complications and reinterventions.

Summary: The use of drains in pancreatic surgery is controversial, and this survey reveals diverse habits, experiences, and opinions among surgeons. Some surgeons believe that drains can reduce the risk of complications such as pancreatic fistula and facilitate early detection and management, while others are concerned about the increased risk of infection and other complications associated with drains. The management of drains also varies widely.

Summary: This study aimed to compare the rates of treatment for post-operative exocrine pancreatic insufficiency (EPI) and diabetes mellitus (DM) in patients with chronic pancreatitis who underwent Duodenal Preserving Pancreatic Head Resections (DPPHR) and Pancreaticoduodenectomy (PD). The results showed that the DPPHR group had a lower rate of new persistent treatment for EPI post-operatively compared to the PD group. There was no difference in the rate of new onset DM between the two groups. Additionally, both groups had similar efficacy in terms of pain control.

Summary: In this study, we described the post-operative complications, pancreatic sequelae, and post-surgical progression/relapse of PNENs in MEN1 patients. Our results showed that a more conservative intervention was associated with the absence of post-surgical complications, but required further pancreas surgeries for tumor relapse in 40.0% of the operated cases.

Summary: This study retrospectively analyzed 450 cases of robot-assisted pancreaticoduodenectomy to describe the experience and learning curve, with improvements in operative time and estimated blood loss observed over time. The inflexion points in the RPD learning curve were identified around cases 100 and 250, and a significantly lower incidence of pancreatic leak was found in the last 350 cases compared to the first 100 cases.

Summary: This study evaluated the cost-effectiveness of open pancreaticoduodenectomy (OPD) versus OPD combined with TRIANGLE operation (OPD-TRIANGLE) for patients with pancreatic cancer. The results showed that OPD-TRIANGLE yielded additional 0.0402 QALYs at an incremental cost of US$1501.83 compared with OPD, and the corresponding ICER was US$37,358.96 per QALY.

Summary: This study surveyed medical oncologists in the Deep South to understand attitudes and practice patterns regarding resectability and referral patterns for colorectal liver metastases. The results showed wide variations in perceptions of resectability and referral patterns among medical oncologists, indicating a need for wider dissemination of resectability criteria and more involvement of liver surgeons in the management of patients with colorectal liver metastases.
