Summary: Liver transplantation provides the best survival benefit for hepatocellular carcinoma (HCC) patients who cannot undergo resection. The Milan criteria have been developed to select suitable candidates for transplantation, aiming to improve survival and reduce the risk of HCC recurrence. However, up to 20% of cases experience HCC recurrence after transplantation, posing a significant challenge due to poor prognosis. Various extended criteria have been proposed to accommodate the increasing demand for organs and the associated higher risk of recurrence. Radiologists need to be aware of the possibility of HCC recurrence involving multiple organs after transplantation. Understanding the location and radiologic appearance of recurrent HCC is crucial for choosing the most appropriate therapy.

Summary: The diagnostic performance of LI-RADS v2018 on gadoxetic acid-enhanced MRI was evaluated in this study for the detection of hepatocellular carcinoma (HCC). Liver transplant candidates (LT group) were compared with patients who underwent surgical resection (SR group). It was found that LI-RADS v2018 may be insufficient in liver transplant candidates, and other diagnostic imaging tests should be considered in patients with smaller lesion size, larger lesion number, and higher Child-Pugh score.

Summary: Hepatocellular carcinoma (HCC) mainly results from liver cirrhosis and its genetic predisposition is believed to be rare. In 334 HCC patients considered for liver transplantation, we found only 7 carriers of pathogenic variants in established cancer-predisposition genes. However, two MRN complex genes (NBN and RAD50) were significantly more frequent among patients. Therefore, HCC's genetic predisposition is rare, but germline testing could be considered in liver transplant recipients for tailored follow-up or targeted therapy.

It is expected that liver biopsy will regain its position in HCC, and combining it with other available methods could improve treatment selection and outcomes.

Summary: The study aimed to determine the relevance and relationship of PIEZO1 and TRPV4 channels in the contractile tone of the hepatic portal vein, which experiences mechanical and osmotic variations as it delivers blood to the liver. Results showed that activation of PIEZO1 leads to vessel relaxation while activation of TRPV4 causes contraction. The modulation of these channels could provide important opportunities for manipulating liver perfusion and regeneration.

The results indicate that survival rates are lower for patients with cholangiocarcinoma compared to usual liver transplant outcomes. However, early-stage cholangiocarcinoma has similar survival to hepatocellular carcinoma, and intrahepatic cholangiocarcinomas have better survival compared to perihilar cholangiocarcinomas.
