Background In cirrhotic patients with hepatocellular carcinoma (HCC), poor differentiation in pre-liver transplantation (LT) biopsy of the largest tumour is used as a criterion for exclusion from LT in some centres. The potential role of pre-LT biopsy at one centre was explored. Methods A prospective database of patients undergoing orthotopic LT for radiologically diagnosed HCC at St James's University Hospital, Leeds during 20062011 was analysed. Results A total of 60 predominantly male (85.0%) patients with viral hepatitis were identified. There were discrepancies between radiological and histopathological findings with respect to the number of tumours identified (in 27 patients, 45.0%) and their size (in 63 tumours, 64.3%). In four (6.7%) patients, the largest lesion, which would theoretically have been targeted for biopsy, was not the largest in the explant. Nine (31.0%) patients with multifocal HCC had tumours of differing grades. In two (6.9%) patients, the largest tumour was well differentiated, but smaller tumours in the explant were poorly differentiated. In one patient, the largest lesion was benign and smaller invasive tumours were confirmed histologically. Conclusions The need to optimize selection for LT in HCC remains. In the present series, the largest tumour was not always representative of overall tumour burden or biological aggression and its potential use to exclude patients from LT is questionable.

Predictors of patient and graft survival following pediatric liver transplantation: Long-term analysis of more than 300 cases from single centre

Summary: Pre-transplant invasive ventilation is a key factor predicting patient survival in pediatric liver transplant recipients. Patients transplanted after 2005 have significantly higher patient survival. Despite the traditional view of worse outcomes for smaller PLT candidates, this study suggests that this viewpoint should be changed.

Summary: The post-operative alanine aminotransferase (ALT) levels do not have an impact on survival outcomes after hepatectomy for colorectal liver metastases. They are also not associated with the length of stay or overall complications.

United Kingdom criteria for liver transplantation in the setting of isolated unresectable colorectal liver metastases

Jamie Murphy, Raj Prasad, Krishna Menon

Summary: This study aims to outline the proposed UK selection criteria and follow-up process for liver transplantation in patients with colorectal cancer liver metastases. The study provides comprehensive inclusion/exclusion criteria and explains the referral framework. Pretransplant assessment criteria and posttransplant oncology-specific outcome measures are also described. The service is anticipated to begin in December 2022, with educational events planned for 2023 to raise awareness of this newly accepted indication for transplantation in the UK.

Independent endothelial functions of PIEZO1 and TRPV4 in hepatic portal vein and predominance of PIEZO1 in mechanical and osmotic stress

Summary: The study aimed to determine the relevance and relationship of PIEZO1 and TRPV4 channels in the contractile tone of the hepatic portal vein, which experiences mechanical and osmotic variations as it delivers blood to the liver. Results showed that activation of PIEZO1 leads to vessel relaxation while activation of TRPV4 causes contraction. The modulation of these channels could provide important opportunities for manipulating liver perfusion and regeneration.

Summary: This study assesses the factors associated with recurrence-free survival (RFS), overall survival (OS), and morbidity and mortality following gallbladder cancer surgery in high income countries (HIC) and low and middle income countries (LMIC). The study found that liver resection did not conclusively improve survival, while extended resections were associated with greater morbidity and mortality. International collaborations are needed to develop evidence-based neoadjuvant and adjuvant treatment strategies to minimize surgical morbidity and prioritize prognostic benefit.

Outcomes After Liver Transplantation With Incidental Cholangiocarcinoma

Nawaz Z. Safdar, Abdul R. Hakeem, Rosemary Faulkes, Fiona James, Lisa Mason, Steven Masson, James Powell, Ian Rowe, Shishir Shetty, Rebecca Jones, Harry V. M. Spiers, Neil Halliday, Jack Baker, Douglas Thorburn, Raj Prasad, Richard Parker

Summary: This study examines the outcomes of liver transplant recipients with incidental cholangiocarcinoma in the United Kingdom. The results indicate that survival rates are lower for patients with cholangiocarcinoma compared to usual liver transplant outcomes. However, early-stage cholangiocarcinoma has similar survival to hepatocellular carcinoma, and intrahepatic cholangiocarcinomas have better survival compared to perihilar cholangiocarcinomas.
