Summary: This study identified risk factors for clinically relevant postoperative pancreatic fistula (CR-POPF) after pancreaticoduodenectomy, with BMI, serum globulin level, pancreatic duct diameter, and postoperative day 3 cumulative fluid balance per body weight identified as independent risk factors for CR-POPF. Postoperative fluid balance was highlighted as a modifiable risk factor to reduce CRPOPF, especially in high-risk patients.

Summary: This study compared six risk adjusted POPF calculators and found that, except for ua-FRS and updated Birmingham, the observed versus predicted rate of POPF was near zero, yielding comparable results. Benchmarks for POPF should prescribe which risk calculators are used to provide consistency in outcome reporting and robust audit processes.

Summary: This study describes a simple surgical procedure termed redo pancreaticojejunal (PJ) anastomosis for the treatment of late-onset complete pancreaticocutaneous fistula (Lc-PF) after pancreaticoduodenectomy (PD). The procedure was performed in six consecutive patients and showed to be a safe and feasible treatment option with acceptable short- and long-term clinical outcomes. It has the potential to become the preferred treatment strategy for Lc-PF after PD.

The inflexion points in the RPD learning curve were identified around cases 100 and 250, and a significantly lower incidence of pancreatic leak was found in the last 350 cases compared to the first 100 cases.

Summary: The systematic literature review and meta-analysis suggest that prophylactic administration of corticosteroid agents and NAC can reduce the severity of pancreatitis as indicated by histopathologic markers, serum amylase and IL-6 levels, with corticosteroid agents showing a net effect in favor of reducing markers of severity of pancreatitis and NAC showing a significant reduction in severity of amylase and necrosis.
