Is preoperative histological diagnosis necessary for cholangiocarcinoma?

Surgery is currently the only curative treatment for patients with cholangiocarcinoma (CCA). Whether histological diagnosis of CCA is necessary before surgery is controversial. Fifteen percent of patients with suspected biliary malignancy who undergo surgery are found to have benign disease. Surgery is a major procedure with significant morbidity and mortality and alternative treatment is available for those known to have benign stenoses. The aim of this review was to determine whether any of the current diagnostic tests have sufficient sensitivity and specificity to identify patients with benign and malignant bile duct stenoses. A literature search was performed until July 2007 to obtain information from studies published in the previous 10 years. Only studies reporting an appropriate reference test (confirmation of malignancy by biopsy, confirmation of benign nature by histology following surgical excision, or at least 6 months of follow-up for all patients) were included for review. The diagnostic odds ratio was used to measure diagnostic performance. Forty-one references of 34 studies were included in this review. None of the studies used differential verification. Six studies used blinding of assessor. None of the diagnostic tests had sufficient diagnostic accuracy to reliably separate patients with benign from malignant biliary strictures. Differentiating benign from malignant bile strictures is an important aim. There is no trial evidence demonstrating benefit in obtaining a preoperative histological diagnosis of CCA. New methods are required for stricture assessment.

Summary: This study examined the feasibility and efficacy of using cardiac output evaluation to optimize fluid administration in patients with acute pancreatitis (AP) in a general surgery ward. The results showed that ward-based GDFT is feasible and may have potential efficacy, but further large multi-site randomized controlled trials (RCTs) are needed to confirm its clinical and cost effectiveness.

Benefits and harms of interventions to improve anxiety, depression, and other mental health outcomes for autistic people: A systematic review and network meta-analysis of randomised controlled trials

Summary: Mental health difficulties are prevalent in autistic individuals, and finding interventions to improve their mental health is a top priority. However, there is currently a lack of high-quality network meta-analysis on the benefits and harms of different interventions. Available evidence suggests that cognitive behavioural therapy may decrease anxiety and depression in autistic individuals, mindfulness therapy may decrease anxiety and depression in autistic adults with previous mental health conditions, and behavioural interventions may provide some benefit for depression in autistic children. Further research is needed to obtain more high-quality evidence. Autistic individuals should have access to mental health interventions available to non-autistic individuals, following person-centred care principles.

The study identified eleven clinical research questions and provided 28 recommendations with an agreement rate of >= 80%.

Thresholds for surfactant use in preterm neonates: a network meta-analysis

Summary: This study conducted a network meta-analysis of different surfactant treatment strategies for respiratory distress syndrome (RDS) and found no clear benefit of surfactant treatment at any threshold of fraction of inspired oxygen (FiO(2)). The use of surfactant at a threshold of 60% FiO(2) may increase the risk of complications. More randomized trials are urgently needed to provide robust evidence for surfactant delivery at different thresholds.

Summary: The incidence of renal tumours is increasing and anatomic imaging cannot reliably distinguish benign tumours from renal cell carcinoma. Tc-99m-sestamibi SPECT/CT has shown high diagnostic accuracy for benign renal oncocytomas and other oncocytic renal neoplasms of low malignant potential in single-centre studies. The primary aim of MULTI-MIBI is to assess feasibility of a multicentre study of Tc-99m-sestamibi SPECT/CT against a reference standard of histopathology from surgical resection or biopsy.

Summary: In this study, a deep hashing network-based method for the registration of laparoscopic ultrasound (LUS) with computed tomography (CT) is proposed. By learning hash codes of vessel images, the registration success rate can be significantly improved. This approach is easily generalizable to other registration problems and does not require annotated data for training.

Large scale simulation of labeled intraoperative scenes in unity

Summary: This study demonstrates the use of synthetic data generated with Unity game engine for training a liver segmentation model in image guided surgery. The results show that synthetic data outperforms real data in some cases, although the overall performance is slightly lower compared to models trained on real data only.
