Summary: This study compared the first-attempt success rate and complications of sedated tracheal intubation versus tracheal intubation under general anesthesia in children with difficult airways. The results showed that sedation and general anesthesia had a similar rate of first-attempt success. However, 27.6% of the sedation cases needed to be converted to general anesthesia to complete tracheal intubation. The overall rate of complications was similar between the groups, and the rate of severe complications was low.

Summary: A model based on intubation-related variables was developed to classify the difficulty of videolaryngoscopic tracheal intubation. The model was validated and found to have good discrimination ability in predicting difficult airway alerts after videolaryngoscopy, outperforming the Cormack-Lehane classification. The findings of this study provide a useful tool for assessing the probability of difficult airway management.

Summary: This study compared videolaryngoscopes with direct laryngoscopes in simulated cardiopulmonary resuscitation scenarios. Videolaryngoscopes showed shorter intubation times in normal airway scenarios, with VividTrac outperforming in difficult airway scenarios. However, Macintosh laryngoscopes demonstrated better glottis views and less esophageal intubation.

Summary: The use of initial broad-spectrum antibiotics in critically ill neonates requiring mechanical intubation for respiratory failure has not been well studied. This study compared the outcomes of neonates receiving ampicillin plus cefotaxime with those receiving ampicillin plus gentamicin. The use of ampicillin plus cefotaxime was associated with worse outcomes, including a higher risk of infections caused by multidrug-resistant pathogens and increased mortality.

Summary: Handling a difficult airway situation is crucial in neonatal care, as failure to secure ventilation can lead to serious complications such as bradycardia and cardiac arrest. While guidelines exist in the UK and Scotland, Norway lacks a national guideline for managing the unanticipated difficult airway in the delivery room and neonatal intensive care unit. Anesthesiology experience shows that a well-defined algorithm, advance planning, and routine practicing can prepare the resuscitation team to respond adequately to the challenges of a difficult airway situation.

Summary: This review discusses the epidemiology, microbiology, diagnosis, and treatment of MDRSE infection, highlighting the challenges in distinguishing colonization from bloodstream infections caused by S. epidermidis. Various studies have identified the prevalence of methicillin resistance and investigated the phylogenetic lineages and resistant genes of MDRSE through culture, mass spectrometry, and genomic analysis. Vancomycin is recommended as the empiric parenteral therapy, and other treatment options include teicoplanin, daptomycin, oxazolidinones, long-acting lipoglycopeptides, and ceftaroline.

The authors present a novel technique for retrograde endotracheal oral-to-nasal conversion using a standard endotracheal tube exchange catheter, providing an alternative method for difficult airways where traditional methods are not effective.
