Summary: This study evaluated off-label prescribing in first-time users of ADHD medication among children and adolescents in Germany, finding that approximately 13% of pediatric first-time users received off-label pharmacotherapy, with prescribing ADHD medication without a confirmed ADHD diagnosis being the most common off-label criterion.

Summary: This study examined the prevalence of potentially inappropriate medication (PIM) use among older people in Finland using the Meds75+ database and compared it with eight other criteria. The annual prevalence of PIM use ranged from 10.7% to 57.0% depending on the criteria used. The results indicate that different criteria emphasize different medicine classes and clinicians should consider this when applying PIM criteria.

Summary: The study found that nurse practitioners (NPs) prescribed fewer initial PIMs and were less likely to refill a PIM after an outpatient visit than physicians. The odds of receiving PIMs during an NP visit varied by age, race/ethnicity, rurality, and number of comorbidities.

Summary: In the context of improved medication management of older patients, the study evaluated the overprescription of potentially inappropriate drugs (alpha 1-blockers and 5 alpha-reductase inhibitors) for benign prostate hyperplasia (BPH). The results showed that approximately 72% of older individuals received potentially inappropriate prescriptions, mainly involving alpha 1-blockers. These prescriptions were associated with a higher frequency of falls.

Summary: In this diagnostic study, the evaluated PIM/PPO sets had poor to fair performance as diagnostic tools to identify inadequate drug treatment, suggesting the need for reconsideration of using PIMs/PPOs as indicators of drug treatment quality.

Summary: Hyperpolypharmacy is common among older people with advanced CKD, with significant international differences in the number of prescribed medications. Practice variation may represent a lack of consensus regarding appropriate prescribing for this high-risk group for whom pharmacological treatment has great potential for harm as well as benefit.

Summary: This study developed prediction models for readmissions based on routine data, specifically focusing on potentially inappropriate prescribing (PIP). The results showed that PIP effectively predicted readmissions for most diseases, suggesting the possibility for interventions to improve modifiable risk factors.

Summary: From 2000 to 2015, there was a decrease in the use of PIM among older people with and without dementia. Despite an increase in comorbidity, PIM use remained more widespread in people with dementia.

Summary: This study investigated the occurrence and risk factors of potentially inappropriate medications (PIM) in older diabetic kidney disease (DKD) patients during hospitalization, and explored the association with polypharmacy. The results showed that PIM was common during hospitalization and that polypharmacy was an independent risk factor for PIM.

Summary: This study aimed to investigate the prevalence of potentially inappropriate medication (PIM) use among underserved older Latino adults. The results showed that one-third of the participants had at least one use of PIM, over half of the sample had polypharmacy, and 46% of the sample took drugs to be used with caution. Regression analysis revealed that PIM use was associated with the number of prescription medications, chronic conditions, sleep difficulty, lack of access to primary care, financial strain, and poor self-rated health.

Summary: This study aimed to identify medication-related problems in primary care patients over 65 years old. The results showed a high prevalence of MRPs, with age and number of drugs being major factors associated with these issues.

Summary: This study evaluated changes in prescribing, potentially inappropriate prescriptions, and prescribing of low-value medicines in older people with multimorbidity and significant polypharmacy. The results showed reductions in the prescription of most drug groups, with the largest reduction observed in antiplatelet prescriptions. The median number of medication changes per person was five, highlighting the complexity of prescribing for patients with polypharmacy. Frequent medication changes could have important implications for patients in terms of adherence and medication safety.

Summary: This study investigates the impact of disability severity level on polypharmacy and the prescription of potentially inappropriate medication (PIM) using health insurance and long-term care insurance claim data. The results show that polypharmacy is more prevalent in older individuals, especially those with mild disability levels, while PIM prescription is more common in older individuals with higher care levels.

Summary: Comparing the rates of inappropriate prescribing between nurse practitioners and primary care physicians, it was found that they were virtually identical. Although nurse practitioners were overrepresented among clinicians with the highest and lowest rates of inappropriate prescribing, discrepancies in rates tended to be larger across states rather than between these practitioners within states.

Summary: Potentially inappropriate prescribing is common among older PLHIV. Explicit conventional tools may need adaptation to meet the needs of PLHIV. Implicit tools may be more valid but are time-consuming to use and complex to standardize.
