Summary: This study investigates the risk factors and optimal amount of intraoperative irrigation for preventing postoperative surgical site infection (SSI) following lumbar spine surgery. The results suggest that insufficient irrigation volume and diabetes are both risk factors for postoperative SSI. In order to reduce the risk of SSI, it is recommended to use more than 1400 mL/h of normal saline for intraoperative wound irrigation.

Summary: This study analyzed the incidence and characteristics of postoperative surgical site infection (SSI) after posterior thor-acolumbar and lumbar instrumentation. The results showed an average SSI rate of 1.8%. Most patients with deep wound SSI can be successfully treated with retention of implants, but early diagnosis, positive revision surgery, strict irrigation, thorough debridement, and long-term antibiotic treatment are crucial.

Summary: This study aimed to investigate the relationship between abdominal aortic calcification (AAC) and the severity of postoperative complications after major hepatobiliary pancreatic (HBP) surgery. The results showed that higher AAC was associated with increased incidence rates of surgical site infections and a higher Comprehensive Complication Index (CCI). Therefore, careful anastomosis and infection control are necessary to prevent the progression of severe postoperative complications among patients with high AAC.

Summary: A meta-analysis was conducted to evaluate risk factors for surgical site wound infections (SSWIs) in hepatic and pancreatic tumour resection (HPTR). A total of 2349 interrelated investigations were reviewed, and 9 investigations with 22,774 individuals were included. The results showed that HT patients with biliary reconstruction had significantly higher SSWI risk, while there was no significant difference in SSWI between PT individuals who underwent different types of surgeries. However, caution should be exercised due to the limited number of selected investigations for this meta-analysis.

Summary: Anaesthetic agents have financial and environmental impacts, and this study aimed to quantify the impacts of different agents used in surgery at an Australian university teaching hospital. The volume of different agents purchased between 2010 and 2020 was analyzed in terms of financial and environmental impact. Replacing the most environmentally damaging and expensive agent with alternatives could have significant financial and environmental benefits. For example, replacing desflurane with low flow sevoflurane could save greenhouse gas emissions equivalent to driving over 1.4 million kilometers in an average petrol car.

The study found that prolonged apnea events and other physiological abnormalities were frequently detected on the first postoperative night, but these abnormalities were not associated with postoperative pulmonary complications (PPCs). Future research should investigate the significance of these detected aberrations.

Summary: Hepatocellular carcinoma (HCC) is a highly aggressive primary liver cancer and the third leading cause of cancer-related death worldwide. Recent years have witnessed an increasing number of treatment options for HCC, including both curative and non-curative therapies. However, these advancements have also posed challenges in disease monitoring, particularly in the early detection of HCC recurrence. This review will discuss and evaluate the current serological and radiological strategies for post-treatment monitoring and prognostication, as well as their limitations.

Summary: In a retrospective study of patients undergoing complex hepato-pancreato-biliary (HPB) surgery, the use of a multimodal intrathecal analgesia (MITA) technique combining intrathecal morphine, clonidine, and bupivacaine was found to result in reduced postoperative opioid use and better analgesia compared to the use of intrathecal morphine alone. However, patients in the MITA group had a higher incidence of postoperative hypotension requiring vasopressor support.
