Summary: ERCP is an advanced endoscopic technique used for diagnosing and treating biliary and pancreatic diseases. Training programs need to ensure trainees have sufficient knowledge of anatomy and physiology, and incorporate models for simulation training. Competency assessment is crucial, and postgraduate training is necessary due to the rapid advancement of methods.

Summary: This study evaluated the incidence and severity of adverse events (AEs) in biliary endoscopic retrograde cholangiopancreatography (ERCP), and identified risk factors and preventive measures for post-ERCP pancreatitis (PEP). The overall incidence of AEs was 10.1%, with a mortality rate of 0.08%. Prophylactic pancreatic stenting and epinephrine spraying were found to prevent PEP effectively.

Summary: The effectiveness of low-dose rectal diclofenac in preventing post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) was investigated in this study. The results showed that low-dose rectal diclofenac did not reduce the incidence and severity of PEP.

Summary: This study retrospectively investigated the success and adverse event rates of endoscopic therapy for symptomatic HJAS after PD. The results showed a moderate technical success rate and a high recurrence rate. Future studies should optimize treatment protocols and compare different treatment methods.

Summary: Endoscopic management for perihilar cholangiocarcinoma is evolving to improve diagnosis and drainage. Different imaging methods are used to determine resectability and optimal surgical planning, while new diagnostic imaging methods such as video peroral cholangioscopy and probe-based confocal laser endomicroscopy are being clinically applied. The approach to preoperative drainage has shifted to endoscopic nasobiliary drainage to reduce the risk of metastasis seeding. Various endoscopic techniques, including the use of self-expandable metallic stents and endoscopic ultrasonography-guided approach, are employed depending on the resectability and prognosis.

Summary: Gastrointestinal endoscopy plays a crucial role in pediatric gastroenterology, with increasing interest in procedures like ERCP and EUS for hepatobiliary and pancreatic diseases. Due to the differences between pediatric and adult endoscopy, specific research and training are necessary for the development of procedures tailored to children.

Summary: This article describes a rare case of biloma as a complication of endoscopic retrograde cholangiopancreatography (ERCP). The patient experienced right upper quadrant discomfort after the procedure and imaging confirmed the presence of biloma. The article highlights the importance of considering biliary tree disruption in patients with right upper quadrant discomfort following an iatrogenic or traumatic event, and suggests that a combination of radiological imaging and minimal invasive techniques can be successful in managing biloma.

Summary: This study evaluated the feasibility, efficacy and safety of endoscopic internalization by cutting an ENGBD tube for acute cholecystitis. The results showed a high success rate and no significant complications, indicating that it can be an effective treatment option for preventing recurrent cholecystitis during the waiting period for cholecystectomy.
