Summary: Through the analysis of 13 studies, it was found that transanal endoscopic microsurgery and radical surgery adhering to total mesorectal excision have similar safety and treatment outcomes in the treatment of early-stage rectal cancer. However, significant differences were observed in perioperative mortality, local recurrence, overall survival, disease-free survival, temporary stoma, permanent stoma, postoperative complications, rectal pain, operation time, blood loss, and time of hospitalization.

Summary: A meta-analysis of 12 studies involving 3526 patients showed that compared to radical surgery (RS), transanal endoscopic microsurgery (TEM) has advantages in terms of operative time, intraoperative blood loss, perioperative mortality, and postoperative surgical complications. However, TEM is associated with higher risks of positive margin, local recurrence, and overall recurrence.

Summary: A circumferential extent of mucosal defect >3/4 was identified as the only independent risk factor for stenosis in treating rectal adenoma and early cancer with TEM. Incision of the stenosis ring using the TEM platform is an effective strategy for treating stenosis.

Summary: This study evaluated the efficacy of endoscopic submucosal dissection (ESD) and transanal endoscopic microsurgery (TEM) in the treatment of rectal neuroendocrine tumors (NETs) ≤ 2 cm. The results showed that although the complete resection rate was higher in the TEM group than in the ESD group, there was no difference in recurrence rates between the two modalities during long-term follow-up.

Summary: A retrospective single-center study on 98 consecutive procedures revealed that endoscopic submucosal resection appears to have advantages over transanal endoscopic microsurgery in terms of similar en bloc resection rate and lower rates of late complications and recurrences. Multicenter randomized controlled trials are recommended to further support these findings.

Summary: This single-center study reports the prognosis of early rectal cancer patients over 60 years old after TEM. The 5-year progression-free survival rate was lower in the high-risk patients group than in the non-high-risk patients group, but there was no statistically significant difference. There was also no significant difference in DFS and OS between patients over and under 70 years old.

Summary: TEM procedure for T2-3 rectal cancer after CRT and cCR showed significant rates of ypT0 and ypT1 with excellent long-term outcomes. The 5-year disease-free survival rate and 5-year overall survival rate were both high, demonstrating that TEM is an oncologically safe and effective treatment for these patients.

Summary: The study evaluated the safety and effectiveness of transanal endoscopic microsurgery for rectal neuroendocrine tumours. A retrospective cohort study was conducted on patients with pathology-confirmed rectal neuroendocrine tumours treated by transanal endoscopic microsurgery from April 2007 to December 2020. The results showed that transanal endoscopic microsurgery is an effective approach for managing primary, incompletely excised and recurrent rectal neuroendocrine tumours with good clinical and oncological outcomes.

Summary: Based on the Law for the Regulation of Health Professions (LOPS), a national survey was conducted among Spanish surgeons to assess their opinions on re-accreditation. The survey revealed a lack of awareness regarding re-accreditation requirements and highlights the need for adequate and reliable information. Most respondents agreed that re-accreditation should be regulated through an objective and predictable official procedure.

Summary: This study found that plasma succinate levels were higher in patients with active Crohn's disease (CD), and that succinate and bacteria promote the transition from white to beige adipose tissue (AT) in CD. The findings suggest a potential role of beige AT in CD and may contribute to the development of therapeutic or diagnostic interventions.

The experimental results show that SERGREEN software can detect differences in ICG fluorescence intensity between normal and ischemic situations, and a reduction of 32.6% or more is correlated with tissue ischemia.

Summary: The aim of this study is to provide a rationale for reducing catheterization time while avoiding acute urine retention in patients undergoing scheduled laparoscopic colon surgery. The study is a multicenter, prospective, controlled, randomized non-inferiority study, with patients randomized into two groups: experimental group (with immediate catheter removal after surgery) and control group (with catheter removal 24 hours post-surgery). The main outcome is the development of acute urine retention, with secondary outcomes being urinary infection and hospital stay.

Summary: Smoking has negative effects on Crohn's disease patients, while quitting smoking after surgery can reduce the risk of recurrence. Both smoking and quitting smoking affect the immune suppression capabilities of adipose stem cells (ASCs) and increase inflammatory markers. Epigenetic analysis shows significant changes in gene methylation patterns in ASCs from smokers and ex-smokers with CD.

Summary: The purpose of this study was to analyze the safety and feasibility of laparoscopic and robotic surgery in upper rectum, sigmoid, and left colon resection and anastomosis, and to compare the short-term differences between these two approaches. The results showed that both laparoscopic and robotic intracorporeal resection and anastomosis were feasible and safe, and achieved similar surgical, postoperative, and pathological outcomes as described in the literature. However, there seemed to be more complications in the laparoscopic group.

Summary: This study aimed to evaluate the long-term occurrence of parastomal hernia (PH) after the use of a prophylactic synthetic non-absorbable mesh in elective end colostomy. Long-term data from three randomized controlled trials were collected and analyzed. The results showed that the presence of a prophylactic mesh did not significantly reduce the development of PH in the overall study population, but the open retromuscular technique had a higher reduction in PH occurrence.

Summary: This study suggests that laparoscopic TaTME can avoid conversion to open surgery and has a significantly lower conversion rate compared to laparoscopic total mesorectal excision (LaTME), especially when performed by experienced surgeons. There were no significant differences in postoperative recovery and complications between the two groups, but laparoscopic TaTME had a lower incidence of anastomotic leakage.
