Summary: The study found that attending physicians advance the scope during colonoscopy faster and with significantly less colonic displacement compared to resident trainees. Those who used torque steering also caused lower maximum colonic displacement than those who used knob steering.

Summary: Thermal ablation of the defect margin after endoscopic mucosal resection (EMR-T) has been shown to significantly reduce residual or recurrent adenoma (RRA) at first surveillance colonoscopy. In an international multi-center prospective trial, EMR-T was found to be a simple, inexpensive, and highly effective auxiliary technique for managing large nonpedunculated colorectal polyps.

Summary: Colonoscopy poses a risk for colon ischemia due to various factors such as impaired splanchnic circulation, medications used for sedation, bowel preparation, bowel wall ischemia from insufflation, and endoscope insertion. Gastroenterologists should be aware of these risk factors to minimize risks, facilitate early diagnosis, and administer appropriate treatment.

Summary: This study aimed to evaluate whether the implementation of synoptic reports led to changes in the rate of repeat preoperative endoscopy for colorectal lesions. The results showed that synoptic reports significantly increased the reporting rates of tattoo placement, photo-documentation, and bowel preparation quality, but did not decrease the rate of repeat preoperative endoscopy. Future studies may need to address guideline deficiencies in order to improve this outcome.

Summary: Management of colonic diverticulitis should involve personalized diagnosis and treatment based on the patient's condition, including the use of imaging and lower endoscopy to exclude alternative diagnoses, selective antibiotic use, and lifestyle modifications to reduce the risk of recurrence. Patients should understand the importance of diet and healthy behaviors in preventing future episodes of diverticulitis.

Summary: Pneumatosis cystoid intestinalis (PCI) is a rare condition characterized by abnormal accumulation of gas within the submucosa, subserosa, or both layers of the intestine. It is classified into primary and secondary forms and can be associated with various underlying etiologies. Colonoscopy, surgery, and radiological findings are commonly used for diagnosis, but colorectal endoscopic ultrasound (EUS) can provide a reliable and non-radiation option. However, more studies are needed to define the best approach for PCI diagnosis due to its rarity.

Summary: The study evaluated the outcomes of intra-procedural deep mural injury (DMI) in large non-pedunculated colorectal polyps (LNPCPs) undergoing endoscopic mucosal resection (EMR). It found that significant DMI can be managed endoscopically with high success rates and does not impact technical success or recurrence.

Summary: Wireless Capsule Endoscopy (WCE) is increasingly used for non-invasive screening of the gastrointestinal tract. A deep learning method using StyleGAN is proposed to create a realistic WCE atlas. Clinically relevant attributes are identified and synthetic images with selected attributes can be generated. The images are evaluated by gastroenterology experts and found to be highly realistic and plausible.

Summary: This study compared the tissue effects of different electrosurgical currents during endoscopic mucosal resection in a porcine model. The results showed that for snare resection, there was no significant difference between MCC and LPCC in terms of tissue effects on the colon wall. However, for margin ablation, APC was less uniform and complete compared with STSC.

Summary: Results showed that 57% of colorectal surgery patients received tattooing for tumor localization at index endoscopy, but in most cases, the tattoo location was not documented. Multivariate analysis revealed that patients with lesions located in the transverse colon, surgeries performed after 2010, laparoscopic procedures, and index endoscopy performed in an urban setting were more likely to receive tattooing at index endoscopy.

A total of 144 patients were included, with a technical success rate of 100% and a clinical success rate of 88.89%. The use of this homemade EVT was found to be feasible, safe, and effective for the management of TGID.

Summary: This study evaluated the reliability of bowel preparation quality scales and found that all five scales had moderate to substantial inter-rater reliability. The study also found that reliability was highest in the right colon and lowest in the left colon. Additionally, the Aronchick score was considered too simplistic for clinical trials, and the assessment of residual fluid in the Ottawa Bowel Preparation Quality Scale was not suitable for central evaluation.

Summary: This study aimed to evaluate the incomplete resection rates (IRRs) when using the cold snare polypectomy (CSP) for colorectal polyps of 4-20 mm. The study found that CSP is safe and feasible for polyps smaller than 10 mm, but CSP failure occurs frequently for polyps larger than or equal to 10 mm, and the IRR remains high.

Summary: This study evaluated the practice of EUS-BD and identified barriers to its widespread adoption. The uptake of EUS-BD as a first-line treatment for MDBO was low, mainly due to lack of high-quality data, fear of adverse events, and limited access to EUS-BD dedicated devices. In salvage situations following failed ERCP, most favored EUS-BD, but in potentially resectable disease, the percutaneous approach was preferred due to concerns about complicating future surgery.

Summary: This systematic review and meta-analysis compared the predictive abilities of different pre-endoscopic scores in prognosticating adverse events in patients with upper gastrointestinal bleeding (UGIB). The Glasgow-Blatchford score (GBS) showed good discriminative ability and clinical utility in predicting the need for hospital-based interventions. Setting the GBS cutoff at 1 or lower effectively identified low-risk patients and allowed for outpatient management.

Summary: Due to the high effectiveness and lack of adverse effects, research has focused on using biological agents and natural products as an alternative method for treating incurable diseases like cancer. This study tested NDV and lavender oil on cancer and normal cell lines, and found that both substances showed cytotoxic effects on cancer cells. Pathological abnormalities were observed in the treated cells, indicating the harmful impacts of the substances on cell appearance. Additionally, programmed cell death was examined using acridine orange and ethidium bromide staining.

Summary: Upper gastrointestinal bleeding (UGIB) is a common gastrointestinal emergency that has significant morbidity and mortality. The epidemiology of UGIB has been changing with a decrease in peptic ulcer disease and an increase in other etiologies. Risk assessment and patient stratification are important for optimal care, with some tools able to identify a low-risk group for outpatient management. Primary care providers play a crucial role in resuscitation, stabilizing patients, providing blood transfusions, and arranging urgent endoscopic assessment and intervention. Managing antithrombotic agents in the acute setting remains challenging.
