Clinical applications of hepatocyte transplantation

The shortage of organ donors is a problem worldwide, with approximately 15% of adult patients with life-threatening liver diseases dying while on the waiting list. The use of cell transplantation for liver disease is an attempt to correct metabolic defects, or to support liver function as a bridge to liver transplantation and, as such, has raised a number of expectations. Most of the available studies briefly reported here focus on adult hepatocyte transplantation (HT), and the results are neither reproducible nor comparable, because the means of infusion, amount of injected cells and clinical variability differ among the studies. To better understand the specific role of HT in the management of end-stage liver disease, it is important that controlled studies, designed on the principles of evidence-based medicine, be done in order to guarantee the reproducibility of results. Summary: Intrahepatic cholangiocarcinoma (iCCA) is a rare and aggressive liver tumor, with increasing incidence and mortality rates even after radical resection. Recent attention has focused on the disease, supporting liver transplantation as a viable treatment option. Genetic mutations activating signaling pathways have been linked to iCCA tumorigenesis, with a strong association found between Notch pathway mutations and tumor size. Efforts in molecular analysis may improve diagnostic capabilities and treatment protocols for better long-term survival in iCCA and HCC-iCCA patients.

Summary: The ethical analysis of performing liver transplantation in patients with resolved or active COVID-19 shows that the decision is ethically justifiable. By considering medical indications, patient preferences, quality of life, and contextual features, the decision is made on a case-by-case basis. Shared decision-making allows for the integration of clinical options with the patient's subjective preferences and considerations, providing valid informed consent tailored to individual circumstances. Including carefully selected SARS-CoV-2 positive donors offers the opportunity to save lives for patients who may have limited chances of receiving a transplant.

Continuous Renal Replacement Therapy after Liver Transplantation: Peri-Operative Associated Factors and Impact on Survival

Summary: This study retrospectively observed and analyzed the early use of continuous renal replacement therapy (CRRT) following orthotopic liver transplantation (OLT). The study found that the early use of CRRT had a low incidence and was associated with worse outcomes, including lower survival and higher postoperative creatinine levels. Factors such as preoperative severity and intraoperative hypotension episodes were found to be associated with the increased use of CRRT.

Risk factors for bile leakage after liver resection for neoplastic disease

Summary: The study aimed to identify risk factors for biliary leakage (BL) after liver surgery. The results showed that BL occurrence was associated with bilio-digestive anastomosis, neoadjuvant chemotherapy, body mass index, anatomical resection, and blood loss. Identifying high-risk patients can guide the positioning of drainage after surgery.

Summary: This study aimed to evaluate the impact of the immune system after graft reperfusion during kidney transplantation in adults in order to identify predictive biomarkers. The study found that the percentage of T-lymphocytes in the liver perfusate was significantly associated with delayed functional recovery and secondary infection. However, there was no significant association between the proportions of NK cells and NK-T cells with clinical outcomes.

Neurological Screening in Elderly Liver Transplantation Candidates: A Single Center Experience

Summary: This study aimed to determine whether cerebral small vessels disease (cSVD) predicts acute neurological complications (NCs) in liver transplant patients aged 65 years or older. The results showed that more severe cSVD was associated with seizures, longer hospital stay, and disability. Hepatic encephalopathy and ascites were predictors of NCs after liver transplantation.
