Rheumatoid arthritis (RA) is a chronic disease characterized by persistent synovitis, systemic inflammation, and the presence of autoantibodies. Neuropsychiatric manifestations are quite common in RA, including depression, cognitive dysfunction, behavior changes, spinal cord compression and peripheral nerve involvement. Potential causes include systemic inflammatory process, neural compression due to bone and joint destruction, side effects of medications and copying difficulties due to the chronicity of the disease. A high level of suspicious is required for an adequate diagnosis and treatment. In this review, we will discuss topographically the main neuropsychiatric manifestations described in RA patients, in an attempt to help in the management of these complex and multifaceted disease. Summary: This study utilized data from an international juvenile SSc cohort to evaluate the effectiveness of pulmonary screening with FVC and DLco in detecting ILD compared to HRCT. The results showed that the performance of PFTs, especially FVC, was limited in detecting ILD in juvenile SSc patients, missing around 60% of children with ILD changes on HRCT. DLco had higher sensitivity but lower specificity than FVC in detecting potential abnormalities on HRCT. These findings support the use of HRCT in tandem with PFTs for ILD screening in juvenile SSc.

Revisiting the use of proton magnetic resonance spectroscopy in distinguishing between primary and secondary malignant tumors of the central nervous system

Milena K. Farche, Natalia O. Fachinetti, Luciana R. P. da Silva, Larissa A. Matos, Simone Appenzeller, Fernando Cendes, Fabiano Reis

Summary: This study evaluated the usefulness of proton magnetic resonance spectroscopy (H-1-MRS) in distinguishing primary from secondary brain tumors. The results showed that secondary tumors had higher NAA/Cr and Cho/Cr ratios, with a good correlation between NAA/Cr and Cho/Cr. By constructing a logistic regression model, the nature of the tumor could be reasonably predicted.

Magnetic resonance imaging texture analysis for quantitative evaluation of the mandibular condyle in juvenile idiopathic arthritis

Ana Lucia Franco Ricardo, Gabriel Araujo da Silva, Celso Massahiro Ogawa, Amanda D. Nussi, Catharina Simione De Rosa, Jaqueline Serra Martins, Sergio Lucio Pereira de Castro Lopes, Simone Appenzeller, Paulo Henrique Braz-Silva, Andre Luiz Ferreira Costa

Summary: This study evaluated the feasibility of using texture analysis (TA) based on magnetic resonance imaging (MRI) to identify patients with juvenile idiopathic arthritis (JIA) likely to progress to temporomandibular joint damage. The results showed significant differences in texture parameters of the condyle between the JIA group and the control group, and a progressive loss of uniformity in grayscale pixels of MRI with increasing age in the JIA group.

Summary: In this study, a convolutional neural network (CNN) is proposed for quality control of corpus callosum (CC) segmentations. The method was validated using magnetic resonance imaging (MRI) data and achieved high accuracy on the test set. Furthermore, the approach demonstrated good generalization capability to data from different domains.

Neuropsychiatric manifestations in childhood-onset systemic lupus erythematosus

Simone Appenzeller, Danilo Rodrigues Pereira, Paulo Rogerio Julio, Fabiano Reis, Leticia Rittner, Roberto Marini

Summary: Neuropsychiatric manifestations are common in childhood-onset systemic lupus erythematosus (SLE) and are difficult to diagnose. Most patients experience these manifestations within the first two years of the disease, with 30-70% of them having multiple events during the course of the disease. These symptoms have significant impact on disability and mortality, but only a few biomarkers have been validated for diagnosis, treatment monitoring, and prognosis.

Summary: This study aimed to compare the levels of Th1 (IL-12) and Th2 (IL-6 and IL-10) cytokines among cSLE, aSLE, and healthy controls over a two-year period. The results showed that cSLE patients had significantly higher levels of IL-12 compared to healthy controls, while aSLE patients had significantly higher levels of IL-6 compared to healthy controls.

Multidisciplinary Management of Fabry Disease: Current Perspectives

Luciana Paim-Marques, Rodrigo Joel de Oliveira, Simone Appenzeller

Summary: Fabry disease is a rare genetic disorder caused by a gene variant that leads to a deficiency in the enzyme alpha-galactosidase A. This deficiency causes the accumulation of a specific lipid in various organs, resulting in a wide range of clinical manifestations and potentially life-threatening complications such as cardiac arrhythmia, kidney failure, and stroke. This review provides an overview of the clinical features and current multidisciplinary management approaches for Fabry disease patients.

Summary: This study found that childhood-onset SLE patients have axonal dysfunction, which is associated with IFN-gamma levels and depressive symptoms. Increased CHO ratios are related to cognitive impairment, while mI/Cr ratios are correlated with cumulative glucocorticoids dosage.
