Amyloid in endomyocardial biopsies

The prognosis of cardiac amyloidosis depends on the nature and origin of the amyloid protein deposited. However, little is known about the prevalence and origin of amyloid in heart muscle biopsies. We therefore examined retrospectively the distribution and origin of amyloid in a consecutive series of endomyocardial biopsies. Endomyocardial biopsies with verified presence of amyloid from 101 patients were included. Amyloid was classified immunohistochemically in each of them. Our collective comprised 63 men and 38 women, with a mean age of 66 years (range 37-85 years). Cardiac amyloidosis was the most common of the AL (54 patients) or ATTR type (42 patients). In five individuals, amyloid remained unclassified. AL amyloidosis was subdivided into AL lambda (45 patients) and AL kappa amyloid (nine patients). AA amyloid was not found in any individual. The amount of amyloid was higher in AL than in ATTR amyloidosis. Genomic DNA was extracted and examined by DNA sequencing in 19 patients with ATTR amyloidosis. Five (26%) individuals carried TTR mutations (p.Val20Ile, p.Val30Met (twice), p.Asp39Val and p.Glu54Asp) and were classified as suffering from hereditary ATTR amyloidosis. Amyloid in endomyocardial biopsies is most commonly of immunoglobulin light chain origin, followed by non-hereditary and hereditary-type ATTR amyloid.

Differential Diagnosis in Hypophysitis: First Report on a Spindle Cell Rhabdomyosarcoma of the Pituitary Gland

Summary: Pituitary gland metastases are rare and often diagnosed during autopsies. The most common primary tumors that metastasize to the pituitary gland are breast, lung, thyroid, and renal carcinomas. We present a rare case of spindle cell rhabdomyosarcoma metastasizing to the pituitary gland. The patient experienced symptoms such as headache, visual disorder, panhypopituitarism, and diabetes insipidus. Despite the inability to surgically remove the tumor, biopsy confirmed the diagnosis and the patient received chemotherapy and irradiation. Even high-risk patients can achieve remission after treatment.

Amyloid myopathy: expanding the clinical spectrum of transthyretin amyloidosis-case report and literature review

Summary: This article describes two patients with ATTR amyloid myopathy, one with ATTR variant amyloidosis (ATTRv) and one with wild-type ATTR amyloidosis (ATTRwt). The ATTRwt patient showed muscular tracer uptake consistent with ATTR amyloid myopathy, while in the ATTRv patient, myopathy occurred after neuropathy and cardiomyopathy. Muscle biopsies confirmed the presence of interstitial amyloid deposits, particularly high in the adipose tissue. This case report highlights the frequent concomitant presence of cardiac ATTR amyloidosis and ATTR amyloid myopathy.

Summary: Dietary habits and food can lead to oral dysbiosis, which affects overall health and can lead to systemic diseases. Fermentable carbohydrates in modern diets, such as processed starch and synthetic carbohydrates, are risk factors for oral dysbiosis. The interconnectivity of oral, lung, and gut microbiomes is not well understood, particularly regarding the role of lactobacilli and Lactobacillus phages in rescuing dysbiotic effects.

Identification of AL proteins from 10 λ-AL amyloidosis patients by mass spectrometry extracted from abdominal fat and heart tissue

Summary: This study analyzed the exact primary structure of AL proteins and their precursor LCs in 10 lambda AL amyloidosis patients. Results showed that all AL proteins had a common C-terminal truncation region and a significant increase in the isoelectric point of V-L. Moreover, no evidence of other common post-translational modifications was found.

Intratumoral heterogeneity affects tumor regression and Ki67 proliferation index in perioperatively treated gastric carcinoma

Summary: The study investigates the problem of intratumoral heterogeneity (ITH) in gastric cancer (GC), and tests the effects of Ki67 and tumor regression on ITH after neoadjuvant/perioperative chemotherapy. The results suggest that Ki67 is significantly associated with tumor regression and may serve as a predictor for chemotherapy sensitivity and an indicator of ITH.

The results showed that cSS was more common in CAA, and MLH with cSS was associated with ICH. The concentrations of cerebrospinal fluid markers differed among the groups.

This review discusses mitigation strategies in sample quality control, DNA repair treatments, sample preparation, and bioinformatic analysis of FFPE-DNA.

The tumor biological significance of RNF43 and LRP1B in gastric cancer is complex and context-dependent

Summary: This study explored the expression and correlation of RNF43 and LRP1B with clinicopathological characteristics in gastric cancer. The expression of RNF43 and LRP1B was significantly correlated with the Lauren phenotype and with each other. Furthermore, differential effects of RNF43 expression were observed between intestinal and diffuse type gastric cancers. These findings suggest that the expression and biological effects of RNF43 and LRP1B in gastric cancer are context-dependent.

SMARCA4 and SMARCE1 in gastric cancer: Correlation with ARID1A, and microsatellite stability, and SMARCE1/ERBB2 co-amplification

Summary: Recent studies have found a connection between certain subunits of the SWI/SNF complex and specific tumor characteristics in gastric cancer. This study aimed to examine the expression patterns of SMARCA4 and SMARCE1, two subunits of the SWI/SNF complex, in gastric cancer and their potential relationship with ERBB2. The results showed significant correlations between the expressions of SMARCA4 and SMARCE1 with ARID1A, p53, and microsatellite status, but not with patient prognosis. Amplification analysis revealed co-amplification of SMARCE1 and ERBB2 in some cases, suggesting potential synergistic effects for improved treatment of gastric cancer.

Frequency of deep-seated cerebral microbleeds in patients with lobar hemorrhages and histopathological evidence for cerebral amyloid angiopathy

Summary: This study aimed to estimate the frequency of deep-seated microbleeds on MRI in patients with lobar hemorrhages and histopathological evidence for cerebral amyloid angiopathy (CAA). The findings showed that approximately 15% of patients with histopathologically diagnosed CAA have deep-seated microbleeds.

Prevalence of amyloid in ligamentum flavum of patients with lumbar spinal stenosis

Summary: This study found that amyloid, mostly of the ATTR subtype, was detected in over three quarters of patients undergoing surgery for lumbar spinal stenosis, and it is associated with age and ligamentum flavum thickness. These findings suggest that histopathological examination of the ligamentum flavum may be helpful for future decision making.

MDM2 amplification is rare in gastric cancer

Summary: The study validated gene amplification, protein expression, and tumor biological function of MDM2 in gastric cancer, and found a significant correlation between MDM2 amplification and protein expression. However, MDM2 amplification and expression were not associated with patient survival.

Human APOE4 Protects High-Fat and High-Sucrose Diet Fed Targeted Replacement Mice against Fatty Liver Disease Compared to APOE3

Summary: Recent studies have found that the human APOE epsilon 4 allele protects against non-alcoholic fatty liver disease (NAFLD), while epsilon 3 promotes hepatic steatosis and steatohepatitis. In this study, mice expressing the APOE3 or APOE4 protein isoforms were fed a high-fat and high-sucrose diet to induce obesity. The results showed that APOE3 mice had higher liver weights and exhibited more severe hepatic steatosis compared to APOE4 mice. Proteome analysis also revealed differences in proteins related to inflammation, damage response, and lipid storage between APOE3 and APOE4 mice. These findings support the association of increased NAFLD risk with the APOE epsilon 3 allele and suggest potential mechanisms of protection conferred by APOE epsilon 4.

Impact of peritumoral brain edema on pre- and postoperative clinical conditions and on long-term outcomes in patients with intracranial meningiomas

Summary: This study systematically examined 696 patients with primary intracranial meningiomas to assess the effect of preoperative peritumoral brain edema (PTBE) on preoperative symptoms, neurological deficits, postoperative complications, and long-term outcomes. Results showed that PTBE significantly increased the incidences of specific preoperative symptoms, neurological deficits, and postoperative complications. Patients with preoperative PTBE required more medical support after surgery, but all patients had favorable outcomes.

Summary: This study assessed the safety and efficacy of donor-derived CAR-T therapy in myeloma patients with prior allo-SCT. The treatment was well tolerated, with no observed adverse effects such as neurotoxicity syndrome or cytokine release syndrome. Importantly, no new graft-versus-host disease was observed, and all patients responded well to treatment.
