Distribution of invasive meningococcal B disease in Italian pediatric population: Implications for vaccination timing

Neisseria meningitidis group B (MenB) is a leading cause of meningitis and sepsis. A new vaccine has been recently licensed. The aim of the present study was to evaluate the epidemiology of MenB disease in pediatric age and define the optimal age for vaccination. All patients aged 0-18 years admitted with a diagnosis of meningitis or sepsis to the 83 participating Italian pediatric hospitals were included in the study. Blood and/or cerebrospinal fluid (CSF) samples were tested by Realtime-PCR and/or culture. One hundred and thirty-six cases (mean age 5.0 years, median 2.7) of MenB disease were found. Among these, 96/136 (70.6%) were between 0 and 5 years, 61/136(44.9%) were between 0 and 2 years. Among the latter, 39/61 (63.9%) occurred during the first year of life with highest incidence between 4 and 8 months. A case-fatality rate of 13.2% was found, with 27.8% cases below 12 months. Sepsis lethality was 24.4%. RT-PCR was significantly more sensitive than culture: 82 patients were tested at the same time by both methods, either in blood or in CSF; MenB was found by RT-PCR in blood or CSF in 81/82 cases (98.8%), culture identified 27/82 (32.9%) infections (Cohen's Kappa 0.3; McNemar's: p < 10(-5)). The study shows that the highest incidence of disease occurs in the first year of age, with a peak between 4 and 8 months of life; 30% of deaths occur before 12 months. The results suggest that the greatest prevention could be obtained starting MenB vaccination in the first months of life; a catch-up strategy up to the fifth year of life could be considered. Our results also confirm that Realtime PCR is significantly more sensitive than culture. In those countries where only isolate positive infections are counted as cases, the incidence of MenB infection results highly underestimated. (C) 2013 The Authors. Published by Elsevier Ltd. Is ketamine infusion effective and safe as an adjuvant of sedation in the PICU? Results from the Ketamine Infusion Sedation Study (KISS)

Summary: This observational study aimed to evaluate the efficacy and safety of ketamine as an adjuvant for analgesia and sedation in the Pediatric Intensive Care Unit (PICU). The results showed that ketamine infusion significantly reduced pain scores and the need for opioids and benzodiazepines. Adverse events were minor and reversible. Overall, ketamine infusion appeared to be a valid strategy for ensuring comfort and sparing traditional sedative drugs in difficult-to-sedate PICU patients.

Sero-Epidemiological Study of Varicella in the Italian General Population

Summary: The aim of this study was to analyze the seroprevalence of varicella in Italy and evaluate the impact of varicella vaccination. The overall seroprevalence was 91.6% with higher rates in females and increasing prevalence in younger age groups. The comparison with previous surveys showed significantly higher prevalence in children and adolescents, indicating the positive impact of varicella vaccination in Italy.

Summary: In Italy, vaccination against serogroup B is recommended for infants aged 3-13 months, serogroup C for infants aged 13-15 months, and serogroups A, C, Y, and W for adolescents aged 12-18 years. A quadrivalent meningococcal conjugate vaccine, MenACYW-TT (MenQuadfi; Sanofi), is recommended based on its high seroprotection rates and low incidence of adverse events in these age groups.

Human Bocavirus in Childhood: A True Respiratory Pathogen or a Passenger Virus? A Comprehensive Review

Summary: Human bocavirus (HBoV) has emerged as an increasingly reported pathogen, primarily associated with respiratory tract infections in adults and children. It can act as a co-infectious agent or an isolated viral pathogen, and has also been found in asymptomatic individuals. This review provides an overview of HBoV's epidemiology, risk factors, transmission, pathogenicity, detection methods, and clinical features, including severe infections requiring hospitalization and rare fatal cases. Tissue viral persistence, reactivation, reinfection, and the burden of HBoV disease are also evaluated.

Health-related quality of life in patients aged 6-18?years with chronic hepatitis C treated with sofosbuvir/velpatasvir

Summary: The aim of this study was to assess the effect of treatment with sofosbuvir/velpatasvir (SOF/VEL) on the health-related quality of life (HRQL) of children with chronic hepatitis C. The study found that treatment with SOF/VEL improved children's physical well-being and showed a trend towards better scores in social support and peers. However, there was poor to moderate agreement between child self-reports and parent proxy reports.

Apneas requiring respiratory support in young infants with COVID-19: a case series and literature review

Summary: The objective of this study was to describe the clinical features of young infants with apneas as a clinical sign of COVID-19. The study included 4 infants who required respiratory support for severe COVID-19 with recurrent apneas. A review of the literature found a total of 17 young infants with COVID-19 and apneas. Most cases (88%) had apnea as an initial symptom and two cases experienced recurrence after 3-4 weeks. The children underwent various neurological workups and required different levels of respiratory support, but all patients had a complete recovery.

Polarimetric Scattering by a Vessel at Different Incidence Angles
