Incidental undifferentiated carcinoma of the prostate. A case with unusual diagnosis

A 69-year-old man presented with an incidental undifferentiated carcinoma of the prostate. Ten years ago the patient had clinical stage I seminoma of the right testis with adjuvant radiotherapy. Follow-up care was without pathological findings. Staging examinations did not show metastatic disease. After radical prostatectomy with pelvic lymphadenectomy, the histological examination revealed a metastatic seminoma in the prostate. Therefore, the diagnosis was a late relapse in an extremely rare location. So far only four other reports describe testicular seminoma with metastases to the prostate.

Summary: This study presents the initial experience of treating patients with unilateral low-risk prostate cancer using vascular-targeted photodynamic therapy (VTP). The short-term functional and oncological outcomes of VTP were compared to those of patients undergoing radical prostatectomy (RP). The results showed that VTP had promising outcomes with lower complication rates compared to RP in a real-world setting. However, the recurrence and progression after VTP still need to be carefully discussed with patients who choose this treatment option.

Summary: Currently, combination therapies are predominantly used for the systemic treatment of prostate cancer. Many considerations need to be taken into account in older and comorbid patients to avoid toxicities and achieve successful therapy and good quality of life. Geriatric screening, assessment, and treatment improvement are recommended for patients over 70 years old who have comorbidities. Fit patients can be treated similarly to non-geriatric patients, while vulnerable and frail patients require special care to minimize toxicities. Individualized concepts, including aspects of palliative care and patient needs, should be established when deciding on further treatment.

Neurocognitive effects of androgen deprivation therapy and new hormonal agents in a sample of patients with metastatic prostate cancer

Summary: This study aimed to provide further data on domain-specific cognitive impairments in metastatic prostate cancer (mPC) patients with androgen deprivation therapy (ADT) and new hormonal agents. The results showed that patients treated with ADT scored significantly lower on neurocognitive tests and had a higher prevalence of neurocognitive deficits.

Couples coping with advanced prostate cancer: an explorative study on decision-making preferences, self-efficacy and fear of progression

Summary: The purpose of this study was to explore the characteristics of treatment decision-making preferences, general self-efficacy, and fear of progression among couples coping with advanced prostate cancer. The study found that more than half of the patients and spouses preferred active involvement in decision-making, and spouses had higher levels of fear of progression.

Summary: This study reported contemporary epidemiology and treatment trends of upper tract urothelial carcinoma (UTUC) in Germany over a 14-year period. The incidence rate showed a non-significant increase, while survival rates were relatively low. Diagnostic and therapeutic procedures for UTUC have increased, and minimally invasive nephroureterectomy is now the predominant surgical approach.

Summary: Focal therapy (FT) is an alternative approach for prostate cancer treatment in selected patients. In Germany, the development of FT followed the Gartner hype cycle, with high-intensity focused ultrasound (HIFU) being the most commonly performed FT. However, newer FT modalities such as Vascular-Targeted Photodynamic Therapy (VTP-TOOKAD) and Transurethral Ultrasound Ablation (TULSA) are becoming increasingly popular.
