Uterine sliding sign: a simple sonographic predictor for presence of deep infiltrating endometriosis of the rectum

Objective To evaluate whether the presence of uterorectal adhesions demonstrated by transvaginal sonography (TVS) could aid as a simple sonographic predictor for deep infiltrating endometriosis (DIE) of the rectum in patients with symptoms suggestive of endometriosis. Methods This was a prospective multicenter study of women scheduled for laparoscopy because of symptoms suggestive of endometriosis. Patients were assessed prospectively using TVS before laparoscopy and radical resection of disease followed by histological confirmation. Sensitivity, specificity, positive (PPV) and negative (NPV) predictive values, accuracy and positive (LR+) and negative (LR-) likelihood ratios were calculated for the observation of a negative uterine 'sliding sign' on TVS in predicting the presence of DIE of the rectum. Results In total, 117 patients underwent laparoscopy and resection. Thirty-four (29%) patients had DIE of the rectum. A negative sliding sign on TVS predicted DIE of rectum with a sensitivity of 85%, specificity of 96%, PPV of 91%, NPV of 94%, accuracy of 93.1%, LR + of 23.6 and LR- of 0.15. Conclusions Sonographic demonstration of uterorectal adhesions reflected by a negative uterine sliding sign is an easy and practical method for prediction of the presence of DIE involving the rectum. This could be a valuable 'red flag' sign for triaging patients to tertiary referral centers and specialized clinics for detailed investigation. Therefore, it is important to focus on the outcomes of adolescent pregnancies and provide family planning and baby care social trainings.

Intrapartum ultrasound for fetal head asynclitism: Is it possible to establish a degree of asynclitism to correlate to delivery outcome?

Summary: The study aimed to investigate the role of intrapartum ultrasound in diagnosing asynclitism and the importance of the degree of asynclitism in labor outcomes. A cohort study was conducted on 41 low-risk pregnant women with singleton-vertex fetus. Severe and moderate asynclitism was observed in 41.7% and 58.8% of women, respectively. Severe asynclitism was associated with increased operative birth and maternal-fetal complications. Detection of asynclitism degree by intrapartum ultrasound could be useful in alerting the obstetrics team to potential perinatal problems during delivery.

Prolonged Dystocic Labor in Neuraxial Analgesia and the Role of Enkephalin Neurotransmitters: An Experimental Study

The results showed a significant reduction in En in the LUS of the PDL group, indicating that the drugs used during LNA may not effectively control the dystocic pain.

Local anesthesia for Altis® single incision sling in women with stress urinary incontinence

Summary: The aim of this study was to verify the efficacy and safety of local anesthesia during Altis (R) single incision placement for stress urinary incontinence. The results showed that local anesthesia had a good objective and subjective cure rate, with shorter operative time compared to spinal anesthesia. There were no significant differences in cure rate and postoperative complications between the two anesthesia methods. Therefore, local anesthesia may be considered a safe alternative to spinal anesthesia for Altis (R) implantation, allowing for outpatient procedures.

The available data are limited to symptomatic women and occasional observations. Hormonal medical therapy cannot be blinded if recognised by the patient. Randomised controlled trials are not realistic for surgery due to the variable nature and low numbers of endometriosis cases. Each diagnosis and treatment is an experiment with an outcome, and experience plays a crucial role in Bayesian updating.

Myomectomy in adult women of reproductive age: a propensity score-matched study for pregnancy rates

Summary: This observational prospective study aimed to determine whether performing myomectomy in patients over 40 years old with symptomatic uterine fibroids and desiring future fertility would have better surgical and reproductive outcomes. The study included laparoscopic intracapsular myomectomy (LIM), open laparotomy (OIM), and non-surgical management as a control group. The results showed that there were no significant differences in reproductive outcomes between operated and non-operated patients, indicating that myomectomy in patients over 40 years old did not have a detrimental effect on future pregnancy rates compared to expectant management.
