Deferoxamine mesylate (DFO) is themost commonly used iron-chelating agent to treat transfusion-related hemosiderosis. Despite the clear advantages for the use of DFO, numerous DFO-related systemic toxicities have been reported in the literature, as well as sight-threatening ocular toxicity involving the retinal pigment epithelium (RPE). The damage to the RPE can lead to visual field defects, color-vision defects, abnormal electrophysiological tests, and permanent visual deterioration. The purpose of this review is to provide an updated summary of the ocular findings, including both functional and structural abnormalities, in DFO-treated patients. In particular, we pay particular attention to analyzing results of multimodal technologies for retinal imaging, which help ophthalmologists in the early diagnosis and correctmanagement of DFO retinopathy. Fundus autofluorescence, for example, is not only useful for screening patients at high-risk of DFO retinopathy, but is also a prerequisite for identify specific high-risk patterns of RPE changes that are relevant for the prognosis of the disease. In addition, optical coherence tomography may have a clinical usefulness in detecting extent and location of different retinal changes in DFO retinopathy. Finally, this review wants to underline the need for universally approved guidelines for screening and followup of this particular disease.

Intravitreal aflibercept for management of choroidal neovascularization secondary to angioid streaks The Italian EYLEA-STRIE study

Maurizio Battaglia Parodi, Maria Vittoria Cicinel, Alessandro Marchese, Chiara Giuffre, Francesco Viola, Giovanni Staurenghi, Monica Varano, Francesco Bandello

Summary: Intravitreal aflibercept is effective in treating choroidal neovascularization secondary to angioid streaks, with improvements in visual acuity and central macular thickness at 48 weeks. Some patients may experience adverse events, warranting further observation and studies with different therapeutic regimens.

Nonclinical Toxicology and Biocompatibility Program Supporting Clinical Development and Registration of the Port Delivery System With Ranibizumab for Neovascular Age-Related Macular Degeneration

Vladimir Bantseev, Joshua Horvath, Giulio Barteselli, Shrirang Ranade, Mauricio Maia, Daniela Bumbaca Yadav, Chris Schuetz, Amy Shelton, Helen S. Booler

Summary: The Port Delivery System with ranibizumab (PDS) is a drug delivery system designed to provide continuous intravitreal release of ranibizumab without introducing any new toxicology-related safety concerns. Toxicology program results showed that the PDS is biocompatible and safe for clinical development.

Predictive factors of radio-induced complications in 194 eyes undergoing gamma knife radiosurgery for uveal melanoma

Maria Vittoria Cicinelli, Maura Di Nicola, Carmen Rosaria Gigliotti, Marco Battista, Elisabetta Miserocchi, Antonella del Vecchio, Pietro Mortini, Francesco Bandello, Giulio Maria Modorati

Summary: This study investigated factors predictive of radiation-related complications in uveal melanoma patients undergoing gamma knife radiosurgery. The most common complications were radiation retinopathy, radiation papillopathy, and neovascular glaucoma. Factors like age and tumor location were associated with the occurrence of complications.

Effect of COVID-19-related lockdown on ophthalmic practice in Italy: A report from 39 institutional centers

Summary: The number of eye surgical procedures in Italy significantly decreased during the lockdown due to the COVID-19 epidemic, with only 70% of recommended surgeries being performed. Elective surgeries decreased by 96.2%, urgent surgeries by 49.7%, and intravitreal injections by 48.5% during the lockdown period.

The Port Delivery System with ranibizumab: a new paradigm for long-acting retinal drug delivery

Shrirang Ranade, Mark R. Wieland, Tammy Tam, Jennifer C. Rea, Judit Horvath, Aaron R. Hieb, Weitao Jia, Lori Grace, Giulio Barteselli, Jay M. Stewart

Summary: The Port Delivery System is an innovative intraocular drug delivery system that continuously releases ranibizumab for the treatment of eye diseases. It has stable and soluble properties, and controlled release is achieved through a release control element. In vitro studies confirmed its reproducibility and demonstrated that it can maintain a stable release rate for at least 6 months.

Effects of circadian rhythm disruption on retinal physiopathology: Considerations from a consensus of experts

M. Parravano, C. M. Eandi, M. Figus, M. Lupidi, F. Menchini, M. Nicolo', V Parisi, L. Toto, F. Viola, S. Vujosevic, G. Querques

Summary: Circadian rhythms, controlled by the internal clock, synchronize with external fluctuations. Melatonin, a hormone synthesized by the pineal gland, helps maintain daily rhythms. Disrupting circadian rhythms is linked to various diseases and accelerates retinal cell aging.

Adaptive Optics of Kyrieleis Plaques in Varicella Zoster Virus-Associated Posterior Uveitis: A Multimodal Imaging Analysis

Paolo Milella, Chiara Mapelli, Marco Nassisi, Gaia Leone, Giada Ruggi, Antonio Scialdone, Giuseppe Casalino, Francesco Viola

Summary: This study reports the adaptive optics (AO) imaging results of Kyrieleis plaques (KP) in patients with Varicella Zoster virus (VZV)-associated posterior uveitis. AO imaging revealed segmental hyporeflectivity confined to the vessel wall without arterial wall disruption or extravascular involvement. The findings suggest that KP are localized within the inner arterial wall, possibly at the endothelial level.

Retromode Scanning Laser Ophthalmoscopy for Choroidal Nevi: A Preliminary Study

Claudia Azzolini, Maura Di Nicola, Francesco Pozzo Giuffrida, Francesca Cappelli, Claudia Bellina, Francesco Viola, Paolo Chelazzi

Summary: The present study aimed to evaluate the diagnostic validity of retromode imaging in choroidal nevi using the cSLO Nidek Mirante. The results showed that retromode scanning laser ophthalmoscopy was able to detect choroidal nevi with a hypo-retro-reflective pattern, even in cases not visible on other imaging modalities. It also provided the most accurate delineation of lesion margins among the examined modalities. These findings demonstrate that RM-SLO is an innovative and non-invasive diagnostic tool for detecting and monitoring choroidal nevi.

Matteo Airaldi, Alessandro Invernizzi, Vuong Nguyen, Stela Vujosevic, Federico Ricci, Pietro Monaco, Marco Nassisi, Daniel Barthelmes, Mark Gillies, Francesco Viola

Summary: This study compared the treatment outcomes of Vascular Endothelial Growth Factor (VEGF) inhibitors for Polypoidal Choroidal Vasculopathy (PCV) and type 1 Macular Neovascularization (MNV) in a Caucasian population. The results showed that PCV had slightly better visual acuity outcomes after 24 months of treatment compared to type 1 MNV, and received fewer anti-VEGF injections.
