Circulating free DNA in a screening program for early colorectal cancer detection

Aims and background. The quantification and molecular characterization of circulating free DNA (cfDNA) have attracted much interest as new and promising, noninvasive means of detecting and monitoring the presence of surgical resectable colorectal cancer (CRC). Instead, the role of cfDNA in the early detection of malignant and premalignant colorectal lesions is still unclear. The aim of this study was to evaluate the predictive power of the quantification and KRAS status of cfDNA in detecting early colorectal lesions in plasma from healthy high-risk subjects. Methods. The study population consisted of 170 consecutive healthy high-risk subjects aged >50 years who participated in the screening program promoted by the Local Health Service (ASL-Milano) for early CRC detection and who underwent endoscopic examination after being found positive at fecal occult blood test (FOBT). Thirty-four participants had malignant lesions consisting of 12 adenocarcinomas (at an early stage in half of the cases) and 22 instances of high-grade intraepithelial neoplasia (HGIN) in adenomas; 73 participants had premalignant lesions (adenomas and hyperplasia), and 63 participants had no lesions. Plasma cfDNA was quantified by quantitative real-time PCR and analyzed for KRAS mutations by a mutant-enriched PCR. KRAS status was assessed also in matched adenocarcinoma and HGIN tissues. The distribution of cfDNA concentrations among FOBT-positive subjects with diagnosed lesion (cases) was compared with that of FOBT-positive subjects without lesions (controls) and its predictive capability (AUC) was assessed. Results. The predictive capability of cfDNA levels was satisfactory in predicting adenocarcinomas (AUC 0.709; 95% CI, 0.508-0.909) but not HGIN and premalignant lesions. The rate of KRAS mutations in plasma was low (5/170 = 3%) compared with the rate observed in the matched adenocarcinoma and HGIN tissues (45%). Conclusions. The use of cfDNA quantification to predict adenocarcinoma at an early stage in high-risk (aged >50 years and FOBT positive) subjects seems to be promising but needs more sensitive methods to improve cfDNA detection.

Summary: The study evaluated the serological and T-cell responses of lymphoid malignancy patients after mRNA vaccination, finding that some patients, especially those receiving anti-CD20 antibody treatment, may not effectively develop protective antibodies. T-cell responses were detected in most patients, even in seronegative individuals, but a small percentage of cases had no cellular or humoral responses. This raises concerns about the vaccine efficacy for LM patients, highlighting the need for continued protective measures.

Contrast-enhanced digital mammography and magnetic resonance imaging: reproducibility compared to pathologic anatomy

COVID-19 Vaccination in Health Care Workers in Italy: A Literature Review and a Report from a Comprehensive Cancer Center

Summary: Despite the global public health emergency of the COVID-19 pandemic, studies show that healthcare workers in Italy exhibit a robust antibody response after receiving the second vaccine dose. In a longitudinal study, 99.81% of HCWs developed an antibody response one month after the second dose, and six months later, all HCWs still showed positivity to antibodies, although at lower levels. The literature review and institutional study both confirm a strong antibody response in HCWs even seven months after completing the cycle of vaccination doses.

Summary: This study investigated the impact of IDH1/2 mutations on progression-free survival, overall response rate, and disease control rate in iCCA patients treated with platinum-based chemotherapy. The results showed that IDH1/2 mutations are not associated with increased sensitivity to platinum-based chemotherapy in iCCA patients. Furthermore, the analysis of TCGA data indicated that IDH1/2 mutated CCA did not show higher HRD compared to wild-type cases.

Summary: This study analyzed a large series of neuroendocrine carcinomas (NECs) to validate previously identified prognostic factors and determine if additional parameters could be isolated. The most relevant predictors for prognosis were Ki-67 index, morphology, stage, and site. However, morphology was not a significant factor when Ki-67 was greater than or equal to 55%.

3Rs Principle and Legislative Decrees to Achieve High Standard of Animal Research

Summary: The 3Rs principle, consisting of Replacement, Reduction and Refinement, aims to promote ethical and scientifically valid animal experimentation. It requires the consideration of alternative models, statistical designs, and improvements in experimental conditions. The principle was first introduced in a document by the Council Directive in 1986 and developed further in the European Parliament and Council Directive in 2010. This article emphasizes the importance of the 3Rs principle, particularly the concept of Reduction, and highlights the need for its consistent implementation among countries and research teams.

Superior rectal artery preservation to reduce anastomotic leak rates in familial adenomatous polyposis patients treated with total colectomy and ileorectal anastomosis

Summary: This study investigated the effectiveness of superior rectal artery (SRA) preservation in reducing anastomotic leakage during total colectomy with ileorectal anastomosis (TC/IRA) in patients with familial adenomatous polyposis (FAP). The results showed that SRA preservation was associated with fewer complications, lower anastomotic leakage events, and lower overall surgical complications and pelvic sepsis rates. However, it increased operative time while reducing hospital stay.

Exploring the association with disease recurrence of miRNAs predictive of colorectal cancer

Summary: Although miRNA signatures were not predictive of colorectal cancer recurrence, a decrease in expression of miRNAs from the CL signature was observed 1 year after surgery, while miR-335-5p from the HgA signature was higher in AWD patients before surgery. These findings suggest the potential use of miRNAs as non-invasive biomarkers for monitoring patients at risk of relapse.
