Vicente Plaza, Isam Alobid, Cesareo Alvarez, Marina Blanco, Jorge Ferreira, Gabriel Garcia, Antonio Gomez-Outes, Fernando Gomez, Antonio Hidalgo, Javier Korta, Jesus Molina, Francisco Javier Pellegrini, Montserrat Perez, Javier Plaza, Manuel Praena, Santiago Quirce, Jose Sanz

Summary: In this study, the Spanish Asthma Management Guidelines (GEMA) have undergone a thorough review, leading to several significant changes. These changes include new FENO cut-off and severity classification in diagnosis, a more restrictive concept of intermittent asthma and expanded treatment options, as well as improvements in other aspects such as pediatric asthma scoring and the addition of new sections.

Short-Acting Beta-2-Agonist Exposure and Severe Asthma Exacerbations: SABINA Findings From Europe and North America

Summary: There is an association between SABA use and severe asthma exacerbations. Patients using 1 or 2 SABAs in the Netherlands, U.S. commercial, and U.S. Medicare datasets did not show an association with severe exacerbations, while this association was confirmed in the United Kingdom and Poland.

The Response to Biologics is Better in Patients with Severe Asthma Than in Patients with Asthma-COPD Overlap Syndrome

Summary: This study compares the effectiveness of biologics in asthma-COPD overlap (ACO) and asthma patients. The results show that ACO patients treated with biologics have worse outcomes compared to asthma patients, with higher incidence of exacerbations and corticosteroid use, and lower percentage of controlled patients.

Summary: This multi-ancestry study identified novel suggestive regulatory loci for asthma exacerbations, located in genomic regions participating in inflammation and host defense, revealing the genetic basis of asthma exacerbations.

Identification of Two Eosinophil Subsets in Induced Sputum from Patients with Allergic Asthma According to CD15 and CD66b Expression

Elena Curto, Eder F. Mateus-Medina, Astrid Crespo-Lessmann, Ruben Osuna-Gomez, Cristina Ujaldon-Miro, Alba Garcia-Moral, Paula Galvan-Blasco, Lorena Soto-Retes, David Ramos-Barbon, Vicente Plaza

Summary: This study identified two subsets of eosinophils (E1 and E2) in induced sputum samples using flow cytometry, and found that their distribution and correlation varied among different types of asthma patients.

How reliably can algorithms identify eosinophilic asthma phenotypes using non-invasive biomarkers?

Summary: The study found that using the algorithm proposed by Heany et al. to classify asthma patients is insufficient for diagnosing eosinophilic phenotypes, and sputum analysis remains the gold standard for assessing airway inflammation.

Summary: MicroRNAs (miRNAs) are increasingly being used as biomarkers for diagnosis and treatment of various diseases. This study analyzed miRNA expression profiles in serum and lung tissue from severe asthma patients with and without oral corticosteroids (OCS) treatment. MiRNA-Seq and RT-qPCR were used for evaluation and validation of miRNA levels. Altered expression of miRNAs in serum and lung tissue was observed, with hsa-miR-221-5p and hsa-miR-769-5p showing the best differentiation between OCS-treated and untreated individuals. These miRNAs were found to be associated with important signaling pathways for asthma and regulated the expression of genes involved in these pathways. The findings suggest that miRNA profiles can serve as biomarkers for OCS treatment in asthma patients.

Characteristics of Induced-Sputum Inflammatory Phenotypes in Adults with Asthma: Predictors of Bronchial Eosinophilia

Astrid Crespo-Lessmann, Elena Curto, Eder Freddy Mateus Medina, Esther Palones, Alicia Belda Soler, Soraya Sanchez Maza, Lorena Soto-Retes, Vicente Plaza

Summary: The purpose of this study was to determine the bronchial inflammatory phenotypes and associated characteristics in patients with asthma attending a specialist clinic. The study found that the most common phenotype was eosinophilic, and peripheral blood eosinophilia was the best predictor of bronchial eosinophilia. This information can help doctors select appropriate treatment and prevent asthma exacerbation.

Clinical Impact of Electronic Monitoring Devices of Inhalers in Adults with Asthma or COPD: A Systematic Review and Meta-Analysis

Noe Garin, Borja Zarate-Tamames, Laura Gras-Martin, Raimon Mila, Astrid Crespo-Lessmann, Elena Curto, Marta Hernandez, Conxita Mestres, Vicente Plaza

Summary: We conducted a systematic review and meta-analysis on electronic monitoring devices of inhalers (EMDs) in adult patients with asthma or COPD to examine their characteristics and clinical impact. Our search yielded eight observational studies and ten clinical trials, covering a wide range of clinical outcomes. The meta-analysis showed that adherence to inhalers improved in the EMD group over a three-month period. An exploratory analysis also found an improvement in ACT score. However, other clinical outcomes showed mixed results. Overall, EMDs have the potential to optimize adherence to inhaled therapy and may be of interest for other clinical outcomes.

Comprehensive Observational Study in a Large Cohort of Asthma Patients after Adding LAMA to ICS/LABA

Vicente Plaza, Javier Dominguez-Ortega, Diego Gonzalez-Segura Alsina, Daniele Lo Re, Antoni Sicras-Mainar

Summary: Adding LAMA to LABA/ICS is recommended to improve control in patients with persistent asthma. This study found that triple therapy reduced exacerbations, improved lung function, and decreased healthcare costs compared to dual therapy, particularly in patients requiring systemic corticosteroids for severe exacerbations.

Comparison of post-COVID symptoms in patients with different severity profiles of the acute disease visited at a rehabilitation unit

Jean Claude Perrot, Macarena Segura, Marta Beranuy, Ignasi Gich, Ma Josepa Nadal, Alberto Pintor, Jimena Terra, Eliot Ramirez, Luis Daniel Paz, Helena Bascunana, Vicente Plaza, Ma Rosa Guell-Rous

Summary: This study retrospectively compared the symptoms in post-COVID patients with different severity levels. The results showed that muscle parameters were lower in the ICU group, while patients who did not require ICU admission had worse anxiety and HRQoL scores. Many patients who did not require mechanical ventilation had respiratory muscle weakness.

Javier Dominguez-Ortega, Vicente Plaza, Vicky W. Li, Eileen Fonseca, Luis Cea-Calvo, Ashley Martin, Marta Sanchez-Jareno, Joanne E. Brady, Jonathan Schelfhout

Summary: This study assessed the prevalence of chronic cough in Spanish adults and compared the characteristics between those with chronic cough and those without. The results showed that individuals with chronic cough had poorer health status, poorer mental health, higher healthcare utilization, and lower productivity at work and home compared to those without chronic cough.
