Summary: This study aims to establish benchmark values for adult-to-adult living-donor liver transplantation (LDLT). By analyzing data from 15 high-volume centers, it was found that benchmark LDLT can reduce waitlist mortality, with Asian centers outperforming others and non-benchmark low-volume centers displaying inferior outcomes.

Summary: The study found that approximately 10% of patients who underwent living donor liver transplantation required retransplantation. Patients who received retransplantation had significantly lower survival rates compared to those who only underwent primary transplantation.

Summary: The study demonstrates that simultaneous splenectomy after living donor liver transplantation can prevent small-for-size graft syndrome and improve graft survival rates. It is recommended to perform simultaneous splenectomy for patients with predicted small grafts, portal hypertension, or high portal pressure.

Summary: This study found that the occurrence of SFSS is correlated with the graft recipient weight ratio, MELD score, and intraoperative portal hemodynamics. Preclamping and post-reperfusion portal pressures exhibited high sensitivity and specificity in predicting SFSS.

Summary: This study presents the surgical technique and long-term outcomes of living donor liver transplantations with renoportal anastomosis for patients with complete portal venous occlusion. The standardized RPA technique using an inferior VC cuff connected to the left renal vein creates a safe end-to-end RPA, with decreased complications and normal liver function in surviving patients.

Summary: Hepatic venous outflow obstruction (HVOO) is a rare but critical vascular complication after adult living donor liver transplantation. It can be categorized into anastomotic stenosis, kinking, and intrahepatic stenosis based on morphology and onset. Interventional radiology, such as balloon angioplasty and stent placement, is effective in treating HVOO, but graft survival may be worse in patients with late-onset HVOO despite successful treatment.

Summary: A systematic review comparing adult living donor liver transplantation (LDLT) and deceased donor liver transplantation (DDLT) found that LDLT is associated with improved patient survival, reduced waiting time, and lower MELD score, despite an increased risk of biliary complications. Hepatic artery thrombosis rates did not differ between the two types of transplantation.

Summary: Living donor liver transplantation (LDLT) is more common in Asia than in North America, where living donor kidney transplantation, including paired exchanges for ABO-incompatible pairs, is prevalent. The first liver paired exchange (LPE) in the United States, completed over two consecutive days, was initiated by a non-directed donor and benefited an ABO-incompatible pair. This innovative approach demonstrates the feasibility of LPE in centers with experience in LDLT, offering new possibilities for transplantation in cases of ABO incompatibility.

Summary: ABO incompatible living donor liver transplantation can be successful, but anti-ABO isoagglutinin rebound may lead to antibody-mediated rejection and graft loss. Adding bortezomib to the desensitization regimen can mitigate the risk of isoagglutinin rebound and prevent clinical AMR in patients with high isoagglutinin titers.

Summary: Portal vein complications (PVCs) after adult living donor liver transplantation (LDLT) are potentially lethal, with early PVS showing higher treatment success rates, while late PVT and PVO are refractory to treatment. Tailored treatment according to the time of onset and deformity patterns of PVC is essential.

Summary: Living donor liver transplantation is an effective solution to organ shortage, but ABO incompatibility or inadequate liver volume often prevents potential living donors from donating. Liver paired exchange (LPE) offers a practical solution to overcome these obstacles, and this study reports the world's first LPE case involving pediatric and adult recipients. This approach aims to avoid surgical complexity in the pediatric recipient and improve the graft-to-recipient weight ratio in the adult recipient of two ABO compatible pairs.

Summary: This study conducted a cost-effectiveness analysis of living-donor liver transplantation (LDLT) compared to conservative management (CM) for adult patients using the Japanese nationwide database. The results showed that LDLT was cost-effective for patients with Child-Pugh class C, while it was not cost-effective for patients with Child-Pugh class B.

Summary: The purpose of this study was to evaluate the treatment response, survival, and safety of a novel TACE using a combination of ethanol-Lipiodol mixture and drug-eluting beads in patients with large unresectable HCC. The results demonstrated favorable tumor response rates and control rates, as well as longer survival time for the patients. Therefore, the combined DEB-TACE treatment can be an effective option for patients with large unresectable HCC.
