Summary: The study evaluated the effects of NR1I2 and ABCB1 genetic polymorphisms on everolimus pharmacokinetics in Japanese renal transplant patients. The results showed significant correlations between different genotypes, gender, age, and liver function with the dose-adjusted AUC(0-12) of everolimus. Therefore, age and liver function should be considered when evaluating dose reductions for everolimus.

Summary: Post-transplant lymphoproliferative disorders (PTLD) are heterogenous lymphoproliferative disorders that develop as a consequence of immunosuppression in transplant recipients. We sought to determine if subtypes of PTLD correlated with different outcomes. Our retrospective review of PTLD occurring in pediatric heart transplant recipients found that infant recipients were less likely to develop PTLD compared to older children. Infants who developed PTLD were diagnosed later and had a lower rate of more aggressive PTLD. Patients with more aggressive PTLD had significantly lower overall survival than patients with low-grade PTLD.

Our analysis of 37 patients receiving Everolimus showed a patient survival rate of 97% and a graft survival rate of 84%. Summary: This study evaluated the safety and effectiveness of kidney transplantation from pediatric deceased donors with acute kidney injury (AKI). The results showed that pediatric AKI kidneys had better post-transplant outcomes compared to adult AKI kidneys. Therefore, this donor pool should be utilized to minimize waiting-list-related mortalities.

Summary: This study retrospectively analyzed the data of 28 kidney transplant recipients who received everolimus-based quadruple therapy for difficult-to-treat rejections. The results showed a 5-year death-censored graft survival rate of 79% and a response rate of 43% in these patients. The 5-year patient survival rate was 94%. Despite some patients discontinuing treatment due to adverse events, this treatment approach may be effective for kidney transplant recipients with difficult-to-treat rejections.

Strict glycemic control, tailored immunosuppressive regimens, and lifestyle interventions are crucial in mitigating post-transplant diabetes complications.

Summary: International consensus guidelines do not include pediatric heart transplant recipients in the vaccination recommendations for live-attenuated varicella (VZV) vaccine due to insufficient evidence. However, VZV infection can be serious or fatal for these patients. A study reported a high early seroconversion rate and mild adverse events in pediatric heart transplant patients who received VZV vaccination, suggesting safe vaccination is possible.
