Rate of Tumor Growth Predicts Recurrence of Hepatocellular Carcinoma After Liver Transplantation in Patients Beyond Milan or UCSF Criteria

Background. It is likely that some patients whose tumor burdens exceed the current transplant criteria have favorable tumor biology, and that these patients would have low risk of tumor recurrence after liver transplantation (LT). To assess the rate of tumor growth as selection criteria for LT in patients with hepatocellular carcinoma (HCC). Methods. We identified all patients who underwent LT for HCC in our institution from 2002 to 2008. Total tumor volume (TTV) was calculated as the sum of the volumes of all tumors on pretransplantation imaging [(4/3)pi(3), where r is the maximum radius of each HCC]. The rate of tumor growth was calculated as per-month change in TTV on sequential pretransplantation imaging before any locoregional therapy. A Kaplan-Meier plot was constructed and Cox regression analysis performed. Results. Ninety-two patients were included in the study. The median follow-up was 19.5 (range 10.7-30.7) months during which 12 patients (13%) experienced recurrence of HCC. Twenty-four patients (26%) had HCC beyond the Milan criteria, and the overall survival rate of the entire group was 72%. Higher pre-LT alpha-fetoprotein (hazard ratio [HR] 1.01; P = .001), poorly differentiated tumors (HR 13; P = .039), the presence of microvascular invasion (HR 7.9; P = .001), higher TTV (HR 1.03; P < .001), and faster tumor growth (HR 1.09; P < .001) were significantly associated with the risk of recurrence. A cutoff value of tumor growth of 1.61 cm(3)/mo was chosen on the basis of the risk of recurrence with the use of a receiver operating characteristic curve. Patients beyond the Milan criteria with tumor growth <1.61 cm(3)/mo experienced less recurrence (11% vs 58%; P = .023) than those beyond the Milan criteria with tumor growth >1.61 cm(3)/mo. Similarly, rate of tumor growth predicted HCC recurrence in those beyond the University of California of San Francisco (UCSF) criteria. Conclusions. Patients with slowly growing tumor who would be currently excluded from LT because tumor burden exceeds traditional Milan and UCSF criteria may have a favorable posttransplantation outcome.

≥3 Nonadvanced Adenomas are More Common in the Era of Contemporary Colonoscopy and Not Associated With Metachronous Advanced Neoplasia

Summary: The study compared baseline adenoma characteristics between patients undergoing colonoscopy in a historic versus contemporary cohort and found an increase in the proportion of patients with >= 3 nonadvanced adenomas in the contemporary cohort. However, follow-up colonoscopies showed that these patients did not have a significantly higher risk of metachronous advanced neoplasia compared to low-risk adenoma patients.

Use of Bedside Ultrasound to Assess Muscle Changes in the Critically Ill Surgical Patient

Beyond tobacco - the secondary impact of substance misuse in chronic obstructive lung disease

Summary: This study aimed to determine the indirect impact of substance misuse on disease severity, healthcare utilization, and healthcare costs in patients with chronic obstructive lung disease. Using data from the National Readmissions Database, the study found that substance misuse was associated with an increase in hospital readmission rate and higher healthcare costs. Targeting substance misuse in this patient population has the potential for significant cost savings to the healthcare system.

Alcohol use disorder and healthcare utilization in patients with chronic asthma and obstructive lung disease

Summary: The study revealed that patients with alcohol use disorder (AUD) are more likely to experience respiratory failure and require mechanical ventilation in the emergency room when admitted with asthma or COPD. Management of migraine in the emergency department: Findings from the 2010-2017 National Hospital Ambulatory Medical Care Surveys

Summary: Analysis of NHAMCS data from 2010 to 2017 showed a positive trend in the medical management of migraine in US emergency departments, with a yearly decline in opioid prescriptions. During this period, ketorolac and metoclopramide were more frequently prescribed. Factors associated with increased opioid administration included female sex, older age, white race, higher pain scores, and Medicare or private insurance as primary payment source.

A spatiotemporal analysis of opioid poisoning mortality in Ohio from 2010 to 2016

Diabetes Mellitus Is Associated with Worse Outcome in Patients Hospitalized for Asthma

Summary: Asthmatic patients with diabetes mellitus (DM) had increased hospital length of stay, cost, and risk for readmission during asthma-related hospitalizations. They were also more likely to develop nonrespiratory complications during their hospital stay compared to patients without diabetes. However, the risk of mortality was similar between the two groups.

This study examined the factors associated with nonutilization and found that deceased donors from higher distressed communities were more likely to have their kidneys not utilized. The study also found regional variations in kidney nonutilization rates and minimal impact on recipient outcomes.

Living Well with Chronic Pain: Integrative Pain Management via Shared Medical Appointments

Mediterranean diet adherence and metabolic syndrome in US adolescents

Summary: The study revealed that adherence to the Mediterranean diet was very low among US adolescents, with Mexican American youths showing slightly higher adherence. Higher income was associated with greater adherence, but adherence to the Mediterranean diet did not seem to be related to metabolic syndrome.
