Hyperuricemia influences chronic cyclosporine nephropathy

Background. The observation that long-standing hyperuricemia is associated with chronic tubulointerstitial disease, afferent arteriolopathy, intrarenal vasoconstriction, and increased vascular resistance raises the hypothesis that hyperuricemia might contribute to chronic cyclosporine (CsA) nephropathy. The aim of the present study was to investigate the effect of hyperuricemia on chronic CsA nephropathy. Methods. Patients who were treated with CsA-based immunsuppressive regimens and underwent a renal biopsy were enrolled in this case-control study. We retrospectively obtained posttransplant baseline serum creatinine, uric acid (UA), mean serum UA, and creatinine values 3 months prior to biopsy. CsA trough levels, mean blood pressure, diuretic and antihypertensive treatment were recorded. Biopsy specimens showing CsA nephropathy (n = 34) were revaluated by a pathologist to score CsA nephropathy according to recent quantitative criteria for calcineurin inhibitor arteriolopathy as proposed by M.J. Mihatsch. Results. As compared with the non-CsA nephropathy group, recipient and donor ages, donor origin and cold ischemia times were similar for the CsA nephropathy group (P > .05). Mean CsA doses, CsA trough (CO), and C-2 levels were not different between the groups (P > .05). Systolic and diastolic blood pressure, glomerular filtration rate, diuretic usage, and antihypertensive treatment were also similar in CsA nephropathy and non-CsA nephropathy groups (P > .05). Mean serum UA level within 3 months prior to biopsy in the CsA nephropathy and non-CsA nephropathy groups were 7.5 +/- 1.4 mg/dL versus 5.7 1.4 mg/dL, respectively (P < .001). Conclusion. Hyperuricemia seems to exacerbate CsA-induced nephropathy.

Summary: This study aims to investigate the diagnostic performance of MRI in predicting the pathologic stage of LARC after neoadjuvant CRT and the role of MRI in selecting patients with ypCR. The results showed moderate accuracy and sensitivity in predicting ypT and N stages, mainly due to underestimating tumor stage and overestimating nodal status. Furthermore, high specificity and NPV but low sensitivity were observed in predicting the complete response.

Long-Term Outcomes of Patients With Amyloidosis Following Kidney Transplant

Summary: This study compared the graft and patient survival rates and complications following kidney transplant in amyloidosis patients and patients with kidney disease due to other causes. The results showed that amyloidosis patients had lower patient survival rates, but similar graft failure and acute rejection rates compared to patients with other diseases.

Summary: This study investigated the distribution of causes and renal survival of granulomatous interstitial nephritis in a tertiary care hospital in Western Turkey. The majority of cases were caused by vasculitis, and renal outcomes were unfavorable despite treatment. The study suggests the need for multicenter studies to determine the etiology and outcome of granulomatous interstitial nephritis at different geographic locations.

Associations between nutritional factors and excessive daytime sleepiness in older patients with chronic kidney disease

Summary: Excessive daytime sleepiness (EDS) is associated with malnutrition in older patients with chronic kidney disease (CKD), highlighting the importance of evaluating EDS and nutritional status together in clinical practice. Future studies are needed to determine the direction of the association between malnutrition and EDS and to evaluate if dietary intervention can improve EDS.

The prevalence of Sjogren's syndrome and sicca symptoms in patients with systemic sclerosis and alpha-smooth muscle actin expression in biopsy specimens from minor salivary glands

Summary: This study investigated the prevalence of sicca symptoms and secondary Sjogren's syndrome (SjS) in patients with systemic sclerosis (SSc), as well as evaluated the expression of alpha-smooth muscle actin (alpha-SMA) in minor salivary gland (MSG) specimens. Results showed that sicca symptoms were common among SSc patients, with nearly one-third fulfilling the criteria for SjS, particularly in the limited subtype. Older age and positive anti-SS-A antibodies were identified as predictors for SjS, and glandular fibrosis was significant but alpha-SMA staining was rare in patients with SSc.
