Summary: This study aimed to investigate the outcomes of lung and liver transplantation using NRP in cDCD donors and compare them with transplants using DBD grafts. The results showed that simultaneous transplantation of lungs and livers from cDCD donors using NRP offers similar outcomes to transplants using DBD grafts.

Summary: The feasibility of a donation after circulatory death (DCD) heart transplantation protocol using cardiopulmonary bypass (CPB) for normothermic regional reperfusion (NRP) was evaluated in this pilot study. The study showed excellent outcomes of DCD heart transplantation, indicating the potential for wider adoption of the DCD protocol and increased availability of donor hearts in the United States.

Summary: Thoracoabdominal normothermic regional perfusion (TA-NRP) is a method used in the United States to recover cardiothoracic allografts from donors after circulatory death (DCD). Limited data on lung transplants using TA-NRP has prompted a national retrospective review. The study found that recipients of TA-NRP DCD transplants had a lower likelihood of prolonged ventilation and similar outcomes to direct recovery DCD transplants.

Summary: This review provides an update on normothermic regional perfusion (NRP) and its logistics, technique, outcomes, as well as ethical issues. There is renewed interest in using NRP to improve the quality and availability of organs from donation after circulatory death (DCD) donors. Evidence suggests that NRP can be conducted in a practical and cost-efficient manner, with better organ yield compared to standard direct procurement and perfusion (DPP). However, the ethical debate surrounding NRP, particularly thoraco-abdominal NRP (TA-NRP), needs to be resolved before wider acceptance.

Summary: In this study, a novel oxygenated hypothermic machine perfusion (HMP) system was compared to static cold storage (SCS) in heart transplantation (HTX). The results showed that hearts preserved with HMP exhibited increased biventricular contractility post-HTX and required less dobutamine for maintaining cardiac output. Additionally, HMP groups had lower levels of myocardial damage. Therefore, DCD HTX can be successfully performed using DPP followed by preservation with HMP.

Summary: An essential method for collecting more abdominal donor organs is controlled donation after circulatory death (cDCD). This study presents a case where transesophageal echocardiography (TEE) was used to effectively procure viable abdominal organs from a cDCD donor in the shortest time frame. TEE use leads to a more efficient and effective abdominal normothermic regional perfusion (A-NRP) procedure with limited complications, allowing observation of circulation in both thoracic and abdominal organs with one fast exam. Although prospective studies are needed, TEE could potentially become a standard practice for all DCD organ retrieval procedures.
