Summary: Paired exchange liver transplantation is an evolving strategy to overcome ABO blood group incompatibility and other barriers in living donor liver transplantation. This study analyzed the experience with liver paired exchange program over the past 9 years and found that with careful attention to ethical and logistical issues, the program can expand the living donor liver pool and facilitate a greater number of living donor liver transplantations. The study involved simultaneous performance of 4 major operations, which posed a challenging task for the transplant team. However, donor survival rate was 100% and baseline clinical parameters and postoperative complications were comparable between different groups.

The patient underwent surgical re-exploration due to bilious output from a surgical drain, where a Duct of Luschka was found and oversewn. Summary: By analyzing the outcomes of patients undergoing liver transplantation (LT) due to bile duct injury (BDI), it was found that although the postoperative mortality is high, LT remains the only possibility of a cure and has acceptable long-term outcomes. Early referral to a tertiary care center is crucial for avoiding long-term complications of BDI. Post-cholecystectomy BDI accounted for 3.4% of all LT in our center.

Plasma D-dimer levels were independently linked to ascites severity in a separate cohort of patients.

Summary: Patients with liver disease often experience complex changes in their blood coagulation system, including decreased platelet count and altered levels of coagulation-related proteins. Conventional coagulation tests do not accurately reflect procoagulant changes, but novel tests can better reflect the overall hemostatic changes. Despite a rebalanced hemostatic status, liver disease patients have a more fragile hemostatic system and are at risk for both bleeding and thrombosis. Many hemostatic complications in liver disease are not directly related to hemostatic failure, highlighting the importance of identifying the underlying cause for appropriate treatment.
