In Europe, living organ donation (LOD) is increasingly accepted as a valuable solution to overcome the organ shortage. However, considerable differences exist between European countries regarding frequency, practices and acceptance of donorrecipient relations. As a response, the Coordination Action project Living Organ Donation in Europe (www.eulod.eu), funded by the Seventh Framework Programme of the European Commission, was initiated. Transplant professionals from 331 European kidney and liver transplant centres were invited to complete an online survey on living kidney donation (LKD) and living liver donation (LLD). In total, 113 kidney transplant centres from 40 countries and 39 liver transplant centres from 24 countries responded. 96.5% and 71.8% performed LKD and LLD respectively. The content of the medical screening of donors was similar, but criteria for donor acceptance varied. Few absolute contraindications for donation existed. The reimbursement policies diverged and the majority of the donors did not get reimbursed for their income loss during recovery. Large discrepancies were found between geographical European regions (the Eastern, the Mediterranean and the North-Western). As a result of this survey we suggest several recommendations to improve quality and safety of LOD in Europe.

Predicting Falls in Nursing Homes: A Prospective Multicenter Cohort Study Comparing Fall History, Staff Clinical Judgment, the Care Home Falls Screen, and the Fall Risk Classification Algorithm

Ellen Vlaeyen, Joris Poels, Uschi Colemonts, Lien Peeters, Greet Leysens, Kim Delbaere, Eddy Dejaeger, Fabienne Dobbels, Koen Milisen

Summary: The study evaluated and compared the predictive accuracy of different screening methods for falls in nursing home residents. Fall history, FRiCA, and a CaHFRiS score of >= 4 showed better sensitivity and negative predictive value compared to clinical judgment. Specificity varied among the screening methods, with different positive predictive values at different time points.

Summary: The D-PPAC and C-PPAC instruments showed strong internal consistency and construct validity in stable COPD patients, correlating with dyspnea, health-related quality of life and exercise capacity. Difficulty scores improved after pharmacological treatment and pulmonary rehabilitation, while amount scores improved after behavioral physical activity interventions.

Summary: Conversion from twice daily to once daily tacrolimus in lung transplant patients requires close monitoring of tacrolimus trough levels. Post-conversion, once daily tacrolimus is safe with respect to renal function, metabolic parameters and allograft function, and improves patient adherence.

Patients' and healthcare professionals' perspectives on the idiopathic pulmonary fibrosis care journey: a qualitative study

Anouk Delameillieure, Fabienne Dobbels, Sarah Vandekerkhof, Wim A. Wuyts

Summary: This study aimed to shed light on the care trajectory of patients with Idiopathic Pulmonary Fibrosis (IPF) based on the perspectives of patients and healthcare professionals. The findings identified pitfalls and suggestions for improvement at individual patient and care team levels, focusing on team-based care, pro-active follow-up, self-management support, and challenges related to eHealth tools. The importance of continuous training for professionals and collaboration with community-based organizations was also emphasized.

Development of an integrated model of care for allogeneic stem cell transplantation facilitated by eHealth-the SMILe study

Lynn Leppla, Anja Schmid, Sabine Valenta, Juliane Mielke, Sonja Beckmann, Janette Ribaut, Alexandra Teynor, Fabienne Dobbels, Nathalie Duerinckx, Robert Zeiser, Monika Engelhardt, Sabine Gerull, Sabina De Geest

Summary: This paper describes the development and application of an integrated care model for allogeneic stem cell transplantation facilitated by eHealth. The SMILe intervention consists of four self-management modules delivered partly face-to-face and partly via a mobile app. The new care model is currently being tested in an effectiveness-implementation trial.

Is degree of chronic illness management in heart transplant centers associated with better patient survival? Findings from the intercontinental BRIGHT study

Maan Isabella Cajita, Kris Denhaerynck, Lut Berben, Fabienne Dobbels, Johan Van Cleemput, Maria Crespo-Leiro, Jan Van Keer, Alain Jean Poncelet, Cynthia Russell, Sabina De Geest

Summary: The study found that heart transplant centers with high-degree chronic illness management had significantly higher mean survival rates at 1 year post-transplantation compared to centers with low-degree chronic illness management, but no differences were observed at other time points. Patients at high-degree chronic illness management centers had lower odds of moderate to severe drinking. There were no significant associations between the degree of chronic illness management and other recommended health behaviors.

Understanding the challenges to investigating and prosecuting organ trafficking: a comparative analysis of two cases

Frederike Ambagtsheer

Summary: The global proliferation of human organ trafficking is not being met with a proportional number of prosecutions, with little research exploring the challenges and no existing studies using a criminal justice framework to explain these issues. Two cases, the Netcare case in South Africa and the Medicus case in Kosovo, involved globally operating criminal networks but faced different obstacles in investigation and prosecution. Recommendations for improving enforcement include better identification of suspicious activity, enhanced cross-border collaboration, and stronger protection for whistleblowers.

Care programs and their components for patients with idiopathic pulmonary fibrosis: a systematic review

Anouk Delameillieure, Sarah Vandekerkhof, Bastiaan Van Grootven, Wim A. Wuyts, Fabienne Dobbels

Summary: This systematic review identified 13 new care models or components developed for patients with idiopathic pulmonary fibrosis (IPF), focusing on team-based and multidisciplinary care provision and education. The most commonly described outcomes were related to health-related quality of life. Given the high demand for integrated care, developing and implementing new care models for patients with IPF should be a priority for research and clinical care.
