Semiquantitative interpretation of anticardiolipin and antiβ2glycoprotein I antibodies measured with various analytical platforms: Communication from the ISTH SSC Subcommittee on Lupus Anticoagulant/Antiphospholipid Antibodies

Arne Vandevelde, Walid Chayoua, Bas de Laat, Jean-Christophe Gris, Gary W. Moore, Jacek Musial, Stephane Zuily, Denis Wahl, Katrien M. J. Devreese

Summary: The study evaluated the traditional 40/80-unit thresholds used for categorization of antiphospholipid antibodies in different analytical systems, and found that alternative thresholds can improve consistency in interpretation. By using different thresholds, the positivity levels of antiphospholipid antibodies can be more accurately assessed.

Monitoring of heparin therapy beyond the anti-Xa activity assay: Evaluation of a thrombin generation assay

Pauline Vermeiren, Arne Vandevelde, Harlinde Peperstraete, Katrien M. J. Devreese

Summary: This study compared the effectiveness of global coagulation assays, including thrombin generation assay (TGA), with the anti-Xa assay for monitoring heparin therapy. The results showed that the TGA may not be sensitive enough for low concentrations of low molecular weight heparin (LMWH), but it could be a useful additional tool for monitoring heparin therapy in heparin-resistant patients.

Laboratory Diagnosis of Antiphospholipid Syndrome: Insights and Hindrances

Arne Vandevelde, Katrien M. J. Devreese

Summary: The diagnosis and risk stratification of antiphospholipid syndrome (APS) are complex and require various laboratory tests. Efforts to standardize and optimize these tests are ongoing to improve diagnostic accuracy and repeatability. There are challenges in laboratory testing, such as interference by anticoagulation therapy for LAC detection and high variability due to different reagents and detection techniques.

Qualitative risk assessment of homogeneity, stability, and residual concentrations of antimicrobials in medicated feed and drinking water in pig rearing

Despoina Georgaki, Femke Vandael, Helena Cardoso de Carvalho Ferreira, Maria Eleni Filippitzi, Patrick De Backer, Mathias Devreese, Jeroen Dewulf, Siska Croubels

Summary: This study conducted a qualitative risk assessment to estimate the risks associated with reduced homogeneity and stability, as well as increased residual concentrations of antimicrobial drugs in medicated feed and drinking water on pig farms. Factors such as human error during the preparation of medicated feed and dosing pump type for medicated drinking water were found to pose moderate to high risks. It is crucial to identify and understand these factors in order to ensure the effectiveness and safety of antimicrobial drug treatment.

Application of the thrombin generation assay in patients with antiphospholipid syndrome: A systematic review of the literature

Rachel Gehlen, Arne Vandevelde, Bas de Laat, Katrien M. J. Devreese

Summary: This systematic review examined if the thrombin generation (TG) assay could be useful for the diagnosis and risk assessment of antiphospholipid syndrome (APS). The findings suggest that APS patients have increased aPC resistance compared to healthy and thrombotic controls, but its diagnostic and prognostic value compared to current strategies is unclear. More research is needed to determine the potential added value of other TG parameters in APS diagnosis.

Summary: Currently, the added value of antiphosphatidylserine/prothrombin antibodies testing in the diagnosis of obstetric antiphospholipid syndrome seems limited compared with other antibodies. However, it may be useful in specific subsets of patients.

Direct Oral Anticoagulant removal by a DOAC filter: Impact on lupus anticoagulant testing - Evaluation on spiked and patient samples

Eleni A. Linskens, Pieter De Kesel, Katrien M. J. Devreese

Summary: This study evaluated the effectiveness of DOAC Filter (DF) in removing DOAC interference in LAC testing. DF was found to effectively eliminate DOAC interference in LAC testing and had an impact on the LAC assay results. Therefore, DF should be used for filtration when performing DOAC measurements.

Adhesive properties of plasma-circulating and platelet-derived microvesicles from healthy individuals

E. O. Artemenko, S. I. Obydennyi, K. S. Troyanova, G. A. Novichkova, D. Y. Nechipurenko, M. A. Panteleev

Summary: In this study, we investigated the adhesive properties of circulating plasma-derived and platelet-derived microvesicles (MVs) and their potential role in thrombus growth. The results showed that both types of MVs weakly bound fibrinogen but did not stably adhere to immobilized fibrinogen. Moreover, the presence of these MVs did not significantly affect thrombus formation. Therefore, MVs present in the plasma of healthy individuals are unlikely to directly contribute to thrombus formation under arterial flow conditions.

Exploration of rotational thromboelastometry (ROTEM) to characterize the coagulation profiles of newly diagnosed pediatric leukemia patients

Summary: This study analyzed and evaluated leukemia-associated coagulopathy in newly diagnosed pediatric leukemia patients using CCTs and ROTEM. The results showed that fibrinogen and platelets played a significant role in causing coagulopathies in this population. This suggests the utility of using CCTs and ROTEM in the pediatric population to evaluate hemostatic function and guide blood product replacement.

Thrombotic triad in microgravity

Mohammad M. Elahi, Alexandra N. Witt, Edward L. G. Pryzdial, Paul B. McBeth

Summary: Thrombotic disease may be underdiagnosed in microgravity conditions, and the underlying factors are still poorly understood. A case of internal jugular vein thrombosis was recently diagnosed in a low-risk female astronaut, and six out of ten additional crew members showed risk factors for jugular venous flow. Observations in space and studies on Earth suggest that microgravity affects blood flow, coagulation, and vascular function.

Driver gene KRAS aggravates cancer-associated stroke outcomes

Haomin Yan, Tsutomu Sasaki, Yasufumi Gon, Kumiko Nishiyama, Hideaki Kanki, Hideki Mochizuki

Summary: The incidence of cancer-associated stroke has increased with the prolonged survival times of cancer patients. In this study, the researchers focused on the driver gene KRAS and found that KRAS mutations exacerbated neurological deficits after stroke and promoted tumor growth. They also identified the upregulation of pro-inflammatory responses and the promotion of STAT3 phosphorylation in mice with KRAS mutations. Additionally, the researchers found that colorectal cancer patients with stroke onset within 3 months after cancer diagnosis had a worse prognosis.

Trimethylamine-N-oxide (TMAO) and basic fibroblast growth factor (bFGF) are possibly involved in corticosteroid resistance in adult patients with immune thrombocytopenia

Lei Liu, Huifang Xu, Jian Wang, Haiyan Wang, Saisai Ren, Qian Huang, Mingyan Zhang, Hui Zhou, Chunyan Yang, Lu Jia, Yu Huang, Hao Zhang, Yanling Tao, Ying Li, Yanan Min

Summary: This study investigated the gut microbiota metabolites and cytokines, chemokines, and growth factors in patients with ITP and found their association with treatment responsiveness. The findings suggest that imbalanced secretion of cyto(chemo)kins/growth factors and inadequate levels of TMAO are characteristic of patients with ITP.
