Early diagnosis and treatment are crucial for improving patient outcomes. The step-up approach, which includes endoscopic interventions, has shown to decrease multiple-organ failure and intervention-related complications. However, the optimal sequence and timing of interventions remain controversial.

The study found that endoscopic resection had a lower total cumulative cost and comparable oncologic outcomes as surgical resection. Summary: Colorectal malignancy is a common cancer with high mortality rates due to metastases. Surgical resection is the primary treatment, but only a small percentage of patients are suitable for curative therapy. Minimally invasive interventional radiological techniques have improved the treatment options for these patients. This review focuses on the role of procedures such as portal vein embolization, radioembolization, and ablation, as well as conventional chemoembolization and chemoembolization with irinotecan-loaded drug-eluting beads. Yttrium-90 microspheres radioembolization has emerged as salvage therapy for unresectable and chemo-resistant metastases.

Summary: The survey revealed that a large portion of surgeons admitted to being afraid of working during the COVID-19 pandemic, with implications for their attitude towards surgical practice. The study also indicated that fear of contracting the disease, fear for their own life, and fear of transmitting the disease to family members were prevalent among surgeons surveyed. Additionally, there was a significant difference in the level of personal protective equipment provided at the workplace between those who were afraid of contracting COVID-19 and those who were not.

Summary: Home parenteral nutrition (HPN) may improve survival in patients with malignant bowel obstruction, but certain clinical and laboratory markers need to be considered to accurately select patients who would benefit from HPN. Discontinuation of chemotherapy, anemia, severe hypoalbuminemia, and water retention may be correlated with shorter survival and in these cases, routine initiation of HPN may not be beneficial. The decision to initiate HPN should be made in conjunction with continuing or beginning chemotherapy.

Summary: Total parenteral nutrition (TPN) is a life-saving nutritional therapy used when enteral nutrition is not possible or sufficient. However, it can cause parenteral nutrition-associated liver damage (PNALD) as a side effect. Risk factors for PNALD in parenterally fed patients include short bowel syndrome, bacterial overgrowth and translocation, disturbances in hepatobiliary circulation, lack of enteral nutrition, and the use of inappropriate fat mixtures. This review explores the pathogenesis of PNALD and discusses possible options for prevention.
