Robotic versus laparoscopic anterior resection of sigmoid colon cancer: comparative study of long-term oncologic outcomes

Robotically assisted colon resection is a new type of surgery for colon cancer. However, the evidence is inadequate for the general adaptation of robotic colon surgery. This study aimed to show the oncologic and perioperative clinical results of robotically assisted anterior resection (R-AR) compared with those of laparoscopically assisted anterior resection (L-AR) for sigmoid colon cancer. A total of 180 patients (sigmoid colon cancer stages 1-3) were assigned to receive either R-AR (n = 34) or L-AR (n = 146) between April 2006 and September 2008. Patient characteristics, perioperative clinical results, and long-term oncologic outcomes were compared between the two groups. The patient characteristics did not differ significantly between the two groups. The mean operation time was 217.6 +/- A 70.7 min for L-AR versus 252.5 +/- A 94.9 min for R-AR (p = 0.016). The total postoperative complication rate was 10.3 % for R-AR versus 5.9 % for L-AR (p = 0.281). The 3-year overall survival rate for all the patients was 93.4 % for L-AR versus 92.1 % for R-AR (p = 0.723). The 3-year overall survival rate was 100 % for both L-AR and R-AR in stage 1, 95.5 % for L-AR versus 100 % for R-AR (p = 0.386) in stage 2, and 88.4 % for L-AR versus 72.9 % (p = 0.881) for R-AR in stage 3. In this study, R-AR showed safety and feasibility in terms of perioperative clinical and long-term oncologic outcomes. However, the advanced technologies of R-AR did not translate into better long-term oncologic outcomes compared with L-AR.

Summary: This study retrospectively analyzed the medical records of patients who underwent surgery for appendiceal mucocele between 2010 and 2018, comparing the perioperative and short-term outcomes of single-incision laparoscopic surgery (SILS) and conventional laparoscopic surgery (CLS), as well as evaluating the oncological safety of SILS. The results showed that SILS for appendiceal mucocele is feasible and safe perioperatively and in the short-term, with favorable oncological outcomes.

Long-term oncologic outcomes of single-incision laparoscopic surgery for colon cancer

Summary: This study compared the long-term postoperative and oncologic outcomes as well as perioperative outcomes between single-incision laparoscopic surgery (SILS) and conventional laparoscopic surgery (CLS) for colon cancer. The results showed that SILS appeared to be safe and effective compared to CLS, with shorter total incision length, less postoperative pain, and similar rates of incisional hernia. The 7-year overall and disease-free survival rates were also similar between the two groups.

Albumin-myosteatosis gauge as a novel prognostic risk factor in patients with non-metastatic colorectal cancer

Summary: This study aimed to examine the individual and synergistic effects of different prognostic markers related to skeletal muscle quality and serum albumin levels in patients with colorectal cancer (CRC). The albumin-myosteatosis gauge (AMG) was found to be a meaningful predictor of survival, with superior prognostic value compared to skeletal muscle index (SMI), skeletal muscle radiodensity (SMD), and albumin alone. Further studies are needed to determine their significance in different ethnic groups.

Summary: This study aimed to investigate the effects of postoperative exercise on the recovery of colorectal cancer patients. The results indicated that patients in the postoperative exercise group had shorter hospital stay, greater readiness for discharge, and increased muscle mass. Therefore, postoperative inpatient exercise may facilitate faster recovery and discharge after colorectal cancer surgery.

Neoadjuvant chemotherapy versus upfront surgery as the initial treatment for patients with resectable, synchronous colorectal cancer liver metastases

Summary: Although perioperative outcomes and overall survival were similar between neoadjuvant chemotherapy (NAC) and upfront surgery-treated patients, better postrecurrence survival was observed in patients with NAC. Additionally, NAC may benefit patients with worse prognoses; therefore, physicians should consider patient disease risk before initiating treatment to identify patients who are most likely to benefit from chemotherapy.

Analgesic effects of combined transversus abdominis plane block and intramuscular electrical stimulation in patients undergoing cytoreductive surgery followed by hyperthermic intraperitoneal chemotherapy: a randomized controlled trial

Summary: This study evaluated the analgesic efficacy of a four-quadrant transversus abdominis plane (4QTAP) block and a combination of 4QTAP block with needle electrical twitch and intramuscular electrical stimulation (NETOIMS) in patients undergoing cytoreductive surgery (CRS) followed by hyperthermic intraperitoneal chemotherapy (HIPEC). The results showed that the combination of 4QTAP block with NETOIMS provided better pain control and improved functional restoration and quality of recovery in patients.

Prediction of Microsatellite Instability in Colorectal Cancer Using a Machine Learning Model Based on PET/CT Radiomics

Summary: This study investigated the feasibility of using preoperative 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/ computed tomography (CT) radiomics with machine learning to predict microsatellite instability (MSI) status in colorectal cancer (CRC) patients. A predictive model incorporating PET radiomic features, called rad_score, was established and evaluated for its ability to predict MSI status. The rad_score showed similar predictive performance in both the training and test sets, and it was found to be an independent predictor of MSI status in CRC. Furthermore, the rad_score outperformed conventional PET parameters in predicting MSI status.
