Laparoscopic total mesorectal excision (TME) is associated with a steep learning curve, but the learning curve for robotic TME is unknown. This study aimed to evaluate the learning curve for robotic TME. Between November 2004 and April 2009, 80 patients underwent robotic TME performed by a single surgeon. The operative experience was divided into two groups: group 1 (the first 40 cases) and group 2 (the subsequent 40 cases). Patient demographics, operative characteristics, and morbidities were compared. The two patient populations selected did not differ statistically in age, body mass index (BMI), preoperative risk assessment, stage, preoperative chemoradiotherapy, or tumor location. The mean operative times in group 1 (310 min) and group 2 (297 min) were similar (p = 0.55), and the mean robotic TME time did not differ between the two groups (60 vs. 64 min; p = 0.65). In addition, the operative times did not improve during the course of the study. There were no differences in EBL, margin status, or number of lymph nodes harvested. Furthermore, there were no differences in conversion rate, time to resumption of diet, length of hospital stay, or postoperative complications. Robot-assisted TME may attenuate the learning curve for laparoscopic rectal cancer resection. Further studies are necessary to establish the role of robotic surgery in minimally invasive rectal operations.

Resolvin D1 suppresses inflammation-associated tumorigenesis in the colon by inhibiting IL-6-induced mitotic spindle abnormality

Summary: The research reveals a decreased level of RvD1 in blood from colorectal cancer patients and mice with inflammation-induced colon cancer, and demonstrates that RvD1 can protect against colorectal carcinogenesis by suppressing IL-6-mediated chromosomal instability. Additionally, RvD1 inhibits the STAT3 signaling pathway by blocking the IL-6 receptor, suggesting a potential novel function of RvD1 as an IL-6R antagonist that may contribute to inhibiting tumorigenesis.

Effect of mistletoe extract on tumor response in neoadjuvant chemoradiotherapy for rectal cancer: a cohort study

Jeong-Heum Baek, Youngbae Jeon, Kyoung-Won Han, Dong Hae Jung, Kyung-Ok Kim

Summary: The study evaluated the effect of mistletoe extract on tumor responses in rectal cancer patients undergoing neoadjuvant chemoradiotherapy. Patients treated with mistletoe extract showed better outcomes in terms of tumor responses, indicating potential efficacy in improving oncologic outcomes. Further prospective and randomized studies are needed to confirm and extend these results.

JNK-mediated Ser27 phosphorylation and stabilization of SIRT1 promote growth and progression of colon cancer through deacetylation-dependent activation of Snail

Yeon-Hwa Lee, Su-Jung Kim, Xizhu Fang, Na-Young Song, Do-Hee Kim, Jinyoung Suh, Hye-Kyung Na, Kyung-Ok Kim, Jeong-Heum Baek, Young-Joon Surh

Summary: SIRT1 and its phosphorylated form at serine 27 are upregulated in colon cancer patients' tissues and cells. The phosphorylated form of SIRT1 regulates the activity of Snail and transcription of interleukin by deacetylation, promoting the proliferation and tumor formation of colon cancer cells.

Lipid raft-disrupting miltefosine preferentially induces the death of colorectal cancer stem-like cells

So-Yeon Park, Jee-Heun Kim, Jang-Hyun Choi, Choong-Jae Lee, Won-Jae Lee, Sehoon Park, Zee-Yong Park, Jeong-Heum Baek, Jeong-Seok Nam

Summary: This study highlights the important role of lipid rafts (LRs) in colorectal cancer, where an LR-disrupting alkylphospholipid drug can suppress CSC populations and their self-renewal activities. By inhibiting the LR/CHEK1 axis, the drug forces CSCs to enter inappropriate mitosis, leading to catastrophic cell death.

Aberrant activation of the CD45-Wnt signaling axis promotes stemness and therapy resistance in colorectal cancer cells

Summary: This study identified an increased expression of CD45 in CRC (EpCAM+) epithelial cells after CRT treatment. High CD45 expression promoted CRC cell survival upon 5-fluorouracil or radiation treatment, while CD45 depletion sensitized CRC cells to CRT. CD45 was found to be preferentially expressed in cancer stem-like cells (CSCs) and played a crucial role in CSC functions such as cancer initiation, repopulation, and metastasis.
