Endoscopic submucosal dissection for metachronous tumor in the remnant stomach after distal gastrectomy

Endoscopic submucosal dissection (ESD) of tumor in the upper part of the stomach is technically difficult and has a high complication rate. We compared the feasibility and safety of ESD for metachronous tumor found in the remnant stomach after distal gastrectomy with ESD for tumor in the upper third of the intact stomach. We retrospectively compared clinical outcome and safety of 13 lesions (11 early gastric cancers (EGCs) and 2 dysplasias) in the remnant stomach and 30 lesions (28 EGCs and 2 dysplasias) in the upper third of the intact stomach. All patients were treated with ESD at a cancer center hospital. En bloc resection rates were 100% (13/13) for remnant gastric tumor and 87% (26/30) for upper-third tumor (p = 0.30). En bloc resection rates with tumor-free margins (R0 resection) were 92% (12/13) for remnant gastric tumor and 73% (22/30) for upper-third tumor (p = 0.24). Curative resection rates were 85% (11/13) for remnant gastric tumor and 67% (20/30) for upper-third tumor (p = 0.29). Median procedure duration for ESD was 68 min (range 32-233 min) in the remnant stomach and 78 min (range 45-261 min) in the intact stomach (p = 0.49). One case of perforation and one of delayed bleeding occurred in the upper-third tumor group, whereas none occurred in the remnant tumor group. Local recurrence was found in no patients in the remnant gastric tumor group but in one patient in the upper-third tumor group. ESD for metachronous remnant gastric tumor was feasible and safe in comparison with ESD for tumor in the upper third of the intact stomach.

Summary: This study evaluated the efficacy of fibrin glue in preventing postendoscopic submucosal dissection (ESD) bleeding in high-risk patients. Helicobacter pylori Eradication Can Reverse Rho GTPase Expression in Gastric Carcinogenesis

Summary: This study evaluated the epigenetic and genetic expression changes of multiple Rho GTPases in Helicobacter pylori-related gastric carcinogenesis. It found that DOCK180 and ELMO1 had higher DNA methylation and expression levels in gastric cancer, while RhoA and Rac1 had lower levels. CDC42 had the same expression pattern as DOCK180 and ELMO1 without DNA methylation. The study also showed that H. pylori eradication could improve the epigenetic field of gastric carcinogenesis.

This second edition consists of standard data elements and conditional data elements, including basic pathologic findings and items necessary for predicting the prognosis of GC patients. Other factors relevant to adjuvant therapy are classified as conditional data elements. The standardized pathology report will be helpful for GC diagnosis and facilitate collaborative studies.

Stomach tissue classification using autofluorescence spectroscopy and machine learning

Summary: A machine-learning-based spectro-histological model was developed to accurately differentiate different layers of gastric tissue using autofluorescence spectra. The model achieved high prediction accuracy for mucosa, submucosa, and muscularis propria, with scores of 92.0%, 90.1%, and 91.4% respectively. The model was tested on both sliced and block tissue samples using a fast fluorescence imaging scanner.

Summary: This study compared the diagnostic performance of SurePath (TM) liquid-based cytology (LBC) and conventional smear (CS) cytology in endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) samples of upper gastrointestinal lesions. The results showed that LBC was not inferior to CS in diagnostic accuracy and had a shorter sample preparation time and better field of view.

Summary: This study investigated the suitability of the eCura system for determining the need for radical surgery after non-curative endoscopic submucosal dissection (ESD) for early gastric cancer (EGC). The results showed that follow-up without additional surgery after non-curative ESD can be a reasonable option for low-risk and even intermediate-risk patients according to the eCura system, but surgery is warranted for high-risk patients.

Clinical Practice Guideline for Gastritis in Korea

Summary: Gastritis is a common disease characterized by inflammation of the gastric mucosa, with various classification systems available. Recent emphasis has been placed on H. pylori gastritis due to its association with gastric cancer development and the potential for prevention through eradication. Although Korea has the highest incidence rate of gastric cancer globally, there are currently no clinical guidelines for atrophic gastritis and intestinal metaplasia. To address this, the Korean College of Helicobacter and Upper Gastrointestinal Research developed evidence-based guidelines through systematic review and made recommendations for key questions. This guideline will need periodic revision as new evidence emerges.

Histamine-2 Receptor Antagonists and Proton Pump Inhibitors Are Associated With Reduced Risk of SARS-CoV-2 Infection Without Comorbidities Including Diabetes, Hypertension, and Dyslipidemia: A Propensity Score-Matched Nationwide Cohort Study

Summary: This study aimed to investigate the effect of histamine-2 receptor antagonist (H2RA) and proton pump inhibitor (PPI) use on the positivity rate and clinical outcomes of COVID-19. The results showed that H2RA and PPI use was associated with a decreased risk for SARS-CoV-2 infection, but did not affect clinical outcomes. The protective effect of H2RA and PPI was not significant in patients with comorbidities such as diabetes, hypertension, and dyslipidemia.
