Endoscopic anatomy and approaches of the cavernous sinus: cadaver study

The objectives of this study were to recognize the endoscopic anatomy of the cavernous sinus and to understand the standard, purely endoscopic endonasal approaches to this anatomic structure. This basic information will facilitate our surgical procedures and decrease the rate of surgical complications. Seven fresh adult cadavers were studied bilaterally (n = 14). We used Karl Storz 0 and 30A degrees, 4 mm, 18 cm and 30 cm rod lens rigid endoscope in our dissections. After cadaver preparation, extended endoscopic endonasal approaches were performed to access the cavernous sinus. In the seven cadavers, the cavernous sinus and superior orbital fissure, on both sides, were widely exposed with extended endoscopic endonasal approach. The antero-inferior portion of the cavernous sinus was exposed by removing the superior and the middle turbinates and the posterior ethmoidal cells (extended endoscopic endonasal transsphenoidal approach); the whole lateral wall of the cavernous sinus was exposed by removing the anterior and the posterior ethmoidal cells (endonasal ethmoido-pterygo-sphenoidal approach: far lateral); and the medial wall of the cavernous sinus was exposed by introducing the 30A degrees endoscope from the contralateral nostril (contralateral endoscopic endonasal transsphenoidal approach). According to the neurovascular architecture, the lateral wall of the cavernous sinus is divided into the superior triangular area, the superior quadrangular area and the inferior quadrangular area. This division can facilitate understanding of the anatomic relations of the cavernous sinus from the endoscopic view. Knowledge of the anatomy of the cavernous sinus obtained with an endoscopic view of cadaver dissections is an essential step in the learning curve of endoscopic skull base surgery, and is important for endoscopic treatment of various pathologies in this region. In this anatomic study, we reviewed the approaches to the cavernous sinus with an endoscopic view and identified the neurovascular relations. This approach will help in performing safer and minimally invasive surgery.

Summary: In this study, we investigated the genotype distributions of SNPs in miR-107 and miR-23b genes related to acromegaly, finding a significant association of the T-allele with certain clinical features in patients. Additionally, patients who responded to SSA treatment showed higher Ki-67 index. No polymorphisms were observed in the miR-23b and miR-107 genes in the Turkish population.

Long term predictive ability of preoperative retinal nerve fiber layer thickness in visual prognosis after chiasmal decompression surgery

Summary: This study suggests that lower mean preoperative RNFL thickness was associated with poorer long-term visual prognosis. Summary: The signal intensity in T2-weighted Magnetic Resonance Imaging can predict postoperative results in acromegaly patients with macroadenoma. The surgical remission rates were significantly lower in the T2-hyperintense group compared to the T2-hypointense and T2-isointense groups in newly diagnosed acromegaly patients. Preoperative T2-intensity images may predict the probability of post-surgical remission in patients with newly diagnosed acromegaly.

The Combination of Dopamine Agonist Treatment and Surgery May Be the Best Option in Challenging Prolactinoma Cases: A Single-Centre Experience

Summary: This study investigated the remission rates, factors related to remission, secondary treatments, and outcomes of prolactinoma patients undergoing endoscopic transsphenoidal surgery (ETSS). The initial surgical remission rate was 60%, and recurrence was detected in 25.9% of patients. After secondary treatments, the long-term biochemical remission rate was 91.1%. Factors associated with failure in surgical remission included male gender, older age, larger tumor size, advanced stage, and elevated prolactin level at diagnosis. Patients who received preoperative dopamine agonist treatment and had a prolactin level of <19 ng/mL in the first postoperative week had a higher likelihood of surgical remission. For difficult cases of prolactinoma, a multidisciplinary approach involving neurosurgery and endocrinology is recommended.

Supportive facilities, flexible programs, universal life policies, and efforts to reduce social stereotypes are important in overcoming gender inequalities faced by women in neurosurgery.
