Experience of meaning in life in bereaved informal caregivers of palliative care patients

Purpose Providing care for terminally ill family members places an enormous burden on informal caregivers. Meaning in life (MiL) may be a protective factor, but is jeopardised in caregiving and bereavement. This study evaluates the following questions: To what extent do bereaved informal caregivers of palliative care (PC) patients experience meaning in their lives? What differences emerge in carers compared to the general German population? How does MiL relate to wellbeing in former caregivers? Methods Eighty-four bereaved PC caregivers completed the Schedule for Meaning in Life Evaluation, the Brief Symptom Inventory, the WHOQOL-BREF, a single-item numerical rating scale of quality of life, and the Satisfaction with Life Scale. The experience of MiL of bereaved caregivers was compared to a representative population sample (n=977). Results The overall MiL fulfillment of bereaved caregivers (69 % female, age 55.5 +/- 12.9 years) was significantly lower than in the general population (68.5 +/- 19.2 vs. 83.3 +/- 14, p<.001), as was the overall importance ascribed to their meaning framework (76.6 +/- 13.6 vs. 85.6 +/- 12.3, p<.001). PC caregivers are far more likely to list friends, leisure, nature/animals, and altruism. Higher MiL was correlated with better life satisfaction and quality of life. Conclusion Coping with the loss of a loved one is associated with changes in MiL framework and considerably impairs a carer's experience of MiL fulfillment. Individual MiL is associated with well-being in PC caregivers during early bereavement. Specific interventions for carers targeted at meaning reconstruction during palliative care and bereavement are needed to help individuals regain a sense of meaning and purpose.

Mortality in Underground Miners in a Former Uranium Ore Mine Results of a Cohort Study Among Former Employees of Wismut AG in Saxony and Thuringia

Summary: Underground miners employed by Wismut AG exhibited significantly higher mortality rates due to silicosis/other pneumoconiosis and lung cancer. Summary: The implementation of low emission zones (LEZs) has been shown to be an effective method to improve urban air quality, particularly in reducing PM mass concentration and EC level. The combination of LEZ with heavy-duty vehicle transit bans in cities like Berlin and Munich significantly lowered PM10 concentrations, with the greatest effects observed in LEZ stage 3. In contrast, the effects of LEZ on NO2 levels were inconsistent across different cities and stages of implementation.

Summary: This study is the largest and most up-to-date pooled study of uranium miners, involving 7 cohorts of male uranium miners with 7754 lung cancer deaths and 4.3 million person-years of follow-up. The study found a linear exposure-response relationship between cumulative radon exposure and lung cancer mortality, which is modified by temporal and exposure factors.

Updated risk models for lung cancer due to radon exposure in the German Wismut cohort of uranium miners, 1946-2018

Summary: UNSCEAR recently recommended focusing future research on the lung cancer risk from low radon exposures or exposure rates on contemporary uranium miners. To meet this recommendation, risk models in the German Wismut cohort were updated using an extended follow-up period. The results showed that lung cancer risk decreased with older age, longer time since exposure, and higher exposure rates among uranium miners, emphasizing the importance of radiation protection against radon.
