Summary: Although guidelines recommend using proton pump inhibitors (PPIs) to reduce gastrointestinal bleeding in patients receiving dual antiplatelet therapy, a study on DAPT patients with low gastrointestinal bleeding risk showed that PPI use did not provide additional gastrointestinal protection and was associated with an increased risk of stroke and myocardial infarction during follow-up.

Summary: This study aimed to determine the incidence, associated outcomes, and predictors of upper gastrointestinal bleeding (UGIB) following acute myocardial infarction (MI). The study found that UGIB was significantly associated with increased risk of all-cause death and stroke, but not recurrent MI. Important predictors of UGIB included haemoglobin, age, systolic blood pressure, blood glucose, smoking status, previous upper gastrointestinal bleeding, and antithrombotic and gastroprotective treatment.

Summary: This study aimed to compare the incidence of gastrointestinal bleeding between hemodialysis (HD) and peritoneal dialysis (PD) patients, and provide guidance for individualized decision-making. The results showed that HD patients had a higher risk of gastrointestinal bleeding compared to PD patients, especially when anticoagulation was needed.

Summary: The study found a decreasing trend in upper gastrointestinal bleeding and a slight increase in lower gastrointestinal bleeding over time, especially in the elderly population. Prescriptions of PPIs and aspirin were found to be increasing and had different associations with the incidences of UGIB and LGIB.

Summary: This meta-analysis found that proton-pump inhibitor (PPI) cotherapy in oral anticoagulant (OAC)-treated patients is associated with a lower risk of gastrointestinal bleeding (GIB). This protective effect is independent of ethnic group and OAC type, except for edoxaban. PPI cotherapy may be particularly beneficial for patients with high risk of GIB.

Summary: The study found no significant difference in the prevalence of HRS on ulcers between patients receiving or not receiving pre-endoscopic PPI therapy, but a higher prevalence of non-ulcer bleeding lesions in patients treated with PPI. PPI therapy was not an independent factor affecting HRS prevalence, while timing to endoscopy was inversely correlated with HRS prevalence in UGIB patients.

Summary: This study found no overall association between habitual dietary flavonoid intake and the risk of ischemic heart disease (IHD). However, stratified analysis by sex and smoking status showed that higher intakes of specific subclasses of flavonoids were associated with a lower risk of IHD among ever-smokers, but not among never-smokers or either sex specifically.

Summary: The purpose of this study was to investigate patients with streptococcal infective endocarditis (IE) and describe the clinical features and outcomes according to streptococcal species. The results showed that S. mitis/oralis and S. gallolyticus were the most frequent streptococcal species causing IE, and S. gallolyticus IE was associated with a lower rate of heart valve surgery during admission compared with S. mitis/oralis IE.

Summary: This study analyzed the efficacy and safety of sodium-glucose cotransporter-2 inhibitors in the treatment of heart failure, and assessed the impact of sex on their effectiveness and safety. The study found that, after adjusting for baseline differences, women had fewer cardiovascular and all-cause deaths, as well as lower rates of HF hospitalizations and total events, compared to men. There were no sex-related differences in adverse events or drug discontinuation. Therefore, sex does not modify the treatment effect of sodium-glucose cotransporter-2 inhibitors.

Summary: This study investigated the diagnostic accuracy of cCTA and the impact of early ICA on long-term clinical outcomes in elderly patients with NSTEACS. The study found that cCTA had a high accuracy in ruling out significant CAD in patients aged ≥75 years, but early ICA did not significantly improve long-term clinical outcomes.

Summary: This study aimed to investigate the mortality according to streptococcal species and seasonal variation. Through a study of patients with streptococcal bloodstream infections in the Capital Region of Denmark from 2008 to 2017, it was found that S. pneumoniae was associated with lower 30-day and one-year mortality, while other streptococcal species may be associated with higher mortality. In addition, patients infected with streptococcal species in the autumn and winter had a higher risk of death within 30 days.

Summary: This study found that gout is common in patients with heart failure and sodium-glucose cotransporter 2 inhibitors can reduce uric acid levels. Gout was associated with worse clinical outcomes, but the efficacy of dapagliflozin was consistent in patients with and without gout. Summary: This study aimed to investigate the recurrence of bacteremia or infective endocarditis (IE) in patients surviving IE and the associated risk factors. Using Danish nationwide registries, the researchers identified the bacterial species involved in IE and found that S. aureus, Enterococcus spp., Streptococcus spp., CoNS, and 'Other' were common pathogens. The recurrence rates of bacteremia or IE with the same bacterial species were 4.8% and 2.6% at 12 months, and 7.7% and 4.0% at 5 years. S. aureus, Enterococcus spp., and CoNS were associated with an increased risk of recurrent infections.

Summary: This study examined the impact of frailty on the initiation of SGLT2 inhibitors and GLP-1 receptor agonists in patients with type 2 diabetes and cardiovascular disease. It was found that frail patients had a significantly lower probability of initiating these two drug therapies, indicating the need for clear guidelines for their use in this population.
