Summary: It has been reported that degenerated and herniated lumbar intervertebral discs express high levels of IL-17, indicating the presence of local immune reactions in patients with low back pain. Although clinical sample analyses from different laboratories confirm this, little is known about how IL-17 is induced in the pathology and its interactions with other inflammatory responses. This conscience review aims to organize current laboratory findings on this topic and provide a trajectory for a comprehensive understanding of the role of IL-17 in the pathology of intervertebral disc disease.

Summary: Ozone therapy for extruded disc herniation acts by modulating the immune response, activating macrophages to phagocytose the extruded nucleus pulposus. It facilitates the transition from an inflammatory phase to a reparative phase of macrophages. Further studies are needed to confirm this mechanism.

Summary: This study explores the QALY and cost-effectiveness estimates based on different statistical methods using the EQ-5D-5L value set, EQ-5D-3L UK value set, and ReQoL-UI. The interaction between item scores, health state profiles, preference-based scores, and mathematical and statistical methods chosen may contribute to the different QALY estimates.

Summary: This study systematically reviewed EQ-5D utility scores among patients with gallstone disease and its improvement on treatment. The results showed that cholecystectomy can improve health-related quality of life (HRQoL) for patients with gallstone disease, but with high heterogeneity.

Summary: The EQ Health and Wellbeing Short (EQ-HWB-S) is a new measure of health and wellbeing that overlaps with, and expands upon, the EQ-5D-5L. The study found moderate to strong associations between the two measures and concluded that the EQ-HWB-S performs similarly or better than the EQ-5D-5L among patient groups, particularly in distinguishing caregivers and those closer to full health.

Summary: This study used Raman spectroscopy and histological methods to examine the components and structures of proteoglycan and collagen in cervical disc herniated nucleus pulposus (NP). They found that proteoglycans and collagen undergo changes in content and structure during degeneration, providing further understanding of the mechanism of cervical disc herniation.

Summary: Intervertebral disc degeneration is a common cause of low back pain and spinal disorders, and there is currently no effective treatment. However, advances in regenerative medicine and tissue engineering suggest the potential of cell-based and biomaterial-based disc regeneration therapies. Yet, there are still many challenges to be addressed before clinical application.

Summary: This study aimed to develop a value set for EQ-5D-5L based on the health preferences of the healthy general adult population in the Philippines. The chosen model to represent the preferences of the Philippine general population for EQ-5D-5L health states was an 8-parameter homoscedastic TTO-only model.

Summary: A consensus was reached among experts from Italian radiological societies to propose an optimized MRI protocol for studying degenerative spine disease, harmonize the nomenclature to international standards, and improve communication among healthcare professionals and patients. The international terminological recommendations were translated and adapted to the Italian context, with clinical cases used to illustrate main classifications.

Summary: This study compared the outcomes of percutaneous cervical nucleoplasty (PCN) and conservative treatment (CT) for symptomatic contained cervical herniated intervertebral disc (CHIVD). The results showed that PCN had significantly better outcomes in terms of pain levels and functional outcomes compared to CT. The study also found that the reduction in disc height after PCN did not affect the clinical outcomes.

Summary: This study aimed to evaluate the associations between different parameters in dogs with cervical intervertebral disc extrusion before ventral slot decompression (VSD) surgery. The study found no significant association between the maximal spinal cord compression ratio (SCCR) and pre-surgical neurological status (NS) grade. There was a significant difference in complete recovery at 10 days between ambulatory and non-ambulatory tetraparesis dogs, but no significant difference at 30 days. There was no correlation between SCCR and recovery time. Caudal cervical extrusion did not show worse NS grade or longer recovery time than cranial extrusion, and the length ratio of CSF attenuation on HASTE images was weakly correlated with post-surgical recovery time.

Summary: This study developed an algorithm to predict the utility of total naso-ocular symptom score in patients with Japanese cedar pollinosis (JCP) and conducted a cost-utility analysis of sublingual immunotherapy (SLIT) for JCP using this algorithm. The results showed that pre-seasonal SLIT for JCP is cost-effective.

Summary: The study aimed to assess the predictive value of serum C-reactive protein (CRP) for postoperative outcomes in paraplegic dogs with acute intervertebral disc extrusion (IVDE). However, the results showed that CRP levels did not effectively predict postoperative outcomes in these dogs. Additionally, a weak linear relationship was found between CRP levels and intramedullary changes on MRI, suggesting some association between the two factors.

Summary: Prolapse of intervertebral disc is a common pathology in neurosurgery, but intradural lumbar disc herniation is a rare occurrence. We present a case of a 30-year-old male with lower back pain radiating to the right lower limb for the past 2.5 years, which started after weight lifting. There were no associated motor or sensory deficits. Magnetic resonance imaging showed prolapse of intervertebral disc at the level of lumbar L4-L5. The patient underwent laminectomy and intradural discectomy with a smooth post-operative recovery and no neurological deficits. Thorough radiological examination can aid in pre-operative diagnosis of intradural lumbar disc herniation.
