Distribution of β-Lactamase Genes Among Carbapenem-Resistant Klebsiella pneumoniae Strains Isolated From Patients in Turkey

Background: The emergence of carbapenem-resistant Klebsiella pneumoniae poses a serious problem to antibiotic management. We investigated the beta-lactamases in a group of carbapenem-resistant K. pneumoniae clinical isolates from Turkey. Methods: Thirty-seven strains of K. pneumoniae isolated from various clinical specimens were analyzed by antimicrobial susceptibility testing, PCR for the detection of p-lactamase genes, DNA sequencing, and repetitive extragenic palindronic (REP)- FOR analysis. Results: All 37 isolates were resistant to ampicillin, ampicillin/sulbactam, piperacillin, piperacillin/tazobactam, ceftazidime, cefoperazone/sulbactam, cefepime, imipenem, and meropenem. The lowest resistance rates were observed for colistin (2.7%), tigecycline (11%), and amikacin (19%). According to FOR and sequencing results, 98% (36/37) of strains carried at least one carbapenemase gene, with 32 (86%) carrying OXA-48 and 7 (19%) carrying NDM-1. No other carbapenemase genes were identified. All strains carried a CTX-M-2-like beta-lactamase, and some carried SHV- (97%), TEM- (9%), and CTX-M-1-like (62%) beta-lactamases. Sequence analysis of bla(TEM) genes identified a bla(TEM-166) with an amino acid change at position 53 (Arg53Gly) from bla(TEM-1b), the first report of a mutation in this region. REP-FOR analysis revealed that there were seven different clonal groups, and temporo-spatial links were identified within these groups. Conclusions: Combinations of beta-lactamases were found in all strains, with the most common being OXA-48, SHV, TEM, and CTX-M-type (76% of strains). We have reported, for the first time, a high prevalence of the NDM-1 (19%) carbapenemase in carbapenem-resistant K. pneumoniae from Turkey. These enzymes often co-exist with other beta-lactamases, such as TEM, SHV, and CTX-M beta-lactamases.

Characteristics in the whole-genome sequence of Klebsiella pneumoniae ST147 from Turkey

Summary: This study aimed to analyze the determinants of antibiotic resistance in carbapenem-resistant Klebsiella pneumoniae through whole-genome sequencing. The isolate showed resistance to all tested beta-lactam antibiotics and carried multiple resistance genes. The clonal subtype and serotype of the isolate, as well as the presence of resistance genes on plasmids, were also determined.

Real-world experience of Quantiferon-CMV directed prophylaxis in lung transplant recipients

Summary: This study reviewed the application of Quantiferon-Cytomegalovirus (QF-CMV) testing in lung transplant recipients and found a strong correlation between QF-CMV results and serostatus. D+/R- patients were unlikely to test positive while receiving prophylaxis. Extended prophylaxis was associated with delayed, reduced, and milder CMV infection. However, the incremental predictive value of QF-CMV was limited after accounting for serostatus.

Summary: In this study, strain typing and structural and functional analysis of ADC-73 beta-lactamase in multidrug-resistant Acinetobacter baumannii isolated from Turkey were conducted. The A24 strain was found to be multidrug-resistant and various virulence factors were identified. Additionally, it was reported for the first time in Turkey that the ST218 isolate of A. baumannii contained ADC-73, and the impact of the G225S mutation on its conformational change and interaction with cefepime and imipenem was investigated using in silico methods.

The impact of daptomycin therapeutic drug monitoring on clinical outcomes: a systematic review

Summary: This study aimed to determine whether daptomycin therapeutic drug monitoring (TDM), including dose adjustment, improves the clinical outcomes of adult patients with Gram-positive infections. A systematic review found no studies exploring the efficacy of routine daptomycin TDM in comparison with fixed dosing regimens. Further well-designed, comparative studies are needed to determine the role of daptomycin TDM in patients with Gram-positive infections.

Oral ciprofloxacin activity against ceftriaxone-resistant Escherichia coli in an in vitro bladder infection model

Therapeutics for Vancomycin-Resistant Enterococcal Bloodstream Infections

Summary: Vancomycin-resistant enterococci (VRE) are a common cause of bloodstream infections (BSIs) with high morbidity and mortality rates. The management of VRE BSI presents significant challenges, including drug dosing, resistance emergence, and optimal treatment strategies. Therapeutic drug monitoring (TDM) shows promise in predicting drug toxicity for VRE-active agents, but there is limited evidence for predicting antimicrobial efficacy for VRE BSIs. This article provides an overview of treatment options, the role of TDM in clinical infection management, and future challenges and perspectives.

Characterisation of key genotypic and phenotypic traits of clinical cystic fibrosis Staphylococcus aureus isolates

Antimicrobial susceptibility of ceftolozane-tazobactam against multidrug-resistant Pseudomonas aeruginosa isolates from Melbourne, Australia

Summary: We investigated the effectiveness of ceftolozane-tazobactam (C/T) in a high-risk population for antimicrobial resistance, specifically patients with cystic fibrosis (CF) and lung transplantation (LTx). Out of 163 clinical Pseudomonas aeruginosa isolates collected from a tertiary hospital in Melbourne, Australia, 81.6% were susceptible to C/T. Among the multidrug resistant (MDR) and extensively drug resistant (XDR) isolates, C/T showed susceptibility in 88.3% and 28.1% of cases, respectively. In conclusion, C/T maintained its activity against a significant portion of MDR and XDR P. aeruginosa isolates from CF and post-LTx patients.

EHR-QC: A streamlined pipeline for automated electronic health records standardisation and preprocessing to predict clinical outcomes

Summary: The adoption of electronic health records (EHRs) has created opportunities for predicting clinical outcomes and improving patient care. However, non-standardized data representations and anomalies present major challenges in digital health research. To address these challenges, we have developed EHR-QC, a tool with two modules: data standardization and preprocessing. We believe that the development and adoption of tools like EHR-QC are critical for advancing digital health.
