Sarcoidosis is a multisystem inflammatory disease, which is characterized by the development of noncaseating granulomas amid healthy tissue. While most commonly affecting the mediastinum and/or lungs, sarcoid can also affect the liver, resulting in hepatic sarcoid. The spectrum of disease in hepatic sarcoid is variable, ranging from asymptomatic or mild liver enzyme abnormalities to end-stage liver disease requiring liver transplantation. Because clinically-significant hepatic sarcoid is rare, research on epidemiology, clinical manifestations, and treatment is limited. The mainstays of hepatic sarcoid treatment have involved steroids and more recently, ursodeoxycholic acid; however, novel immunomodulatory agents provide new possibilities for management. Questions remain regarding screening, diagnostic modalities, and what to treat with and when. The purpose of this review is to summarize the available research on the epidemiology, clinical course, and management of hepatic sarcoid, and identify gaps in our knowledge to motivate future research.

Mortality and Hepatic Decompensation in Patients With Cirrhosis and Atrial Fibrillation Treated With Anticoagulation

Summary: The study found that warfarin and DOACs were associated with reduced all-cause mortality in patients with cirrhosis and atrial fibrillation. In secondary analysis, DOACs had a lower incidence of bleeding compared to warfarin. Future studies should further investigate these observed associations.

Summary: More than 10% of NAFLD patients in the United States are lean, with Asians accounting for almost half of them. Lean patients have lower prevalence of cirrhosis, CVD, and metabolic abnormalities compared to non-lean patients. Asian participants have significantly lower prevalence of these conditions than non-Asians across all BMI categories.

Emricasan to prevent new decompensation in patients with NASH-related decompensated cirrhosis

Summary: Emricasan was found to be safe but ineffective in treating decompensated NASH cirrhosis in a double-blind, placebo-controlled study with 217 participants. This study may provide guidance for future clinical trials in patients with decompensated cirrhosis related to non-alcoholic steatohepatitis.

Patterns of Alcohol Use After Early Liver Transplantation for Alcoholic Hepatitis

Summary: Early liver transplantation for alcoholic hepatitis can save lives, but concerns about continued alcohol use exist. This study identified distinct patterns of alcohol use post-transplantation and found that early alcohol use was associated with higher risk of mortality at 3 and 5 years. Pre-transplant characteristics were associated with post-transplant alcohol use patterns, which can inform candidate selection and addiction care after transplantation.

Summary: The number of patients with alcoholic hepatitis (AH) in the liver transplant waitlist has been increasing in recent years. AH patients have a shorter waiting time and higher survival and transplant rates compared to other patients with high Model for End-stage Liver Disease (MELD) scores.

Autoimmune hepatitis and metabolic syndrome-associated disease development: a US cohort study

Summary: This study found that patients with autoimmune hepatitis (AIH) are at a higher risk of developing metabolic syndrome-associated diseases (MSADs), possibly due to the long-term use of corticosteroid therapy and general population trends. The initial treatment regimen was found to affect the development of new-onset diabetes, and the presence of MSADs was associated with lower rates of biochemical remission in AIH patients.

Summary: This study aimed to investigate racial and ethnic inequities in liver transplantation (LT) waitlisting for medical and psychosocial reasons. The findings showed that Black race and Hispanic/Latinx ethnicity were associated with not being waitlisted for psychosocial reasons. Health systems and LT centers should develop programs to address these disparities and promote equity in LT access.

Summary: Early liver transplantation for alcohol-associated hepatitis is controversial due to the potential for patient recovery and thus avoiding the need for transplantation. This retrospective study found that recompensation of liver function is rare among severe alcohol-associated hepatitis patients declined for transplantation, with higher MELD-Na values and older age associated with short-term mortality, and a history of failed alcohol rehabilitation associated with long-term mortality.
