Neurologic involvement in scleroderma: A systematic review

Objectives: To perform a systematic review of neurologic involvement in Systemic sclerosis (SSc) and Localized Scleroderma (LS), describing clinical features, neuroimaging, and treatment. Methods: We performed a literature search in PubMed using the following MeSH terms, scleroderma, systemic sclerosis, localized scleroderma, localized scleroderma en coup de sabre, Parry-Romberg syndrome, cognitive impairment, memory, seizures, epilepsy, headache, depression, anxiety, mood disorders, Center for Epidemiologic Studies Depression (CES-D), SF-36, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Patient Health Questionnaire-9 (PHQ-9), neuropsychiatric, psychosis, neurologic involvement, neuropathy, peripheral nerves, cranial nerves, carpal tunnel syndrome, ulnar entrapment, tarsal tunnel syndrome, mononeuropathy, polyneuropathy, radiculopathy, myelopathy, autonomic nervous system, nervous system, electroencephalography (EEG), electromyography (EMG), magnetic resonance imaging (MRI), and magnetic resonance angiography (MRA). Patients with other connective tissue disease knowingly responsible for nervous system involvement were excluded from the analyses. Results: A total of 182 case reports/studies addressing SSc and 50 referring to LS were identified. SSc patients totalized 9506, while data on 224 LS patients were available. In LS, seizures (41.58%) and headache (18.81%) predominated. Nonetheless, descriptions of varied cranial nerve involvement and hemiparesis were made. Central nervous system involvement in SSc was characterized by headache (23.73%), seizures (13.56%) and cognitive impairment (8.47%). Depression and anxiety were frequently observed (73.15% and 23.95%, respectively). Myopathy (51.8%), trigeminal neuropathy (16.52%), peripheral sensorimotor polyneuropathy (14.25%), and carpal tunnel syndrome (6.56%) were the most frequent peripheral nervous system involvement in SSc. Autonomic neuropathy involving cardiovascular and gastrointestinal systems was regularly described. Treatment of nervous system involvement, on the other hand, varied in a case-to-case basis. However, corticosteroids and cyclophosphamide were usually prescribed in severe cases. Conclusions: Previously considered a rare event, nervous system involvement in scleroderma has been increasingly recognized. Seizures and headache are the most reported features in LS en coup de sabre, while peripheral and autonomic nervous systems involvement predominate in SSc. Moreover, recently, reports have frequently documented white matter lesions in asymptomatic SSc patients, suggesting smaller branches and perforating arteries involvement. (C) 2013 Elsevier Inc. Additionally, patients with overlap features had a higher frequency of interstitial lung disease.

Summary: This study aimed to develop a standardized steroid dosing regimen for physicians treating childhood-onset systemic lupus erythematosus complicated by lupus nephritis. By evaluating patient course and various parameters, a consensus-based dosing regimen was developed and validated.

Summary: Sjogren's Syndrome is an autoimmune disease with systemic manifestations, but there is currently no standardized approach to diagnose and evaluate these manifestations. A panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis, summarizing the epidemiologic and clinical features of the disease and providing recommendations.

Revisiting the use of proton magnetic resonance spectroscopy in distinguishing between primary and secondary malignant tumors of the central nervous system

Summary: This study evaluated the usefulness of proton magnetic resonance spectroscopy (H-1-MRS) in distinguishing primary from secondary brain tumors. The results showed that secondary tumors had higher NAA/Cr and Cho/Cr ratios, with a good correlation between NAA/Cr and Cho/Cr. By constructing a logistic regression model, the nature of the tumor could be reasonably predicted.

Summary: This study evaluated the feasibility of using texture analysis (TA) based on magnetic resonance imaging (MRI) to identify patients with juvenile idiopathic arthritis (JIA) likely to progress to temporomandibular joint damage. The results showed significant differences in texture parameters of the condyle between the JIA group and the control group, and a progressive loss of uniformity in grayscale pixels of MRI with increasing age in the JIA group.

Automated quality check of corpus callosum segmentation using deep learning

Summary: In this study, a convolutional neural network (CNN) is proposed for quality control of corpus callosum (CC) segmentations. The method was validated using magnetic resonance imaging (MRI) data and achieved high accuracy on the test set. Furthermore, the approach demonstrated good generalization capability to data from different domains.

Neuropsychiatric manifestations in childhood-onset systemic lupus erythematosus

Summary: Neuropsychiatric manifestations are common in childhood-onset systemic lupus erythematosus (SLE) and are difficult to diagnose. Most patients experience these manifestations within the first two years of the disease, with 30-70% of them having multiple events during the course of the disease. These symptoms have significant impact on disability and mortality, but only a few biomarkers have been validated for diagnosis, treatment monitoring, and prognosis.

Summary: This study aimed to compare the levels of Th1 (IL-12) and Th2 (IL-6 and IL-10) cytokines among cSLE, aSLE, and healthy controls over a two-year period. The results showed that cSLE patients had significantly higher levels of IL-12 compared to healthy controls, while aSLE patients had significantly higher levels of IL-6 compared to healthy controls.

Multidisciplinary Management of Fabry Disease: Current Perspectives

Summary: Fabry disease is a rare genetic disorder caused by a gene variant that leads to a deficiency in the enzyme alpha-galactosidase A. This deficiency causes the accumulation of a specific lipid in various organs, resulting in a wide range of clinical manifestations and potentially life-threatening complications such as cardiac arrhythmia, kidney failure, and stroke. This review provides an overview of the clinical features and current multidisciplinary management approaches for Fabry disease patients.

Summary: This study found that childhood-onset SLE patients have axonal dysfunction, which is associated with IFN-gamma levels and depressive symptoms. Increased CHO ratios are related to cognitive impairment, while mI/Cr ratios are correlated with cumulative glucocorticoids dosage.

Summary: This study aimed to investigate whether systematic calibration improves the scoring proficiency of JAMRIS-SIJ and whether contrast-enhancement enhances its performance. The results showed that calibrated readers achieved greater reliability in scoring specific inflammatory and structural lesions. Sensitivity and reliability for scoring inflammatory lesions were higher on fluid-sensitive sequences compared to contrast-enhanced sequences. Therefore, systematic calibration should be implemented before using JAMRIS-SIJ in clinical trials, and it is unlikely that contrast-enhanced MRI will improve the performance of this method.

Summary: This study evaluated whether the initial use of glucocorticoid (GC) bridging in RA patients leads to a higher probability of long-term GC and bDMARD use. The results showed that patients who initially started GC had a higher risk of later GC use, but the risk of bDMARD use was not significantly increased.

Cardiovascular safety of zoledronic acid in the treatment of primary osteoporosis: A meta-analysis and systematic review

Summary: This study assessed the cardiovascular safety of zoledronic acid in the treatment of primary osteoporosis. The results showed that in women with primary osteoporosis, zoledronic acid may increase the risk of atrial fibrillation and arrhythmias, but the cardiovascular risk in men with osteoporosis is uncertain.
