Aquatic exercise improves the monocyte pro- and anti-inflammatory cytokine production balance in fibromyalgia patients

Current hypotheses of the etiology of fibromyalgia (FM) include inflammatory disorders. We evaluated the effect of a pool-aquatic exercise program (8 months, two weekly 60-min sessions) on the inflammatory cytokine production by isolated monocytes, and on the serum concentration of C-reactive protein (CRP), in a group of female FM patients. Monocytes from FM patients released more IL-1 beta, TNFa, IL-6, and IL-10 than those from an age-matched control group of healthy women (HW). This inflammatory disorder in FM women was also manifested by high circulating concentrations of CRP. Increased IL-6 with a concomitant decreased TNFa spontaneous release was found after 4 months (midway through) of the exercise program. At the end of the program (8 months), monocytes from FM patients showed diminished spontaneous production of pro-/anti-inflammatory cytokines, with a similar spontaneous release of IL-1 beta and IL-6 to that of HW, but a lower production of TNFa and higher of IL-10. Lipopolysaccharide-induced production of IL-1 beta, TNFa, IL-6, and IL-10 also decreased at the end of the exercise program, although IL-10 remained higher than HW. The anti-inflammatory effect of the exercise program was also corroborated by a decrease in the circulating CRP concentration. Exercise also improved the health-related quality of life of FM patients.

Summary: This investigation examined the effects of a synbiotic in athletes and sedentary individuals, specifically on the immune system, autonomic regulation, and body composition. The results showed significant differences in heart rate variability between sedentary individuals and athletes, but no significant differences between the experimental and control groups. Additionally, there may be differences in the effects of the synbiotic on fat mass and body-fold sum between athletic and sedentary groups due to training. No significant differences were found in cytokines after the study protocol. It is necessary to conduct further research to investigate the chronic effects of the synbiotic on the analyzed biomarkers.

Summary: The aim of this research was to analyze the effects of a synbiotic nutritional supplement on patients diagnosed with fibromyalgia (FM) and chronic fatigue syndrome (CFS), focusing on the interaction between inflammatory and stress responses mediated by the cytokine-HPA axis, as well as mental and physical health. The study found that the synbiotic supplement decreased levels of stress, anxiety, and depression, and improved quality of life for FM patients. In addition, it activated the HPA axis to balance the increased inflammation observed in FM patients. The study concludes that synbiotic nutritional supplements can improve the dysregulated immunoneuroendocrine interaction and perceived levels of stress, anxiety, depression, and quality of life in FM patients.
