Positive tip culture with candida and negative blood culture: to treat or not to treat? A systematic review with meta-analysis

Background: Invasive candidiasis is a major invasive fungal infection. It has high lethality, and even higher if not treated early. There is no consensus on antifungal treatment in patients with positive catheter tip culture for Candida spp. The objective of this study was to evaluate the impact of antifungal therapy and mortality of patients with positive culture for Candida spp. in catheter tip that have negative blood culture. Methods: The PubMed database was searched to identify articles related to Candida and catheter. Articles with adequate data were included. Results: Of 1208 studies initially screened, 5 met the selection criteria. All were retrospective studies. In all, 265 patients were evaluated for outcomes 'candidemia' and 'invasive candidiasis' and 158 for the outcome 'mortality.' Antifungal therapy had no impact on the development of invasive fungal disease (Odds ratio (OR) = 1.41; 95% confidence interval (CI) = 0.56-3.52). Also there was no benefit of therapy on mortality (OR = 1.02; 95% CI = 0.54-1.95). Conclusion: Due to the poor quality of the studies no conclusion can be made. Randomized prospective studies are needed to better evaluate this therapeutic strategy.

Summary: This study aimed to develop a decellularization protocol that allows the removal of cells and myelin sheath without detrimental effects on nerve architecture. The addition of sonication to detergent-based decellularization methods was found to increase the effectiveness of removing unwanted cellular components from peripheral nerve allografts, as confirmed by histology and scanning electron microscopy.

Summary: The decellularization of bovine bone is an effective method for repairing bone defects. The new protocol used in this study successfully removed bone marrow components and cell debris, while preserving the structure of collagen fibers and extracellular matrix. However, the decellularized bovine bone showed moderate toxicity.

Trend analysis of carbapenem-resistant Gram-negative bacteria and antimicrobial consumption in the post- COVID-19 era: an extra challenge for healthcare institutions

Summary: This study analyzed the incidence density trend of carbapenem-resistant Gram-negative bacteria and antimicrobial consumption in 99 critical care facilities in a low/middle-income country. The findings showed an increase in the incidence density of carbapenem-resistant Acinetobacter baumannii (CRAB) in 2020, which was strongly correlated with the incidence density of COVID-19. There was also an increase in polymyxin consumption in 2020, but it was not significantly correlated with CRAB or COVID-19 incidence density, likely due to empirical and untargeted prescribing.

Pseudomonas aeruginosa in tracheal aspirate: Colonization, infection, and recurrence

Summary: This study aimed to investigate the risk factors associated with the presence of P. aeruginosa, as well as the risk factors related to the recurrence and death of lower airway infections in inpatients. The results showed that previous pulmonary disease was associated with colonization by P. aeruginosa. Aminoglycosides were the most effective drugs in susceptibility tests and were successfully used as monotherapy. Septic shock was a risk factor for death in infected patients. The use of adequate antimicrobial therapy was associated with increased survival, regardless of the infection classification.

Activity of multipurpose contact lens solutions against Staphylococcus aureus, Pseudomonas aeruginosa, Serratia marcescens and Candida albicans biofilms

Summary: The use of contact lenses has increased the risk of microbial keratitis. This study tested the effectiveness of 14 multipurpose contact lens solutions against bacterial and fungal biofilms. The results showed that only five solutions had a significant effect on bacterial biofilms, while none were able to eradicate fungal biofilms. In conclusion, multipurpose contact lens solutions have better bactericidal activity against planktonic cells than biofilms.

Sonication as a tool for disrupting biofilms and recovering microorganisms in bladder catheters

Summary: This study aimed to evaluate the recovery capability of strict, facultative, and aerobic microorganisms on bladder catheters using conventional culture, sonication, urinary analysis, and mass spectrometry. The results showed that sonicated catheters had a higher positivity rate than urine samples for anaerobic and aerobic microorganisms, indicating that bladder catheter sonication is more sensitive for detecting these microorganisms.

Summary: This study describes a case of severe S. clavata infection in a Brazilian children's hospital, including characterization of antifungal minimal inhibitory concentration, S. clavata biofilm, and molecular features. The study found that S. clavata can form biofilms and is sensitive to certain antifungal drugs. These findings are important for the development of novel treatments, especially for biofilm-associated infections.
