Summary: Primary sclerosing cholangitis is a rare disease often associated with inflammatory bowel disease. The pathogenesis may involve changes in cholangiocyte function, gut microbiome, human leukocyte antigen haplotypes, and immune system dysregulation, but these fail to explain the association with inflammatory bowel disease.

Summary: This study found that the overall prevalence of PSC in IBD patients was 2.16%, with the highest prevalence in South America and the lowest in Southeast Asia. UC patients had the highest prevalence of PSC, while CD patients had the lowest.

Summary: Observational studies suggest a significant causal association between inflammatory bowel disease (IBD) (including ulcerative colitis and Crohn's disease) and Primary sclerosing cholangitis (PSC) in a European population based on two-sample Mendelian randomization (MR). The analysis found a positive causal effect of IBD on PSC, providing implications for prevention and treatment of both diseases. Further analyses revealed statistical heterogeneity and sensitivity of the single nucleotide polymorphisms (SNPs) used in the study.

Summary: This study compared the fecal microbiota in PSC-IBD patients, IBD-only patients, and healthy controls, revealing significant differences in intestinal microbiota composition between PSC-IBD and IBD-only patients. Certain bacterial species could potentially serve as microbial signatures for PSC-IBD or IBD. Further large-scale studies are needed to better understand the role of gut microbiota in PSC pathogenesis with underlying IBD.

Summary: The risk of death following CRC diagnosis is not affected by a diagnosis of IBD in recent years. There is a high proportion of post colonoscopy CRC among IBD-CRC patients, which has remained unchanged over the years and requires detailed root-cause analysis and interventions.

Summary: Observational studies have shown a higher incidence of primary biliary cholangitis (PBC) in patients with inflammatory bowel disease (IBD) compared to healthy individuals. This study used genetic data from genome-wide association studies (GWAS) to investigate the causal relationship between IBD and PBC. The results suggest that genetically predicted IBD increases the risk of PBC, but genetic susceptibility to PBC does not alter the risk of IBD. This has important implications for understanding the etiology of PBC and managing patients with IBD.

Summary: Inflammatory bowel disease (IBD) combined with primary sclerosing cholangitis (PSC) should be considered as a distinct health condition, and further research is needed. PSC-IBD has clinical characteristics different from classical IBD and involves disruptions to both innate and adaptive immunity. Understanding the gut-liver axis and the pathogenesis of PSC-IBD is important for developing effective therapies and implementing appropriate surveillance and monitoring strategies for high-risk patients.

Summary: This study conducted a systematic review and meta-analysis to compare the survival outcomes of IBD-associated and non-IBD-associated CRC. The results showed that IBD-CRC patients had a significantly worse overall survival than those without IBD. This may be attributed to more aggressive histological characteristics and a lower rate of R0 resections at the primary tumor site.

Summary: The study systematically identified ultrasound scoring indices for assessing inflammatory bowel disease activity, with common components including bowel wall thickness, colour Doppler imaging, and bowel wall stratification. The correlation between ultrasound indices and reference standards, as well as sensitivity, specificity, accuracy, and other properties, varied within certain ranges. However, reliability and responsiveness data were limited, and most studies were rated with unclear or high risk of bias.

Summary: NK cells from patients with active IBD exhibit an unbalanced metabolic profile, with reduced mitochondrial function and diminished killing capacity. These metabolic abnormalities may contribute to defective killing, leading to secondary infections and increased cancer risk in IBD patients.

Summary: The study investigated the association between SNPs in smad7, TLR2, and TLR4 and the development of UC-associated colorectal cancer in 90 patients with UC. The results showed that none of the eight SNPs in smad7, TLR2, or TLR4 were associated with the development of UC-associated colorectal cancer at allelic or genotypic level.

Summary: Patients with IBD have reduced immune responses following vaccination, especially when receiving anti-TNF therapy which may decrease antibody titres to COVID-19 vaccines. It is recommended that IBD patients be vaccinated at the earliest opportunity, ideally in remission with minimized corticosteroid dose before vaccination. Further research is needed to explore the impact of different biologics on vaccine response to COVID-19 in the IBD population and the potential for booster vaccines or heterologous prime-boost vaccinations.

Summary: The study identified a significant enrichment of FimH-expressing bacteria in the gut microbiome of CD patients, with FimH playing a crucial role in adhesion and inflammation induction. The specific FimH-blocker TAK-018 was shown to prevent bacterial adhesion and inflammation, potentially offering a promising treatment option for CD patients.

Summary: Patients with inflammatory bowel disease (IBD) have attenuated antibody responses to COVID-19 vaccination, and the use of TNF antagonists exacerbates this effect. Patients who do not seroconvert following vaccination are particularly vulnerable. Our study highlights the importance of booster vaccination programs for patients with IBD.

Summary: In the Irish National CRC Screening Programme, a quarter of participants did not perceive personal risk of cancer, did not view CRC as a serious disease, and did not see the benefits of screening. Roughly one-fifth found screening to be stressful, with fear of cancer diagnosis and test results being key stressors. Strategies to increase uptake in round 2 include greater use of media and involving healthcare professionals in disseminating information on the benefits of screening.
